Gastric bypass in patients with BMI < 40 but > 32 without life-threatening co-morbidities: preliminary report.

BACKGROUND: Surgical intervention is currently indicated for patients with BMI > 40 or > 35 with life-threatening comorbidities. Patients with BMI 32-40 without these comorbidities not only have the increased propensity to develop them but also suffer from similar psychosocioeconomic consequences. These patients may not respond to non-surgical treatment of obesity any better than those with BMI > 40. The question has been raised whether to offer them surgical intervention. METHODS: A study was carried out to determine outcome of surgery on patients with BMI > 32 but < 40 without life-threaten

Long-term outcomes of elective human sperm cryostorage.

BACKGROUND: Sperm cryopreservation allows men with threatened fertility to preserve their progenitive potential, but there is little data on long-term outcomes of elective sperm cryostorage programmes. METHODS AND RESULTS: Over 22 years, 930 men sought semen cryostorage in a single academic hospital, of which 833 (90%) had spermatozoa cryostored. Among 692 (74%) men surviving their illness, sperm samples were discarded for 193 (21% of all applicants, 28% of survivors) and cryostored spermatozoa were used for 64 men (7% of all applicants, 9% of survivors) in 85 treatment cycles commencing at a

Acute viral respiratory infections in children before and during the COVID-19 pandemic in a Peruvian reference hospital.

INTRODUCTION AND BACKGROUND: Invasive home mechanical ventilation is used for patients with chronic respiratory insufficiency. This elaborate and technology-dependent ventilation is carried out via an artificial airway (tracheal cannula) to the trachea. Exact numbers about the incidence of home mechanical ventilation are not available. Patients with neuromuscular diseases represent a large portion of it. RESEARCH QUESTIONS: Specific research questions are formulated and answered concerning the dimensions of medicine/nursing, economics, social, ethical and legal aspects. Beyond the technical a

Real-world treatment patterns and management gaps of lumbar disc herniation in the United States.

BACKGROUND: Lumbar disc herniation (LDH) affects approximately 1% to 3% of the population annually and leads to substantial physical burden, quality-of-life burden, and productivity loss. Commonly used interventions, including pharmacological and epidural steroid injections (ESIs), have limited high-quality evidence to support their effectiveness in the long-term for treating LDH beyond symptom relief. In general, there is a lack of consensus for timing of treatment after LDH onset and limited data on real-world treatment of LDH. The objective of this study was to describe current real-world t

Recovery and Return to Sport in Pediatric Osteochondral Defects of the Knee: Evaluating the Influence of Lesion Size.

BACKGROUND: Osteochondral defects (OCDe) of the knee in pediatric patients can significantly impact long-term joint function. This study aimed to evaluate the impact of lesion size on patient characteristics, treatment decisions, and outcomes. HYPOTHESIS: We hypothesized that larger lesions would be associated with worse outcomes and delayed return-to-sport (RTS), and that surgical modalities would vary by lesion size. METHODS: This IRB-approved retrospective study included 112 patients surgically treated for knee OCDe between January 2012 and December 2023 at an academic tertiary care cente

Real-world longitudinal treatment trajectories following opioid use disorder diagnosis among commercially insured beneficiaries in the United States.

INTRODUCTION: Early initiation and continuity of treatment are critical for the effective management of opioid use disorder (OUD). Despite the availability of effective medications for OUD (MOUD), many individuals never initiate treatment, and those who do often discontinue early. A better understanding of real-world treatment patterns is needed to inform strategies for improved long-term engagement in care. This study uses a novel technique to characterize and visualize real-world treatment trajectories following an OUD diagnosis among commercially insured individuals. METHODS: Retrospective

Economic Evaluation of Lecanemab for Early Symptomatic Alzheimer's Disease in South Korea.

BACKGROUND/OBJECTIVES: Alzheimer's disease (AD) exerts a considerable economic burden on South Korea's aging population. Lecanemab, an amyloid-targeting therapy, has demonstrated efficacy in mitigating cognitive decline in early-stage AD but remains non-reimbursed in South Korea due to concerns over its economic feasibility. This study aimed to examine the cost-effectiveness of lecanemab using nationwide claims data for cost estimation within the South Korean healthcare system. Considering the substantial societal burden of AD, we also evaluated the cost-effectiveness of lecanemab from a limit

Reasons for switching oral antipsychotic medications and related patterns of care and costs in patients with schizophrenia initiating monotherapy treatment: Claims-linked chart study.

BACKGROUND: Antipsychotic switching is frequent in schizophrenia and is associated with poor clinical outcomes, increased health care resource utilization (HCRU), and increased health care costs. Research describing the reasons for antipsychotic switching in patients with schizophrenia and the associated impacts on HCRU and costs is limited. OBJECTIVE: To explore the reasons for oral antipsychotic medication (OAM) switching and describe HCRU and costs associated with OAM switching, stratified by reasons for switching, in patients with commercial or Medicare Advantage insurance in the United S

Survival disparities in primary malignant spine tumors: a national analysis of academic and community hospital care.

OBJECTIVE: Primary malignant osseous tumors of the mobile spine are rare and require complex, multidisciplinary care. Treatment outcomes may differ between academic and community hospitals due to variations in resources, expertise, and infrastructure. This study evaluates demographics, care delivery patterns, and clinical outcomes, including both short- and long-term survival, for patients treated at academic versus community cancer programs using the National Cancer Database. METHODS: A retrospective analysis of a prospectively maintained database was performed. Nine hundred forty-three pati

Predicting obstetric anal sphincter injuries in a modern obstetric population.

BACKGROUND: Perineal lacerations are common at the time of vaginal delivery and may predispose patients to long-term pelvic floor disorders, such as urinary incontinence and pelvic organ prolapse. Obstetric anal sphincter injuries, which are the most severe form of perineal lacerations, result in disruption of the anal sphincter and, in some cases, the rectal mucosa during vaginal delivery. Long-term morbidity, including pain, pelvic floor disorders, fecal incontinence, and predisposition to recurrent injury at subsequent delivery may result. Despite several studies that have reported risk fac

Perceptions of Diseases of Despair by Members of Rural and Urban High-Prevalence Communities: A Qualitative Study.

IMPORTANCE: Diseases of despair (ie, mortality or morbidity from suicidality, drug abuse, and alcoholism) were first characterized as increasing in rural White working-class populations in midlife with low educational attainment and associated with long-term economic decline. Excess mortality now appears to be associated with working-class citizens across demographic and geographic boundaries, but no known qualitative studies have engaged residents of rural and urban locales with high prevalence of diseases of despair to learn their perspectives. OBJECTIVE: To explore perceptions about despai
