[Central suspension operation of hyoid and lower jaw for OSAS].

OBJECTIVE: To evaluate the prospective effect and minimize the postoperative trauma of applying the central suspension operation of hyoid and lower jaw for the treatment for OSAS in 28 cases. METHOD: The data of 28 patients with obstructive sleep apnea syndrome(OSAS) operated with central suspension operation of hyoid and lower jaw and uvulopalatopharyngoplasty(UPPP) were analyzed. RESULT: One case (3.6%) reported unchanged snoring and apnea and hyponea index. Twenty-seven cases (96.4%) improved after the operation for at least 6 months to 3 years. The complications include tonsil bleeding (

Extended supracricoid partial laryngectomy with tracheocricohyoidoepiglottopexy.

Extended supracricoid partial laryngectomy with tracheocricohyoidoepiglottopexy (TCHEP) was studied as an alternative to total laryngectomy in 16 patients with glottic carcinoma presenting a 10-15 mm of anterior subglottic extent. The technique of the procedure is described. Results were analyzed for tumor free margins, tracheostomy decannulation, oral alimentation, phonation and disease free interval. The 3-year survival and local control rate were 68% and 86.7%, respectively. Our preliminary data suggest that the TCHEP procedure is a viable alternative to total laryngectomy for patients pres

The position of the larynx in children and its relationship to the ease of intubation.

Lateral radiographs of the upper airway of children aged from birth to puberty were used to determine the changes in position, with growth, of the tip of the epiglottis, the hyoid, the glottis, and the inferior margin of the cricoid cartilage. There is a marked descent of these structures relative to the cervical spine during the first two years of life. Their position then remains fairly static until puberty, when further descent of laryngeal structures occurs. Using the same methods to determine ease of laryngoscopy with the head and neck in different positions it was shown that in infants t

The value of micro-CT imaging in the forensic evaluation of neck trauma.

AIM: Forensic examination of the hyoid-larynx complex (HLC) is crucial in diagnosing (inflicted) neck trauma. Clinical Total-Body CT (TBCT) scans of the (non-)calcified HLC lack sensitivity for fracture detection. Micro-computed tomography (micro-CT) is an upcoming imaging modality capable of scanning samples at microscale resolution up to 1 µm. Soft-tissue contrast can be established with iodine staining, known as diffusible iodine-based contrast-enhanced CT (diceCT). We study the additional value of micro- and diceCT in detecting HLC fractures and hemorrhages in forensic cases, compared to s

Post-mortem imaging of laryngohyoid fractures in strangulation incidents: first results.

Fractures and soft-tissue injuries of the neck are of great importance in forensic pathology, as they help in assessing whether strangulation took place, and if so, how severely. In this study, we examined the usefulness of post-mortem imaging with multislice computed tomography (MSCT) in detecting lesions of the laryngohyoid structures and the surrounding soft-tissues. For this purpose, we examined MSCT images of the neck of eight deceased persons who had suffered different types of strangulation and compared the findings with those obtained at the subsequent forensic autopsy. In six of the e

The relationship between craniofacial anatomy and obstructive sleep apnoea: a case-controlled study.

The aim of the study was to identify craniofacial and pharyngeal anatomical factors directly related to obstructive sleep apnoea (OSA). The … s increased (P = 0.021), soft plate length, thickness and area were all greater (P < 0.001) and the hyoid bone was more inferiorly positioned in OSA subjects (P < 0.001). This study identifies a significant number of craniofacial and pharyngeal anatomical factors directly related to OSA.

Perinatal lethality and multiple craniofacial malformations in MSX2 transgenic mice.

MSX2 is a homeodomain transcription factor that has been implicated in craniofacial morphogenesis on the basis of its expression pattern during mouse development and the finding of a missense mutation (P148H) in humans affected with Boston-type craniosynostosis. We have generated transgenic mice carrying a 34 kb DNA fragment encompassing a human MSX2 gene encoding either wild-type or mutant (P148H) MSX2. Inheritance of either transgene resulted in perinatal lethality and multiple craniofacial malformations of varying severity, including mandibular hypoplasia, cleft secondary palate, exencephal

Complications of cleft palate repair in large animals.

Seven foals, 2 horses, and 2 calves were presented to the George D. Widener Hospital for Large Animals at New Bolton Center and the Large Animal Clinic at Auburn University for surgical repair of cleft palate. All animals had preexisting pneumonia. Initial repair of cleft palate in 6 foals, 2 horses, and 2 calves was performed via mandibular symphysiotomy. Primary repair of cleft soft palate in a foal was attempted through a pharyngotomy incision after fracture of the basal hyoid bone; however, exposure was inadequate and mandibular symphysiotomy had to be done. Dehiscence of a portion of the …

Thyroglossal duct cyst: the New York Eye and Ear Infirmary experience and a literature review.

Thyroglossal duct cysts often present in childhood but can also afflict the adult population. In 1920, Sistrunk described surgical management and advocated the removal of the central portion of the hyoid bone, following the cyst tract to the base of the tongue. This surgical technique has not changed since its description 60 years ago. In this paper, a retrospective review of 70 thyroglossal duct cyst excisions performed at the New York Eye and Ear Infirmary from 1988 through 1996 is presented. The patient population consisted of 43 females (61%) and 27 males (39%). The average age at presenta…

Legal issues associated with PCP abuse--the role of the forensic expert.

These cases illustrate some of the complex issues associated with PCP-related litigation. The concept that malice is implied when an experienced drug user commits a crime while under the influence of the drug is not held in most states, at the present time. The authors have now reviewed in detail four cases of unexpected death following the use of neck holds in PCP-intoxicated individuals. In all of the cases, multiple carotid compression holds had been attempted, according to the history. Skin abrasions, hemorrhage into the soft tissues of the neck, and fractures of the hyoid bone and thyroid…
