Autotransplantation of the left kidney into the iliac zone was fulfilled. A direct pelvic-vesical anastomosis was made for total tuberculosis of the ureter, which resulted in perfect clinical recovery. The patient was reexamined 6 months later and found to be practically healthy. On the basis of the absence of the vesical-pelvic reflux in the autotransplanted kidney the authors propose to refuse of the intestinal plasty of the ureter in favour of the vesical-pelvic (pelvic-vesical) anastomosis.

Computed tomography has had a dramatic impact on the practice of uroradiology. The transverse image plane and excellent density differentiation allow more accurate diagnoses and often preclude the necessity for more invasive diagnostic tests or surgery. Refinements in equipment and diagnostic techniques promise further improvements in diagnosis and disease monitoring.

Although lithium was known to cause changes in renal physiology, it was considered safe until reports in late 1977 suggested that it may induce chronic irreversible nephropathy. Later reports documented similar lesions in non-lithium-treated patients with affective disturbances. Thus, the data are inconclusive. Concern for the kidney during lithium therapy is now greater than it was originally, but confidence in lithium's safety is higher than it has been for the past four years. Physician appreciation of the foregoing factors, careful patient selection, and informed consent are important in l…

Lithium alters renal physiology (e.g. polyuria) and is nephrotoxic in severe intoxications. With proper precautions, lithium was otherwise presumed safe through the mid-1970's. In 1977, the controversial question arose whether lithium may induce a chronic renal disease documented by biopsy. Later, similar lesions were demonstrated in affective disturbance patients never exposed to lithium. The possible role of lithium as an inducer of kidney damage is undetermined; but concern for the kidney in lithium patients is expressed. Better pre-lithium patient selection and renal work-up, lower serum l…

Fibrolipomatosis of varying degree was observed in 24 transplanted kidneys. In 8 patients there was no history of urinary tract obstruction or multiple infections, leaving earlier rejection periods as the probable factor responsible for development of fibrolipomatosis after transplantation.

Epidermoid cyst of the kidney is a rare entity. In the case here presented, epidermoid cyst was an incidental finding at urography performed in a 67-year-old man in connexion with splenectomy for polycythaemia. The cyst was interpreted as an old tuberculous focus. Two years later partial nephrectomy was performed because of repeated attacks of renal colic and signs of recurrent pyelonephritis. The diagnosis of epidermoid cyst was not made preoperatively in this case, or in the cases described in the literature.

Replacement lipomatosis and renal sinus lipomatosis are parts of a spectrum of fatty replacement of destroyed or atrophic renal tissue. When long-standing inflammation exists in a kidney, especially with calculus disease, replacement lipomatosis may be the end result. Awareness of this process along with the specific radiologic findings will allow a correct preoperative diagnosis.

Some neoplastic processes which involve the kidney develop not as a distinct localized mass, but rather as an infiltrating process which replaces the renal parenchyma, causes little or no mass effect, and contains little if any neovascularity. These neoplasms include (1) carcinoma of the renal pelvis when it invades the parenchyma (transitional cell and squamous cell); (2) blood-borne metastatic squamous cell carcinoma to the kidney (most frequently from the lung): (3) renal lymphoma of the infiltrating variety; and (4) infiltrating sarcomatous type of hypernephroma. While the urographic and a…

Renal pathological changes were studied by renal biopsy in 31 patients bitten by tropical poisonous snakes which included seasnake (2 cases), cobra (11 cases), green-pit viper (8 cases) and Russell's viper (10 cases). Renal failure was present only in patients bitten by seasnake and Russell's viper. Glomerular mesangial proliferation was noted in the victims of cobra, green-pit viper and Russell's viper. There was deposition of IgM AND C3 in the glomerular mesangium with extension along the capillary walls. Fibrin deposition was seen in green-pit viper cases. By electron microscopy electron de

We reviewed 81 cases of secondary carcinoma of the kidney, the clinical detection of which still remains a challenge. All lymphomas were excluded from this study. There was no direct relationship of the cell type of the primary tumor to the degree of renal invasion. Generally, functional renal impairment was not a significant entity unless renal destruction was extreme. Ureteral obstruction was infrequent in this group. Angiography was helpful in determining the metastatic spread as well as the degree of renal involvement. The varied aspects of the primary tumor's pathophysiology have been a c…

Tuberous sclerosis is essentially a hereditary disease dominated by hamartomas in various organs. Symptoms of the disease are referable to expanding growth of these hamartomas, especially in the kidney where these benign lesions most often present with pyuria, hematuria or pain from bleeding. They are apt to be erroneously diagnosed as malignant both clinically and microscopically. Renal symptomatology is most often to be found in the adult patient.
