Early efficacy and complications of releasable sutures for trabeculectomy in primary angle-closure glaucoma: a randomized clinical trial.

PURPOSE: To compare the postoperative intraocular pressure (IOP) and incidence of early complications after trabeculectomy with releasable s … e in the occurrence of shallow chamber, choroidal detachment, macular edema, additional surgery, or hyphema (P=0.56 to 1.0). CONCLUSIONS: The technique of releasable sutures for trabeculectomy used in this study did not demonstrate significant advantages over standard trabeculectomy. Releasable sutures were associated with some decrease in visual acuity …

23-gauge vitrectomy with intraocular foreign body removal via the limbus: an alternative approach for select cases.

PURPOSE: To study the outcome of removal of retained intraocular foreign bodies (RIOFBs) via limbus using 23-gauge transconjunctival suturel … vement. There were no intraoperative complications. Postoperative complications include microscopic hyphema and loose blood in vitreous cavity in one eye. CONCLUSION: The advantages of 23-gauge TSV for removal of RIOFB may be passed on to select cases. RIOFB removal through limbal route obviated the need for scleral port enlargement and preserved capsula …

Predictors and outcomes of ocular hypertension after open-globe injury.

PURPOSE: Evaluate predictors and outcomes of ocular hypertension after open-globe injury. PATIENTS AND METHODS: This is a retrospective, ca … r hypertension defined as IOP≥22 mm Hg at >1 visit or requiring treatment. Increased age (P<0.001), hyphema (0.025), lens injury (P<0.0001), and zone II injury (P=0.0254) are risk factors for developing ocular hypertension after open-globe injury. Forty-eight (74%) patients with ocular hypertension were treated medically, 8 (12%) underwent filtering or gl …

Ex-PRESS implantation versus trabeculectomy in uncontrolled glaucoma: a meta-analysis.

OBJECTIVE: To evaluate the efficacy and tolerability of Ex-PRESS implantation (Ex-Press) compared with trabeculectomy (Trab) in the treatmen … ualified success rate. Ex-Press was associated with a significantly lower frequency of hypotony and hyphema than Trab, with pooled ORs of 0.29 (0.13, 0.65) and 0.36 (0.13, 0.97), respectively. CONCLUSION: Ex-Press was associated with equivalent efficacy to Trab in lowering IOP. Comparable proportions of patients reached the IOP target with Ex-Press and T …

OBJECTIVE: To evaluate the early therapeutic effect of endoscope-assisted goniosynechialysis combined with phacoemulsification and intraocul … e intraocular pressure, visual acuity, anterior chamber depth and anterior chamber angle. RESULTS: Hyphema occurred in 3 eyes after the operation and was absorbed gradually. The intraocular pressure decreased significantly in all the patients after the operation (P<0.05) but showed no significant differences between the acute and chronic cases (P>0.05). …

Transscleral diode laser therapy for traumatic cyclodialysis cleft.

This case report describes a new surgical approach for the treatment of cyclodialysis. The treatment of cyclodialysis cleft associated with hypotony is in a period of evolution. In this report, a patient with traumatic cyclodialysis associated with hypotony maculopathy was treated with contact transscleral diode laser therapy to the cleft areas. The contact diode laser therapy resulted in closure of the clefts, reversal of hypotony, and resolution of maculopathy with improvement in vision. The results of this study show that this technique can be less invasive than other surgical options.

Ocular injuries--treat or refer?

The primary care physician is often called on to treat ocular emergencies. Two such injuries --chemical burns and traumatic central retinal artery occlusion --call for quick action and immediate referral. The evaluation of other injuries should proceed in logical order, beginning with the lids and proceeding to the pupils, globe, and retina and optic nerve. This systematic evaluation allows the examiner to identify patients who need referral to an ophthalmologist. Minor injuries can be safely treated by following the guidelines given.

Complications of implant surgery in children.

Rapid recognition and appropriate therapy will prevent or remediate most of the complications occurring in children after cataract aspiration and IOL implantation. Some of the complications of pediatric IOL implantations are related to the increased scleral pliability and decreased rigidity which predispose scleral collapse, vitreous loss, flat anterior chamber and corneal endothelial damage. A second group of complications is associated with the enhanced inflammatory and fibrotic responses peculiar to a child's eye. This group includes operative striate keratitis and iridocyclitis, late IOL p
