The temporal bone: an organ in search of a histopathology.

The pathology of the temporal bone and the inner ear in particular has been a neglected area for histopathologist. This review sets out to answer a number of questions on the topic: why is it neglected; how do you learn its anatomy; how do you process it; why do it; and who should do it? The histopathological features of cholesteatoma, otosclerosis and presbyacusis are discussed in detail and future areas of research are identified.

Zinc: the neglected nutrient.

Zinc was first recognized as essential for animals at the University of Illinois School of Agriculture in 1916, when it was found that zinc-deficient baby pigs were runty, developed dermatitis on their legs, and were sterile. Zinc deficiency was first recognized in man by Dr. Ananda Prasad of Detroit 26 years ago when he measured serum and hair zinc levels in young male Egyptian dwarfs who had failed to mature and were small in stature. By simply adding zinc to their regular diet, they grew in height and became sexually mature. It is now recognized that dwarfism in males is frequent around the

Role and results of cortical mastoidectomy and endolymphatic sac surgery in Menière's disease.

It is almost impossible to establish the natural history of Menière's disease and, by the same token, it is difficult to confirm the efficacy of endolymphatic sac surgery. The authors doubt the logic of sac operations. Only two controlled trials of sac surgery have been found and both cast doubt upon its value. There do not appear to be any controlled trials indicating that it works. In 14 cases of incapacitating Menière's disease, where vestibular nerve section was indicated, cortical mastoidectomy was offered and accepted in the expectation that two-thirds might be spared the more major proc

Significance of tomography in Meniere's disease: visualization and morphology of the vestibular aqueduct.

The revival of endolymphatic sac surgery has led to many investigations in Meniere's disease, including lateral tomography of the vestibular aqueduct. Contrary to recent literature, we found no difference in the presence and morphology of the vestibular aqueduct when assessed radiologically in a group of Meniere's patients and a control group. Our assessment of this information in regard to etiology and treatment of Meniere's disease is presented.

[Microsurgical neurolysis of the 8th cranial nerve in cochleo-vestibular disorders using an extended transtemporal approach].

A series of 17 patients suffering from progressive cochleovestibular disturbances (8), severe attacks of Menière's disease (5), progressive or sudden sensori-neural deafness (2), vestibular neuronitis (1) or unbearable tinnitus (1) is reported. The eighth cranial nerve was exposed from its root at the brainstem to its exit into the fundus of the internal auditory canal (CAI), by an extended middle fossa approach. The surgical technique and the intraoperative observations are described. All cases showed morphological abnormalities such as neuro-vascular compression of the nerve by arterial or v

Proceedings of the GDR Vertige 2019 annual meeting devoted to endolymphatic hydrops.

The GDR Vertige is a federative research group gathering the different components of the French neuro-otology community. The annual meeting of the GDR Vertige is an opportunity for interactive exchanges between scientists, clinicians and industrialists, on basic issues related to vestibular function, as well as translational questions regarding the management of vestibular disorders. For its fifth edition, the annual meeting of the GDR Vertige, which took place in September 2019 in Marseille (France), was devoted to one of the most peculiar phenomena of neuro-otology: endolymphatic hydrops. Autoimmune inner ear disease (AIED): A diagnostic challenge.

Autoimmune inner ear disease (AIED) has been defined as a condition of bilateral sensorineural hearing loss (SNHL), caused by an 'uncontrolled' immune system response. The inner ear can be the direct target of the immune response, but it can be additionally damaged by a deposition of circulating immune complexes or by systemic immune-mediated diseases. The clinical expression of immune-mediated inner ear disease shows a progressive bilateral and asymmetric SNHL profile, which typically benefits from a steroid and immunosuppressive therapy. The onset of AIED is between 3 and 90 days. Cochlear s

Mining immune epitopes in the inner ear.

Etiologies for many inner ear disorders, including autoimmune inner ear disease, sudden sensorineural hearing loss, and Meniere's disease, remain unknown. Indirect evidence suggests an immune-mediated process involving an allergic or autoimmune mechanism. We examined whether known immunogenic proteins share sequence similarity with inner ear proteins, which may lead to cross-reactivity and detrimental immune activation. Comprehensive bioinformatic analyses of primary sequences of intact and mutated proteins associated with human hearing loss and all proteins known to be expressed in the human

Pathophysiology of the Meniere attack: facts and theories.

A pathophysiological interpretation is discussed of the clinical findings that in one out of three cases at the onset of Meniere's disease the cochlear and vestibular symptoms start simultaneously and that during the course of Meniere's disease most attacks show both cochlear and vestibular symptoms. It is concluded that the Meniere attack is not caused by a slowly expanding endolymphatic system that finally ruptures or leaks, but that the pathogenesis of the attack is different from that of the underlying endolymphatic hydrops.

Sensorineural deafness and labyrinth damage due to lymphocytic choriomeningitis. Report of a case.

Lymphocytic choriomeningitis (LCM) occurred in a hamster handler, produced a "flu"-like illness, and caused unilateral sensorineural deafness and labyrinth damage. Unilateral sensorineural deafness and labyrinth damage caused by LCM virus have not been previously described in the literature.
