[Rapid destructive arthropathy of the shoulder].

Within the general context of rapid destruction of the humeral head, destructive arthropathy of the shoulder, described here in six cases, is a diagnosis of elimination. Being neither infectious inflammatory, microcrystalline, nor neurological, this curious variety of degenerative pathology of the shoulder involves the following: 1) a particular group of sufferers: women aged 65 to 81 years; 2) prior signs, at least radiological, of deterioration in the rotator cuff; 3) rapid erosive osteolysis of the head of the humerus reducing its radiological area by 25 per cent in less than six months; 4)…

A variant of mirror hand. A case report.

We describe a rare variant of mirror hand in a 20-year-old man who presented with multiple fingers. Radiographs revealed two ulnae (one vestigial) and a radius. There was duplication of the humeral head. The unique features of this case are the age of patient before the start of treatment and extension of the duplication proximal to the elbow.

Posterior circumflex humeral artery pathological lesions with digital ischemia in an elite volleyball player: A case report and literature review.

The posterior circumflex humeral artery, a branch of the axillary artery, is compressed by the humeral head during repeated abduction and external rotation of the shoulder joint owing to its anatomical structure. This damages the vascular endothelium, resulting in thrombi, arterial dissection, and aneurysms, a condition known as posterior, circumflex humeral artery pathological lesions. A thrombus may form at the site and becomes a peripheral embolus, resulting in peripheral arterial occlusion.A 21-year-old right-handed elite man college volleyball player noticed coldness and pain in his right…

Glenohumeral and scapulohumeral kinematic analysis of patients with traumatic anterior instability wearing a shoulder brace: a prospective laboratory study.

PURPOSE: To assess the effectiveness of a novel glenohumeral joint immobilizer, the S2 Shoulder Stabilizer®, by evaluating shoulder kinemati … antly lower (p = 0.0022) mediolateral scapular rotation angle in the limbs wearing the immobilizer. Humeral head translation during abduction-adduction and internal-external rotation was also lower in the patients wearing the brace. CONCLUSIONS: The immobilizer significantly limited joint excursion in all planes of movement except internal rotation. The narro …

Fracture dislocation of the glenoid fossa with open physis: a case report.

BACKGROUND: Scapular fractures are uncommon injuries that account for 1% of all fractures. Ten percent of these lesions involve the glenoid fossa and intraarticular displacement is rare, not exceeding 10% of the cases. The classification of intraarticular glenoid fractures was described by Ideberg on the basis of his findings in 100 patients with this type of injury. His classification, however, does not include a dislocation of the glenoid fossa without displacement of the humeral head. METHODS: We report the case of a fracture dislocation of the glenoid fossa without displacement of the hum…

Arthroscopic repair of posterior instability and reverse humeral glenohumeral ligament avulsion lesions.

Posterior glenohumeral subluxation is not as uncommon as once believed. Unidirectional instability as a result of a traumatic event or the primary direction of multidirectional instability as a result of overuse can lead to recurrent involuntary subluxations. Failure of the posterior restraints can occur at the glenoid labral attachment, mid-capsule, and laterally, as in a RHAGL avulsion. Soft tissue reattachment to bone is by way of suture anchors. This may occur at the glenoid medially or laterally along the humeral head insertion (RHAGL lesion). Capsular plication and superior shift can obl…

Neglected traumatic posterior dislocations of the shoulder: controversies on indications for treatment and new CT scan findings.

We report seven missed traumatic posterior dislocations of the shoulder in six patients. In all seven shoulders, anteroposterior radiographs did not provide the diagnosis in the emergency room after the trauma (epileptic seizure in four patients) and diagnosis was delayed for at least 24 h. The final diagnosis was reached by clinical suspicion and computed tomography (CT) scan of the seven shoulders. Two shoulders were treated by closed reduction and three were treated by closed reduction and percutaneous pinning, although four of these five shoulders presented a defect in the humeral head inv…

Effect of a chondral-labral defect on glenoid concavity and glenohumeral stability. A cadaveric model.

One of the primary stabilizing mechanisms of the glenohumeral joint is concavity-compression, the maintenance of the humeral head in the concave glenoid fossa by the compressive force generated by the surrounding muscles. This mechanism is active in all glenohumeral positions but it is particularly important in the functional mid-range, in which the capsule and ligaments are slack. The effectiveness of concavity-compression in the stabilization of a joint can be characterized in terms of the ratio between the maximum dislocating force that can be stabilized in a given direction and the load co…

Shoulder and Elbow Injuries in Adult Overhead Throwers: Imaging Review.

Overhead throwing, particularly in baseball, subjects the shoulder and elbow to various unique injuries. Capsular contracture following repetitive external rotation shifts the humeral head posterosuperiorly, predisposing to glenohumeral internal rotation deficit (GIRD), Bennett, posterosuperior internal impingement (PSI), and superior labrum anterior-posterior (SLAP) lesions. GIRD represents loss of internal rotation at the expense of external rotation. Bennett lesion represents ossification of the posteroinferior glenohumeral ligament due to repetitive traction. PSI manifests with humeral hea…

Anatomical reduction and fixation of reverse Hill-Sachs lesion: description of a surgical technique.

Reverse Hill-Sachs lesions pose a complex challenge in the acute management of posterior shoulder dislocations, often resulting in joint incongruity and compromised function. This article describes a surgical technique for the anatomical reduction and fixation of humeral head impaction injuries, aiming to restore the native sphericity of the articular surface and optimize joint stability. The proposed technique enables the precise realignment of the humeral cartilage and subchondral bone, thereby minimizing residual deformity and preserving the biomechanical integrity of the glenohumeral joint…

Percutaneous fixation of valgus displaced fracture of the proximal humerus using a single screw.

Valgus-impacted proximal humerus fracture is a classic but rare entity in shoulder traumatology. Surgical treatment is controversial, with increasing use of minimally invasive techniques. Our technique uses a minimally invasive approach under fluoroscopic control. Raising the humeral head to reduce the valgus enables spontaneous and well-positioned reduction of the tuberosities and screw fixation between the greater tuberosity and the humeral shaft. Indications comprise valgus-impacted fracture without comminution of the medial epiphyseal-metaphyseal hinge or greater tuberosity; the rotator cu…

Assessment and treatment of glenohumeral joint deformities in children suffering from upper obstetric brachial plexus palsy.

We present our approach to gleno-humeral joint deformities as sequelae from severe upper obstetric brachial plexus palsy. In 50 consecutive children with severe medial rotation contracture of the shoulder after obstetric brachial plexus palsy, we used magnetic resonance imaging to evaluate joint incongruence and dysplasia; showing frequently various deformities of the glenoid, the humeral head and pathologic changes in their relationship. The most severe deformity is true glenohumeral dysplasia. These diagnostic findings might influence our choice and technical details within surgical procedur…

Use of 3D Planning and Patient-specific Guides for Proximal Humerus Corrective Osteotomy Associated With Shoulder Prosthesis Implantation in Proximal Humeral Varus Malunion.

Humeral stem prosthesis implantation in case of proximal humerus varus malunion (type 1D fracture sequelae) is often complicated by greater tuberosity fracture and by posterosuperior rotator cuff iatrogenic damage. Moreover, the varus malunited humeral head could lead to scapular impingement and reduce the range of motion. To address this problem, we introduced a new surgical procedure consisting in a proximal humerus osteotomy, planned with three-dimensional (3D) preoperative virtual surgery, and performed with patient-specific surgical guides, to correct humerus deformity before the implanta…

Posterior circumflex humeral artery injury with distal embolisation in professional volleyball players: a discussion of three cases.

Presented are three cases of volleyball players with ischaemia of the hand due to arterial emboli originating from an injured posterior circumflex humeral artery (PCHA). An operative treatment with ligation of the PCHA was performed in all patients because of the proximity of the occlusion to the axillary artery. After a rehabilitation programme, all patients could return to their previous level of competition. During overhead motion, the PCHA is prone to injury in its position overlying the humeral head and its course through the quadrilateral space. Recognition of the vascular origin of thes…
