Orbital passage of pterygopalatine ganglion efferents to paranasal sinuses and nasal mucosa in man.

Parasympathetic nerves of pterygopalatine ganglion origin are considered to enter the orbit and distribute to the nasal mucosa with the anterior ethmoidal nerve. As their distribution has never been demonstrated the present study was undertaken to seek evidence of their passage and to identify their relationship with the ethmoidal nerves. The soft tissues of the pterygopalatine fossa and orbit from sixteen sides of twelve cadavers were removed in one piece and either dissected or cut coronally into slabs and prepared histologically using montages of thin resin-embedded sections at intervals su

Endoscopic transorbital approach to anterolateral skull base through inferior orbital fissure: a cadaveric study.

BACKGROUND: Endoscopic transorbital approach (eTOA) has been announced as an alternative minimally invasive surgery to skull base. Owing to the inferior orbital fissure (IOF) connecting the orbit with surrounding pterygopalatine fossa (PPF), infratemporal fossa (ITF), and temporal fossa, the idea of eTOA to anterolateral skull base through IOF is postulated. The aim of this study is to access its practical feasibility. METHODS: Anatomical dissections were performed in five human cadaveric heads (10 sides) using 0-degree and 30-degree endoscopes. A stepwise description of eTOA to anterolateral

Maxillary osteomyelitis in a young female with osteopetrosis: unveiling an uncommon connection.

Osteopetrosis encompasses a spectrum of conditions marked by heightened bone density due to faulty osteoclast-mediated bone resorption, leading to an accumulation of immature bone and thickened cortical structures. This condition gives rise to bone fragility, blood cell irregularities, nerve entrapment and growth challenges, all stemming from disrupted bone remodelling. Craniofacial distinctiveness, encompassing anomalies in the skull and jaw, is a frequent occurrence. Osteopetrosis presents a range of clinical signs, including facial and dental anomalies. The diagnostic process involves thoro

Use of ventriculostomy in the treatment of septic cavernous sinus thrombosis (SCST).

We present a novel treatment with the use of intraventricular antibiotics delivered through a ventriculostomy in a patient who developed septic cavernous sinus thrombosis after sinus surgery. A 65-year-old woman presented with acute on chronic sinusitis. The patient underwent a diagnostic left maxillary antrostomy, ethmoidectomy, sphenoidotomy and sinusotomy. Postoperatively, the patient experienced altered mental status with episodic fever despite treatment with broad-spectrum antimicrobial therapy. MRI of the brain showed extensive meningeal enhancement with the involvement of the right trig

Leiomyosarcoma of maxilla: a case report with review of literature.

One challenging feature of head and neck pathology is that a dizzying array of spindle cell lesions occurs here which ranges all the way from reactive, very aggressive forms to malignant lesions. Leiomyosarcoma is one such malignant tumour of mesenchymal origin exhibiting smooth muscle differentiation; presenting generally nonspecific signs and symptoms. Here we present a case of leiomyosarcoma in a 21 year old female patient associated with single reddish pink swelling present in the posterior right maxillary tuberosity region with moderate facial asymmetry. On histopathological analysis, all

Trigeminal neuralgia due to vertebrobasilar dolichoectasia: three case reports.

We report here 3 cases of trigeminal neuralgia (TN) due to vertebrobasilar dolichoectasia (VBD) and discuss the clinician's role in such cases. All cases presented at our clinic with paroxysmal, electric shock-like pain over their maxillary or mandibular gingiva. To confirm a diagnosis of TN, magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) were performed, and contact of the trigeminal nerve with a tortuous vertebrobasilar artery (VBA) was detected. Patients were informed about the therapeutic algorithm of TN before starting treatment. When medication became ineffectiv

Trigeminal neuralgia: An overview from pathophysiology to pharmacological treatments.

The trigeminal nerve (V) is the fifth and largest of all cranial nerves, and it is responsible for detecting sensory stimuli that arise from the craniofacial area. The nerve is divided into three branches: ophthalmic (V1), maxillary (V2), and mandibular (V3); their cell bodies are located in the trigeminal ganglia and they make connections with second-order neurons in the trigeminal brainstem sensory nuclear complex. Ascending projections via the trigeminothalamic tract transmit information to the thalamus and other brain regions responsible for interpreting sensory information. One of the mos

[Anatomic study of the submental artery relationships for submental flap raising].

INTRODUCTION: Submental flap is useful for intra-oral reconstructions and reconstructions of the lower two thirds of the face. Dissection is delicate because of a difficult exposure under the lower rim of the mandible, numerous collateral arterial branches and the proximity of the marginal branch of the facial nerve. The aim of our work was to propose anatomical landmarks in order to facilitate the submental flap raising. MATERIAL AND METHOD: Ten bodies preserved in Biomet liquid were dissected bilaterally. The anatomic relationships between the marginal branch of the facial nerve and the man

Rationale for tilted implants: FEA considerations and clinical reports.

The prevalence of the elderly population, as well as life expectancy, increased in the final decades of the 20th century, as described in the World Health Organization 2004 Annual Report. The edentulous condition therefore has a negative impact on the oral health-related quality of life. Patients wearing complete dentures for many years infact, and especially in the mandible, are often unsatisfied because of the instability of the prosthesis during speaking and eating. To date dental implant treatment is well documented as a predictable treatment for partial or complete edentulism. On the othe

Management of Temporomandibular Joint Reankylosis: A Case Series.

Background Temporomandibular joint ankylosis is a severe debilitating clinical condition where there is fusion of the mandible with the temporal bone. It is often a challenge to the maxillofacial surgeon as the surgical treatment protocol must be tailored individually according to the time of presentation of the ankylosis, and proper postoperative aggressive physiotherapy must be advocated, which is essential for a successful outcome. This is a case series of six recurrent temporomandibular joint ankylosis, in which the historical Esmarch surgery was done, and the pterygomassetric sling was in

Peripheral and central distribution of major branches of the facial taste nerve in the carp.

The major pathways of the peripheral facial taste system in the carp, Cyprinus carpio, are the maxillary (Max), mandibular (Mand), palatine (Pal) and recurrent nerve rami. The peripheral distribution of the sensory fibers of these branches (B) was determined by means of electrophysiological techniques. Max.B., Mand.B. and Pal.B., each of which arises from the gasserian-geniculate ganglionic complexes, were found to innervate respectively, the upper lip and the adjacent skin, the internal and external surface of the lower lip region, and the upper lip and the anterior palate, ipsilaterally. Challenging diagnosis of a large maxillary lesion: the multi-disciplinary approach.

We report the case of a fit and well pregnant patient in her 20s who presented with a progressively expansile swelling of the left maxilla. Despite extraction of the presumed causative tooth, the lesion continued to enlarge, extending to the orbital floor and resulting in infraorbital nerve paraesthesia. Post-partum radiological imaging demonstrated a markedly displaced upper left third molar associated with a large lytic lesion obliterating the left maxillary sinus. Differential diagnoses included a dentigerous cyst, ameloblastoma or malignancy.Surgical enucleation of the lesion with removal
