BACKGROUND: Trochlear dysplasia is one of the most consistent anatomic factors that can lead to recurrent patellar dislocations. Various trochleoplasty procedures have been described to treat patellar dislocations in high-grade dysplasia by creating a new congruent trochlea.
PURPOSE: To present the midterm efficacy and outcome of the sulcus-deepening trochleoplasty procedure in patients with recurrent patellar dislocations due to high-grade trochlear dysplasia. The clinical evaluation results, the amount of radiological correction, and the prevalence of arthritis were recorded.

The purpose was to investigate morphological changes of the hip joint in patients with recurrent dislocation of the patella combined with trochlear dysplasia. There was a retrospective analysis of 148 patients and 150 control subjects. The selection of participants was randomized. Hip and knee data for all participants were obtained from X-ray or computed tomography measurements. Nine values, that is, central edge angle (CEA), acetabulum index angle (AIA), femoral neck-shaft angle, sharp angle, the depth of the acetabulum, femoral anteversion angle (FAA), anterior acetabular section angle (AAS

Twenty athletes with distal malalignment who sustained unilateral traumatic patellar dislocation remained impaired by chronic instability. Surgery was performed at a mean age of 18 years. Posttraumatic attenuation of the medial patellofemoral ligament was repaired near the margin of the patella in 10 knees and avulsion or attenuation posterior to the vastus medialis obliquus in 10 knees. Advancement of the medial patellomeniscal ligament at the margin of the patella and normalization of the Q angle to 10 degrees by tibial tubercle osteotomy were performed in each knee. Distal lateral retinacul

Fractures of the ipsilateral tibia and femur, frequently combined with soft tissue damage and dislocations, describe floating knee injuries, a complicated orthopedic condition. Epidemiological data suggest that floating knee injuries account for a small but significant proportion of traumatic orthopedic injuries, with a higher incidence observed in younger males engaged in high-risk activities. Anatomically, floating knee injuries involve fractures of the femur and tibia, ligamentous disruptions, and soft tissue damage, contributing to the complexity and severity of these injuries. An extensiv

Whereas traumatic lateral dislocation of the patella is a well-described clinical entity, other types of patellar dislocations, specifically those that involve patellar rotation, are unusual. These rare types also present a therapeutic problem in which reduction often requires a general anesthetic and, possibly, an open procedure. A 16-year-old boy sustained a laterally directed blow to his knee, resulting in dislocation of the patella. The patella had rotated 90 degrees in the vertical plane and became wedged against the lateral femoral condyle. Initial attempts at closed reduction in the eme

Blunt trauma to the knee of sufficient force to result in knee dislocation or fracture is commonly associated with popliteal artery injury. The challenging problem of bilateral popliteal artery injury has been rarely reported. We describe a case of bilateral popliteal artery injury after bumper crush injury between two automobiles that illustrates a successful method of management. Expeditious revascularization with minimum ischemia time was obtained by using the posterior approach, rather than the conventional medial approach, allowing two surgical teams to work simultaneously.

A 9 year-old female child presented with recurrent arthritis of ankles, left knee and unequal leg length. Clinical examination revealed mild valgus deformity in her left knee with grade 2 effusion, arthritis of both ankles and deformity in her left wrist. Examination of the affected joints showed no evidence of tenderness upon active or passive movements and the patient did not show any limping upon gait analysis. Past history of the patient revealed evidence of previous dislocation of her left hip and previous fibular fracture. Revision of her previous x-rays showed left hip dislocation, frac

A 14-year-old girl presented with left knee pain and swelling after an injury. Magnetic resonance (MR) imaging showed a transient lateral patellar dislocation with patellar osteochondral fracture, medial patellofemoral ligament tear and underlying femoral trochlear dysplasia. Open reduction and internal fixation of the osteochondral fracture, plication of the medial patellar retinaculum and lateral release were performed. As lateral patellar dislocation is often clinically unsuspected, an understanding of its characteristic imaging features is important in making the diagnosis. Knowledge of th

This case report describes a rare case of recurrent dislocation of the patella which was accompanied with trochlear dysplasia and malalignment of the patella in a 15-year-old girl. She complained of hemoarthrosis and recurrent patellar dislocation in the early knee flexion phase. Plain radiography and computed tomography (CT) showed patellar malalignment (quadriceps angle 20°) and severe dysplasia of the trochlea of the femur (sulcus angle 170°). Surgery was performed, consisting of trochleoplasty in addition to proximal and distal realignment. Trochleoplasty was undertaken using a modified De

Osteoarthritis is the leading cause of hip and knee pathology in the geriatric population. Hip and knee arthroplasty are the definitive interventions to alleviate pain and restore physical functioning. Complications related to these procedures do occur: the most com-mon of these are infection, thromboembolism, dislocations, and periprosthetic fractures. New improvements related to minimally invasive and computer-assisted navigation surgery techniques are promising and already have shown excellent outcomes in patients exposed to joint arthroplasty.

CASE: A 30-year-old man presented 11 months after a Gustilo type IIIA open knee fracture-dislocation, previously treated with femur fixation and posterior cruciate ligament reconstruction, with persistent painful instability and contracture. Examination showed a positive dial test at 30° and 90° and varus thrust gait. Imaging revealed anterolateral tibial plateau deficiency with external rotatory subluxation. Staged treatment involved gradual correction using a hexapod external fixator followed by patient-specific 3D guided intra-articular osteotomy and posterolateral corner reconstruction. He

The incidence of congenital absence of the anterior cruciate ligament (ACL) is extremely low. Congenital ACL absence has most often been found in association with conditions such as knee dislocation, knee dysplasia, proximal focal femoral deficiency, and fibular hemimelia. We report on the incidental finding of ACL aplasia in a patient with a medial meniscal tear and history of leg-length discrepancy. As has been found in prior case studies, this patient had hypertrophy of the meniscofemoral ligament of Humphrey, which likely lent her stability. This case highlights the importance of different…

Dislocation of the patella represents a frequent knee problem in childhood and adolescence. There are traumatic, recurrent, habitual and chronic forms. Many anatomical variations, which promote patellar dislocation, are known. The first traumatic dislocation is primarily treated conservatively with the exception of concomitant osteochondral fragments or very large soft tissue damage which justify surgical interventions. Recurrent, habitual and chronic dislocations are best cured surgically by vastus medialis advancement, reconstruction of the medial patellofemoral ligament, strengthening of th

BACKGROUND: There is currently no consensus regarding the optimal surgical treatment method for patients with recurrent patella instability. Our goal was to evaluate the long-term results of combined arthroscopic medial reefing and lateral release, to identify possible risk factors for recurrent dislocations and residual complaints after surgical treatment and to assess functional outcome.
METHODS: We performed a retrospective study of 38 patients (43 knees) treated with all-inside technique between 2001 and 2010. The functional outcome was evaluated with the Kujala score, while pain intensit

Hip dislocations are relatively uncommon in children, they may occur before five years of age as a result of seemingly trivial trauma. A 5-year-old boy was admitted with knee pain six weeks after a fall from bed. Physical examination showed obturator dislocation of the left hip. Owing to the late presentation and without attempting closed reduction, the patient was treated with open reduction and capsulorrhaphy. At the end of a year follow-up, the patient had no complaints and no functional restriction.

PURPOSE: Tibial tuberosity medialisation may be followed by osteoarthritis in the long term due to increased patellofemoral contact pressure and altered knee joint kinematics. We aimed to (1) systematically assess the grades of tibiofemoral and patellofemoral osteoarthritis ten to 30 years after the Elmslie-Trillat procedure and (2) to evaluate long-term knee function and activities of daily living.
METHODS: A total of 26 patients (30 knees) were re-evaluated at a minimum follow-up of 12.3 years (mean 20.9 ± 4.1 years). Osteoarthritis was graded according to Kellgren and Lawrence (K/L) and Sp

BACKGROUND: With the improvements of modern surgical techniques and hip prosthesis, the causes for dislocation after total hip arthroplasty (THA) have changed. The aim of this retrospective analysis was to identify new plain radiography findings to predict dislocation after THA.
METHODS: Five thousand five hundred thirteen consecutive primary THAs performed between January 2000 and December 2014 were reviewed through a nested case-control study design. Among them, 38 patients with 39 hip dislocations (dislocation group) were selected and matched to 78 hips in 78 patients without dislocation (

This review summaries the knowledge of the treatment of peroneal nerve palsy. Isolated peroneal nerve palsy is often seen after fracture of the knee or knee dislocation. In cases with chronic peroneal nerve palsy and foot drop, tendon transfer of the posterior tibial muscle tendon to the dorsum of the foot is a possibility to be considered. This procedure is indicated for isolated peroneal nerve palsy with good ankle mobility, good strength of the posterior tibial muscle and no chance of spontaneous remission.
