Mycoplasma pneumoniae (M. pneumoniae) is a well-recognized pathogen primarily associated with respiratory tract infections. However, in rare instances, it can lead to extrapulmonary manifestations, including myocarditis. We present a case of a 15-year-old male who developed fulminant myocarditis, cardiogenic shock, and cardiac electrical storm attributed to M. pneumoniae infection. He underwent a combination of intra-aortic balloon pump (IABP) and veno-arterial extracorporeal membrane oxygenation (VA-ECMO) for cardiac support, ultimately surviving despite the intracardiac thrombus formation an

Acute total occlusion of the left main artery is a fatal event and is often accompanied by cardiogenic shock. Patients who experience this event have high mortality rates. Early percutaneous coronary intervention (PCI) with hemodynamic support has proven to improve clinical outcomes for these patients. Here we report a case of a 60-year-old man, who came into our emergency room with an acute anterior wall myocardial infarction accompanied by cardiogenic shock. He had a totally occluded left main artery on coronary angiography, necessitating cardiopulmonary resuscitation, followed by PCI with i

Cardiac rupture, such as ventricular free-wall rupture, ventricular septal perforation, and papillary muscle rupture, is a life-threatening complication of acute myocardial infarction. Herein, we report a very rare case of combining these three types of ventricular rupture. A 71-year-old woman underwent mitral valve replacement and left ventricular free-wall rupture repair after an acute myocardial infarction. She was transferred to our hospital under mechanical support by venoarterial extracorporeal membrane oxygenation and intra-aortic balloon pump due to circulatory collapse. Transthoracic

BACKGROUND: Myocardial infarction (MI) complicated by cardiogenic shock during non-cardiac surgery is a rare but fatal complication. The management of intraoperative MI is challenging.
CASE PRESENTATION: A 77-year-old hypertensive man with good functional capacity was scheduled for hepatectomy. After the start of liver resection, the electrocardiogram monitor showed ST depression, and the patient developed refractory cardiogenic shock. Transesophageal echocardiography revealed severe hypokinesis of the anteroseptal wall. The surgery was suspended, and an intra-aortic balloon pump was placed f

Cardiogenic shock continues to present a daunting challenge to clinicians, despite an increasing array of percutaneous mechanical circulatory support devices. Mortality for cardiogenic shock has not changed meaningfully in more than 20 years. There have been many attempts to generate risk scores or frameworks to evaluate cardiogenic shock and optimize the use of resources and assist with prognostication. These include the Intra-Aortic Balloon Pump in Cardiogenic Shock (IABP-SHOCK) II risk score, the CardShock score and the new CLIP biomarker score. This article reviews the Society for Cardiac

The intra-aortic balloon pump (IABP) has been in routine use for various cardiovascular indications for 5 decades. However, its regular use has been increasingly questioned. The aim of IABP counterpulsation is to enhance coronary blood flow and to reduce afterload of the left ventricle. In-vivo studies, however, provided insight into how physiological responses might counteract the desired effects of the device. Large randomized trials did not show benefit of IABP in the main indications such as infarct-related cardiogenic shock, high-risk myocardial infarction without shock and elective high-

Percutaneous coronary intervention (PCI) is establishing as the last remaining revascularization option in an increasing number of patients affected by complex coronary artery disease not suitable for surgery. Over the past decade, percutaneous left ventricular assist device (pLVAD) has increasingly replaced intra-aortic balloon pump to provide hemodynamic support during such non-emergent complex high-risk indicated procedures (CHIP) averting the risk of circulatory collapse and of adverse events in long lasting and/or complicated procedures. This review article aims to report the key factors

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is a method widely used to support circulation in patients with fulminant myocarditis (FM). However, a common complication associated with ECMO is left ventricular (LV) overload.
METHODS: This case series observed the effects of intra-aortic balloon pump (IABP) and beta-blockers for the treatment of LV overload after ECMO. The cases of eight patients with FM who underwent ECMO from September 2009 to July 2016 were reviewed.
RESULTS: Six of the eight patients survived. After ECMO treatment, insufficient LV unloading occurred in six patien

A 61-year-old female patient was diagnosed with dilated cardiomyopathy with severe left ventricle dysfunction. Two days after admission, continuous renal replacement therapy was performed due to oliguria and lactic acidosis. On the fifth day, an intra-aortic balloon pump was inserted due to low cardiac output syndrome. Beginning 4 days after admission, she was supported for 15 days thereafter with an extracorporeal left ventricular assist device (LVAD) because of heart failure with multi-organ failure. A heart transplant was performed while the patient was stabilized with the LVAD. She develop

Ergonovine have been used for the prevention and treatment of postpartum or postabortion hemorrhage. Although this modality has been considered relatively safe in the obstetric patients, there were a few cardiac events associated with this drug in the post-delivery or post-abortion patients, especially in patients with cardiovascular risk factors. We experienced cardiac arrest in a non-parturient with no discernible risk factors. Although resuscitated, she also suffered from pulmonary edema with unstable hemodynamics and low oxygenation. To manage the patient, extracorporeal membrane oxygenati

Intra-aortic balloon pumps (IABP) are commonly used in the setting of an acute myocardial infarction that is complicated by cardiogenic shock or mechanical complications such as a ventricular septal defect or papillary muscle rupture. IABP has also been shown to be useful in patients with refractory and hemodynamically unstable ventricular arrhythmias and refractory post-myocardial infarction angina. We report a case in which IABP was used in a patient with dilated cardiomyopathy and normal coronary arteries, who presented with persistent, recurrent and refractory ventricular tachycardia. His

Although the intra-aortic balloon pump (IABP) saves many lives, complication rates as high as 36% have been reported. Many of the complications require surgical attention. In 794 patients of the Massachusetts General Hospital in Boston, an IABP was inserted into either the femoral or iliac artery between Nov 1, 1968, and June 30, 1981. Eighty-seven major vascular complications occurred in 70 (8.8%) of them. Of those 70 patients, 95% denied previous symptoms of vascular disease, and 78% had had normal pulses before insertion. The two most common complications were limb ischemia (n = 36) and art

We present the case of a 19-year-old man admitted with cardioembolic stroke and rapidly evolving cardiogenic shock as the first manifestation of arterial tortuosity syndrome (ATS). Severe biventricular dysfunction, extreme left atrial enlargement, and new-onset atrial fibrillation explained the embolic event. However, the management was dominated by the profound arterial abnormalities characteristic of ATS. Marked hypoplasia and tortuosity of the iliofemoral arteries (3 mm), ascending aorta (19 mm), and thoracic aorta rendered both mechanical thrombectomy and peripheral VA-ECMO implantation te

A 72-year-old female underwent off-pump coronary bypass grafting one month after heparin exposure. Immediately after protamine administration, she developed hypotension due to acute graft failure. Grafting to left anterior descending branch was revised under intra-aortic balloon pump insertion and she was transferred to intensive care unit under stable hemodynamic condition. However, she gradually developed low cardiac output syndrome and echocardiography showed new onset of myocardial infarction. Coronary angiography on the first postoperative day revealed diffuse serious coronary thrombosis

Gastrointestinal complications after cardiopulmonary bypass (CPB) procedures are rare, but when they do occur, they carry a significant incidence of morbidity and mortality. Over a 5-year period spanning 1988-1992, 4923 CPB procedures were performed and 64 patients were identified who suffered a GI complication, giving an incidence of 1.3 per cent. The most frequent complications were GI bleeding (40%) and pancreatitis (34%). Other complications included acute cholecystitis (11%), perforated duodenal ulcer (8%), ischemic bowel (5%), and diverticulitis (2%). Complications occurred most frequent

Cardiogenic shock (CS) is a time-sensitive, dynamic, and multifactorial syndrome that can progress quickly to multiorgan failure and become fatal if not addressed urgently. We present a case of a 47-year-old man who presented with non-ST-segment elevation myocardial infarction complicated by cardiogenic shock who was found to have ischemic cardiomyopathy with multivessel coronary artery disease. His clinical course was complicated by hemodynamic collapse requiring escalation of temporary mechanical circulatory support from intra-aortic balloon pump to Impella 5.5 to venoarterial extracorporeal

In advanced heart failure, refractory hemodynamically unstable ventricular tachycardia (VT) can be life-threatening. The utilization of short-term temporary mechanical circulatory support (MCS) has been described. Still, it is limited to the intra-aortic balloon pump (IABP) or Impella 2.5/CP (Abiomed Inc., Danvers, MA, USA) which may only provide up to 1-2.5 L/min of added support. Escalation of MCS therapies should be considered. Referrals to advanced tertiary heart transplant centers should be done early to afford patients the best chance at an optimal outcome, with the option for heart tran
