Virtual Three-Dimensional Model Analysis in the Assessment of the Maxillary and Mandibular Donor Sites on Cone-Beam Computed Tomography Images.

Using the Mimics software to assess the maxillary and mandibular donor sites on cone-beam computed tomography (CBCT) images. This cross-sectional study was conducted on 80 CBCT scans. Data in DICOM format were transferred to the Mimics software version 21, and a maxillary and a mandibular mask according to cortical and cancellous bones were virtually created for each patient based on Hounsfield units (HUs). Three-dimensional models were reconstructed, and boundaries of donor sites, including mandibular symphysis, ramus, coronoid process, zygomatic buttress, and maxillary tuberosity, were defin

Intermaxillary Fixation Screws (IMF) & Surgery First Approach: A Novel Combination and Case Report.

Orthognathic surgery, a pivotal treatment for patients with significant facial aesthetic and occlusal concerns, traditionally involves a phased approach comprising preoperative orthodontics, surgery, and postoperative orthodontic treatment. However, the extended duration of conventional methods often leads to patient dissatisfaction. In response, the surgery-first approach (SFA) has emerged as an alternative, aiming to streamline treatment and reduce complexity. This case report explores the implementation of the SFA without preoperative orthodontics, focusing on its integration with intermaxi

Management of Temporomandibular Joint Reankylosis: A Case Series.

Background Temporomandibular joint ankylosis is a severe debilitating clinical condition where there is fusion of the mandible with the temporal bone. It is often a challenge to the maxillofacial surgeon as the surgical treatment protocol must be tailored individually according to the time of presentation of the ankylosis, and proper postoperative aggressive physiotherapy must be advocated, which is essential for a successful outcome. This is a case series of six recurrent temporomandibular joint ankylosis, in which the historical Esmarch surgery was done, and the pterygomassetric sling was in

Modified transnasal endoscopic medial maxillectomy through prelacrimal duct approach.

OBJECTIVE: We previously reported a modified endoscopic medial maxillectomy (modified transnasal endoscopic medial maxillectomy through prelacrimal duct approach [MTEMMPDA]) to resect inverted papilloma (IP), for which the inferior turbinate (IT) and nasolacrimal duct (ND) can be preserved. MTEMMPDA is a safe and effective method to obtain wide, straight access to the maxillary sinus (MS). However, there are few reported cases of patients who underwent MTEMMPDA, and even fewer of patients who underwent partial osteotomy of the apertura piriformis and the anterior wall of the MS. In this study,

The clinical and histologic efficacy of xenograft granules for maxillary sinus floor augmentation.

The objective of this study was to investigate the potential of xenograft (cancellous bovine bone) granules to form vital bone in non-natural bone-forming areas of maxillary sinuses. Fourteen sinus augmentations were performed in 14 patients. Surgical outcomes were uneventful, and sufficient radiopaque volume was present radiographically to place dental implants in all sites. Clinical reentry at 6 months revealed bone formation at the osteotomy site. Histologic evaluation of the obtained bone cores revealed that xenograft granules were integrated and surrounded by woven bone and lamellar bone

Well leg compartment syndrome postorthognathic surgery under hypotensive anesthesia and reverse Trendelenburg position: a case report and literature review.

INTRODUCTION: Well leg compartment syndrome is a rare postoperative condition that occurs as a result of non-traumatic ischemic insult. It has similar pathophysiology to compartment syndrome; however, no definitive etiology is identified. Several risk factors such as patient position, leg position, young age, and high BMI for the development of this condition are considered. CASE REPORT: A 30 years old male smoker and overweight, underwent mandibular and maxillary osteotomies in reverse Trendelenburg position. He developed a WLCS post-operative and treated with fasciotomy. CONCLUSION: Increa

Patient with Crouzon Syndrome Treated with Modified Le Fort III Osteotomy without Previous Orthodontic Treatment: Case Report and a Review of the Literature.

Crouzon syndrome is the most common type of craniofacial dysostosis anomaly which presents a great challenge for clinicians since birth. Multiple synostoses in the sutures of the cranial base in this syndrome result in the hypoplasia of the midface, shallow orbits, a short nasal dorsum, maxillary hypoplasia, and, in severe cases, obstruction of the upper airways. Apart from esthetic and functional problems, these patients suffer from various psychological problems which mandate correction of midface deformities at younger ages. The aim of this report is to describe the case of a 26-year-old fe

Correction of Unilateral Congenital Zygomatico-Mandibular Fusion.

Sygnathia, or fusion of the jaw, is a rare condition in children, occurring either in isolation or as part of a larger overall syndrome. Consequences of this bony fusion may range from feeding difficulties to a complete inability to protect the airway. Owing to the uncommon nature of this problem and the high recurrence of bony fusion, standardized treatment protocols do not yet exist, making individual reports particularly useful for guiding the first-time management of such patients. In this report, we describe the case of a male infant with complete bony fusion of the right zygomatic maxill

Oral rehabilitation of an edentulous patient using two-segment le fort I repositioning and implant-supported fixed prostheses.

Anteroposterior (AP) deficiencies present a restorative treatment challenge. Complex, multidisciplinary planning is necessary for the success of the treatment. This clinical report describes an approach to managing a complex complete oral rehabilitation of an edentulous patient with skeletal transverse and AP deficiencies with a history of facial trauma to the left zygomaticomaxillary complex. This was further complicated by a hopeless remaining dentition and pneumatization of the maxillary sinuses. Treatment included initial bony augmentation of the vertically and horizontally deficient maxil

Stability after maxillary segmentation for correction of anterior open bite: a cohort study of 33 cases.

The aim of this cohort study was to evaluate the stability after multi-segmentation of the maxilla for correction of anterior open bite deformities. A total of 33 patients who underwent segmented maxillary osteotomy between 1994 and 2006 were included in the study. Rigid fixation with plates and postsurgical intermaxillary fixation for 6 weeks was applied to each patient. All patients were then followed in a standardized examination procedure at months 6, 12, 18 and up to 30 months postoperatively. Vertical and horizontal relation of the incisors was measured both clinically and on cast models…

[Surgical treatment methods in combined nasal deformities].

The paper presents the procedure of surgical correction of the twisted nose, with high humped ridge of the nose, in particular. Inferior resection of the segment from the cartilage of the nasal septum and cross-lateral osteotomy are followed by manual impression of the ridge. Mobilized bone clivi are pushed inside, osteochondral part of nasal pyramid moves backwards under the base of the maxillary frontal processes. The ridge of the nose moves backward till the frontal septum reaches the vomer. The wedged pyramid does not allow lateral dislocation of the ridge. Anteroinferior part of the septu

New biomaterials and methods for craniofacial bone defect: chondroid bone grafts in maxillary alveolar clefts.

The purpose of this study was to evaluate autogenous osteogenic marrow within chondroid bone grafts in simulated alveolar defects of mice in order to determine the ability of the graft material to effectively close the cleft from an osseous standpoint and to observe the effect of the grafting procedure. Critical-sized defects were made in the premaxillary bones of male mice using a surgical trephine and a low-speed dental engine as a model of the maxillary alveolar cleft for testing bone-inductive agents. Premaxillary trephine defects were not repaired by fibrous tissue or bone formation 30 da
