Common variable immunodeficiency (CVID) and selective IgA deficiency (SIGAD) are the most common primary antibody deficiencies. These two diseases may have coincidence in one family and SIGAD can progress to CVID which suggest common underlying genetic defects between SIGAD and CVID. This study was designed to find the prevalence of multiple cases in families of Iranian patients with CVID or SIGAD.Serum samples were collected from all available first-degree relatives of 37 patients (23 patients with CVID and 14 with SIGAD) to check the levels of immunoglobulin and their subclasses and detect a…

X-linked hyperimmunoglobulin M (HIGM) syndrome may increase the susceptibility of patients to disseminated cryptococcal infections primarily due to CD40L deficiency that causes defective cross talk between T- and B-cells, thus preventing class switching. In HIGM syndrome, serum IgM levels are elevated with severe reduction in serum immunoglobulin G (IgG) and IgA levels. In addition, the expression of CD40L (CD154) on in vitro-activated T-cells is severely reduced or absent. Here, we describe a rare, and perhaps, the first reported case in India of a 3-year-old male child with X-linked HIGM imm

Coeliac disease is a common cause of chronic diarrhea in children and adults. It is also frequently detected in children exclusively affected by iron deficiency anemia, hypocalcemia, short stature, dental enamel defects, epilepsy and intracranial calcifications, etc. The coeliac disease diagnosis may be facilitated by the use of some immunological tests like anti endomysial (AEA) or anti gliadin (AGA) antibodies detection. From December 1990 to September 1992 anti endomysial IgA and anti gliadin IgG antibodies were respectively detected in 1680 and 1598 sera from children and adults affected b

Hyper IgM syndrome is a well known genetic (primary) immunodeficiency disorder which was first described in 1961. It is caused by B lymphocyte deficiency characterized by normal or elevated serum IgM levels and low or zero levels of IgG, IgA, IgE resulting from isotype-switching deficiency. Clinical manifestations are dominated by recurrent infections, especially involving the digestive tube of the ENT sphere and the lungs. This syndrome is caused by B-cell immunoglobulin class switch deficiency and decreased capacity to induce proliferation of T lymphocytes. The net result of these deficienci

Immunoglobulin class switch recombination (CSR) deficiencies are rare primary immunodeficiencies, characterized by a lack of switched isotype (IgG, IgA, or IgE) production, variably associated with abnormal somatic hypermutation (SHM). Deficiencies in CD40 ligand, CD40, activation-induced cytidine deaminase, and uracil-N-glycosylase may account for this syndrome. We previously described another Ig CSR deficiency condition, characterized by a defect in CSR downstream of the generation of double-stranded DNA breaks in switch (S) mu regions. Further analysis performed with the cells of five affec

The increasing incidence of primary brain lymphoma appears to be real in immunologically normal people. However, lymphoma of the central nervous system has a much higher incidence in patients with immune deficient status as in renal and cardiac transplants, patients with IgA deficiency, or Wiskott-Aldrich syndrome. Thus, the increased predisposition of AIDS patients to brain lymphoma is not surprising, this being the second most frequent cause among cerebral mass lesions in the AIDS population. We report a case of a 36 year old white woman with AIDS and a cerebral mass lesion which we demonstr…

OBJECTIVES: To evaluate the serum biochemical levels in celiac disease (CD) patients.
METHODS: It was a cross-sectional study carried out on 70 subjects, including 40 patients with CD and 30 healthy controls. This study was conducted at Jouf University from November, 2020 to October, 2021. The collected blood specimens were used to perform serum iron, serum lipids, liver enzymes, and human tissue transglutaminase IgA antibodies (anti-HTTG). The hematological parameters including hematocrit and MCV were determined to establish the diagnosis of iron deficiency.
RESULTS: Serum iron was signific

It is now generally believed that subclinical coeliac disease is common in the general population. We have undertaken screening for this dis … opsy. Coeliac disease was diagnosed in 11 subjects, most of whom had no serious symptoms. Selective IgA deficiency was found in 4 subjects, 1 of whom also had coeliac disease. The prevalence of subclinical coeliac disease in the study group was 3.28 per 1000. Coeliac disease screening is feasible and involves only slight discomfort to the general population. Suc …

Immunoglobulin deficiency with increased IgM (hyper-IgM recurrent infection syndrome) is thought to be due to a defect of IgM/IgG isotype switching in B cells. Considerable genetic heterogeneity (X-linked, autosomal dominant or autosomal recessive inheritance, acquired forms) exists. We describe a case of hyper-IgM syndrome associated with massive lymphadenopathy in a 10-year-old girl with normal B-cell function. Absolute and relative increases of T-suppressor cells (CD8+) and a decrease of circulating CD4+ ("helper") cells were observed. The CD4+ cells present were all of the CD45R+ subtype,

Fifty-six tuberculosis patients and twenty-eight control subjects were evaluated in a comprehensive investigation of cellular and humoral immune function in pulmonary TB. The patient group showed significantly higher levels of secretory IgA and serum IgG, IgA and IgM than did the control group but 7% of patients displayed a selective secretory IgA deficiency. Levels of alpha-1-antitrypsin were also significantly higher in the patient group. There were no significant differences in levels of total haemolytic complement, C'3 and C'4. In moderate to moderately advanced TB patients there were no s…

A panel of previously characterized monoclonal antibodies: OKT3, OKT4, OKT8, OKT10, OKT11, OKIa1, OKM2; 3A1, 4F2, UCTH1 and 5/9 were used to evaluate peripheral blood mononuclear cells in patients with severe primary immunodeficiencies: three patients with severe combined immunodeficiency, five with X-linked agammaglobulinemia, 20 with common variable hypogammaglobulinemia, 11 with IgA defect, and one with an unclassified form of T cell defect and hypogammaglobulinemia. Surface markers for T and B cells and in some cases functional assays, were also performed. Our results indicate a heterogene

Selective immunoglobulin M deficiency (SIgMD) is a rare pediatric antibody defect with poorly defined thresholds for immunoglobulin replacement therapy (IgRT). We retrospectively reviewed 5,122 serum IgM measurements (2005–2024) and identified 21 children meeting SIgMD criteria (IgM < 20 mg/dL with normal IgG/IgA). Nine had isolated SIgMD, while 12 presented with comorbid conditions. Recurrent tonsillopharyngitis was more frequent in isolated cases, whereas low isohaemagglutinin titres and lymphopenia were confined to the comorbidity group. IgRT was initiated in 11 patients, predominantly thos

Serum IgG subclasses and IgG, IgA and IgM were measured in 35 patients with frequent infections and unusually severe infections, 45 normal children were compared. There is no significant difference between the geometric means of two age-matched groups (p greater than 0.05) and the proportion of low IgG subclass. We detected 9 cases with low IgG subclass in our selected patients, but we have 6 cases with low IgG subclass in our normal population without symptoms. For the low IgG subclass cases, total IgG level remain at normal age-related values, no IgA deficiency was found in our 78 cases. It …

This report concerns an immune deficiency disorder in a girl with eczema. She has had recurrent infections including three severe attacks of herpes simplex and five attacks of pneumococcal meningitis. There is a moderate lymphopenia, dysgammaglobulinaemia with high IgG, high IgA, and low IgM; lymphocyte transformation with phytohaemagglutinin is impaired. Production of circulating antibody is abnormal, as are delayed hypersensitivity reactions. Although there is no thrombocytopenia, the resemblance to the Wiskott-Aldrich syndrome is discussed.

The study involves 10 children with primary immunologic deficiency of the humoral and cellular immunity diagnosed on the basis of the clinical symptoms of the disease and immunologic investigation. In 7 children, out of whome three were infants, suffering of recurrent respiratory tract infections, examinations revealed either absence or deficiency of the IgA in the serum and saliva. 2 children presented a classical picture of the Wiskott-Aldrich's syndrome followed by eczema, recurrent infections and trombocytopenia. Having studied the immunologic status in these two children in vitro and in v

Vitamin A and D deficiencies and insufficiencies are prevalent worldwide in developed and developing countries. Vitamin metabolites are functionally intertwined in that they are high-affinity ligands for related receptors of the nuclear receptor superfamily. The effects of vitamin A deficiencies (VAD) on antibody responses to respiratory virus vaccines have already been demonstrated. Of particular concern was the reduction in IgA, a first line of defense against pathogens in the respiratory tract. Here, we describe the individual and combined effects of vitamin A and D deficiencies in mice imm

IgG subclass deficiencies can increase the risk of pulmonary infections or other severe recurrent infections. Various groups of patients were studied in an attempt to learn more about (1) measuring the IgG subclass levels (particularly IgG4 levels) of possible high-risk patients; (2) identifying those patients who are at risk of developing recurrent infections, and (3) treating these patients once they have been identified. Individual cases are presented, and associations of isolated subclass deficiencies are examined with respect to infection. Replacement therapy with plasma or intravenous im

We describe an interesting case of severe asthma who was not showing satisfactory response to standard treatment. Investigations revealed him to be suffering from allergic bronchopulmonary aspergillosis (ABPA). After starting systemic steroids he showed marked improvement initially only to have recurrent symptoms within a year. He was investigated further and found to have chronic pulmonary aspergillosis in the form of chronic cavitary pulmonary aspergillosis (CCPA) and aspergilloma as also the presence of selective IgA deficiency.
