Obstetric fistula is the presence of a hole between a woman's genital tract and either the urinary or the intestinal tract. Better knowledge of the risk factors for obstetric fistula could help in preventing its occurrence. The purpose of this study was to assess the characteristics of obstetric fistula patients. We conducted a search of the literature to identify all relevant articles published during the period from 1987-2008. Among the 19 selected studies, 15 were reports from sub-Saharan Africa and 4 from the Middle East. Among the reported fistula cases, 79.4% to 100% were obstetrical whi

The present study examines peptide YY responses to regional intestinal perfusion of fatty acids of different chain length--dodecanoate and oleate. Six dogs with chronic gastric, duodenal, and jejunal fistulas were studied. Proximal perfusates were administered into the duodenum and diverted through an intestinal fistula placed 45 cm beyond the duodenal cannula. Distal perfusates were administered into the caudal stoma of this intestinal stoma. Peptide YY responses to proximal, distal, and whole-gut perfusion were compared. Proximal perfusion with oleate or dodecanoate failed to release peptide…

The diagnosis of ileus caused by biliary stones occurs very rarely, with the range of 2 % worldwide. This complication of cholecystolithiasis caused by the stone fistulation into gastrointestinal tract and its subsequent obstruction occurs mostly in elderly and has a high mortality rate. During the course of ten years (1996-2006) in the 2nd surgical clinic FNLP in Kosice, 1640 cholecystectomies and 255 operations, due to the obstruction ileus, were performed. Biliary ileus was recorded in four cases. In two cases, the reason being an obstruction caused by a travelled stone into jejunum, one ev

Three patients with cholecystoduodenal fistulae due to gallstone penetration, resulting in major gastrointestinal haemorrhage are presented. Diagnosis and treatment of this condition are discussed.

Feeding by tube enterostomy is safe, economical and well tolerated by most patients. It will ameliorate or reverse the catabolic effects of protein-calorie malnutrition. Feeding tube enterostomy is indicated in patients with the combined findings: a need for long term nutritional therapy; inability to ingest adequate nutrients by mouth, and a normally functioning gastrointestinal tract. These feedings are most efficaciously delivered into the stomach and jejunum and the surgeon must be thoroughly familiar with the differences in commercial formulations prior to their use.

Formation of a fistula between the gallbladder and bowel may allow a gallstone to enter the intestinal tract. In the two cases described here, the colon was involved. The authors describe diagnosis and management of these cases and make treatment recommendations based on their experience.

BACKGROUND: As the number of patients with inflammatory bowel disease (IBD) increases, the incidence of IBD-related colorectal cancer (CRC) is also on the rise. Crohn's disease (CD)-related CRC has been reported to have a poorer prognosis than sporadic CRC, and the early detection of CD-related CRC is difficult. Japanese patients with CD are reported to have a higher frequency of anorectal cancer than the Western population; however, methods for early diagnosis have not yet been established because of perianal pain during the examination.
CASE PRESENTATION: We report a case of CD-related anal

During the period 1978 to 1991, 19 patients were treated for fistula of the urinary bladder, 12 of the fistulas were enterovesical, four vesicovaginal, one vesicouterine and two vesicocutaneous. Most of them occurred after diverticulitis or operation, or when the patient suffered from a malignant disease. Four patients received conservative treatment. One patient decided against an operation, two were inoperable, and one fistula closed with use of an indwelling catheter. 15 were operated on. 12 extirpations of fistula were carried out. Additional resection of intestine was performed in seven p

Indications, technique and prognosis of 129 enterostomies in Crohn's disease are reported. Advanced perianal or entero-genital fistulas represented the predominant indication (60% of primary, 46% of repeated stomas). 85% of stomas were created in combination with intestinal resections. Preferred type of stoma was a loopileostomy (76% of primary stomas), whereas colostomies were avoided whenever possible because of increased complications. Patients initially presenting with rectal involvement or perianal fistulas were prone to need a stoma during the course of their disease while intraabdominal

BACKGROUND: Colovaginal fistula (CVF) is a rare, late complication of laparoscopic sacrocolpopexy that significantly impairs quality of life. We report a case occurring 9 years postoperatively to highlight its presentation and management.
CASE: A G3P3 woman in her seventies presented with yellow, foul-smelling vaginal discharge 9 years after laparoscopic total hysterectomy with sacrocolpopexy. Colonoscopy revealed a rectovaginal fistula; histology excluded diverticular disease. A multidisciplinary team performed laparoscopic removal of the eroded mesh and sigmoid resection with primary anasto

Bouveret syndrome describes an ectopic migrating gallstone resulting in gastric outlet obstruction via a cholecystoduodenal/cholecystogastric fistula. Due to its rarity, no current guidelines or formal consensus exist regarding its management or follow-up. We present a rare case of Bouveret syndrome in a man in his mid 80s. Attempts to endoscopically remove the impacted stone that caused the gastric outlet obstruction failed. We subsequently treated him with lithotripsy and gastrotomy alone with no further interventions, and contribute this case to existing literature regarding this complicati

Aorto-duodenal fistula (ADF) is a rare clinical condition characterized by abnormal communication between the 3rd or 4th portion of the duodenum and the aorta, typically associated with aneurysmal disease. The incidence of ADF is expected to increase, however, there remains a lack of consensus on the optimal approach for intestinal restoration. In this study, we present three cases of ADF and their respective treatments using three distinct surgical techniques. Initially, endovascular aortic repair was employed as a life-saving procedure, followed by elective open repair. The choice of surgica

We describe the case of a 76-year-old woman with a spontaneous nephroduodenal fistula. The patient was initially evaluated for gastrointestinal and urinary symptoms associated with fever and anemia, after which she was admitted with the diagnosis of right chronic pyelonephritis, hydronephrosis, and renal lithiasis. The fistula was diagnosed incidentally by percutaneous pyelography during a right nephrostomy and was later confirmed with an abdominal CT scan. A multidisciplinary decision was made to surgically treat the fistula (right nephrectomy plus duodenal repair); the surgery had a short-te

BACKGROUND: The appendix has a unique place in surgical history. Although the first ever appendicectomy involved a fistula to the skin, fistulae involving the appendix remain uncommon and can lead to unique surgical considerations.
METHODS: A systematic review of the literature was performed for case reports of appendiceal fistulae. We excluded cases in which the patient had a history of appendicectomy. Cases were categorized by site and aetiology, with information regarding relative frequency and demographics obtained.
RESULTS: A total of 301 case reports of fistula involving the appendix w
