Impact of a decline in Colorado Medicaid managed care enrollment on access and quality of preventive primary care services.

BACKGROUND: Beginning in 1997 the Colorado Medicaid program de-emphasized managed care and shifted children from enrollment in a health maintenance organization (HMO), which required an enrollee to have an assigned primary care physician, to either the unassigned fee-for-service (UFFS) program in which the enrollee was not required to have a primary care physician (PCP) or to the primary care physician program (PCPP) in which the enrollee was required to select a participating PCP if one was available. The proportion of Medicaid enrollee-months in HMOs dropped from 75.4% in 1997 to 29% in 2003…

Public hospitals and substance abuse services for pregnant women and mothers: implications for managed-care programs and Medicaid.

Although an increasing proportion of the US population receives health services through managed care, pregnant women and mothers eligible for Medicaid who are involved with alcohol or other drugs are often excluded from these programs due in large part to lack of information on costs, service needs, and service use. To develop such information policy, service settings, and managed-care plans, the project conducted a national survey using a provider group with experience in caring for this population, the member universe of the National Association of Public Hospitals and Health Systems. The su…

Expenditures for care of children with chronic illnesses enrolled in the Washington State Medicaid program, fiscal year 1993.

OBJECTIVE: We calculated expenditures for children with one of eight selected chronic health conditions who were enrolled in the Washington State Medicaid program and compared them with payments for all Medicaid-enrolled children. We examined variation in mean, median, and total expenditures and identified expenditure sources. METHODS: This study analyzed Medicaid claims data for 310 977 children aged 0 to 18 who were enrolled at any time in fiscal year 1993. Tracer conditions were used to examine expenditure variation within and between diagnostic groupings. A total of 18 233 children (5.9%)…

Differences in utilization, complications, and mortality after cancer surgery by HIV status among Medicaid beneficiaries from 2001-2021.

BACKGROUND: People with HIV (PWH) experience higher cancer-specific mortality and may have worse surgical outcomes than people without HIV (PWoH), though the limited prior evidence largely predates the treat-all antiretroviral therapy (ART) era. We examined postoperative outcomes among PWH and PWoH enrolled in Medicaid in 26 states and Washington, D.C. from 2001-2021. METHODS: We identified the first inpatient/outpatient surgery for anal, bladder, breast, colorectal, female genitourinary, gastroesophageal, head and neck, kidney, liver, lung, ovarian, or pancreatic cancer among adults with con…

Risk factors for mechanical ventilation and mortality among hospitalized Medicaid patients with COVID-19 in Kentucky, 2020-2021.

This retrospective cohort study examined patient characteristics and comorbidities associated with mechanical ventilation and mortality among 28,128 hospitalized Kentucky Medicaid patients aged 18-64 years with COVID-19 in 2020-2021. Logistic regression estimated adjusted odds ratios (aORs) for mechanical ventilation and Cox regression estimated adjusted hazard ratios (aHRs) for mortality, controlling for demographics, rurality, and comorbidities. Mechanical ventilation was required in 18.8% of patients, and 8.4% died. Older age (45-54: aOR = 1.25; aHR = 1.28; 55-64: aOR = 1.35; aHR = 1.61), m…

Congenital heart defect (CHD)-related telehealth and other outpatient utilization among Medicaid and private insurance beneficiaries: 2018-2023.

IntroductionAccessing specialty cardiac care presents challenges for U.S. individuals with congenital heart defects (CHD), despite recommendations for regular cardiac follow-up. Additionally, the impact of the COVID-19 pandemic and telehealth expansion on CHD-related health care is not well-understood. Therefore, our objective was to evaluate changes in CHD-related telehealth and other outpatient utilization in the Medicaid population from 2018 to 2022 and a privately-insured population from 2018 to 2023.MethodsUsing claims from national Medicaid (2018-2022) and private insurance (2018-2023) d…

Timely Receipt of New Medication Treatment After Acute Care Encounters for Opioid Overdose in the U.S. Medicaid Population.

BACKGROUND: Acute care encounters for opioid overdose are increasingly recognized as critical opportunities to connect patients to opioid use disorder (OUD) treatment. However, national data on receipt of medication for OUD (MOUD) after overdose events are limited, particularly for new MOUD initiation. OBJECTIVE: To examine MOUD receipt and timing after emergency department or inpatient encounters for opioid overdose, including patient characteristics associated with new MOUD initiation. DESIGN: Retrospective analysis of 2014-2019 national Medicaid claims data. PARTICIPANTS: Acute care opio…

Using PDMP data to enhance public health surveillance: Opioid and concurrent benzodiazepine use in a state Medicaid program.

Accurate opioid dispensing surveillance is critical for guiding public health interventions; however, claims data alone may underestimate opioid utilization and opioid-related quality measures. This study evaluated the impact of linking prescription drug monitoring program (PDMP) data with administrative claims on opioid prescription metrics, Concurrent Use of Opioids and Benzodiazepines (COB) quality measure reporting, and predictors of additional concurrent user identification. Trends in opioid prescription metrics were assessed using 2019-2022 PDMP-linked Mississippi (MS) Medicaid claims. A…

Less Restrictive Medicaid Policies for Direct-Acting Antiviral Access Are Associated With Greater Declines in Hepatocellular Carcinoma Deaths.

BACKGROUND: In the United States (US), chronic hepatitis C virus (HCV) is the leading cause of hepatocellular carcinoma (HCC). Direct-acting antivirals (DAAs) cure HCV and reduce HCC risk, but Medicaid DAA coverage varies across states. AIM: We assessed whether Medicaid DAA access was associated with trends in HCC-related deaths. METHODS: We analyzed CDC WONDER death certificate data (1999-2023) to assess HCC-related mortality. US states were grouped based on Medicaid DAA prior authorization restrictions using the Hepatitis C: State of Medicaid Access scoring system: A+/A (n = 28), B (n = 11…

Oxidative stress, burnout, National Institutes of Health, work relative value units, and Medicaid: Need for an emergency transfusion.

There are several similarities between oxygen transport and medical economics. In a critically ill, anemic patient, increasing inspired oxygen is not an effective strategy to improve tissue oxygen delivery. A transfusion of packed red cells to increase hemoglobin increases arterial oxygen content and delivery to the tissues. Similarly, low reimbursement by Medicaid and reduced funding from National Institutes of Health (NIH) and National Science Foundation (NSF) cannot be compensated by increased provider work hours. Increased federal funding of Centers for Medicare and Medicaid Services (CMS)…

Changes in Use of Hepatitis C Direct-Acting Antivirals After Access Restrictions Were Eased by State Medicaid Programs.

IMPORTANCE: Direct-acting antivirals (DAAs) are safe and highly effective for curing hepatitis C virus (HCV) infection, but their high cost led certain state Medicaid programs to impose coverage restrictions. Since 2015, many of these restrictions have been lifted voluntarily in response to advocacy or because of litigation. OBJECTIVE: To estimate how the prescribing of DAAs to Medicaid patients changed after states eased access restrictions. DESIGN, SETTING, AND PARTICIPANTS: This modified difference-in-differences analysis of 39 state Medicaid programs included Medicaid beneficiaries who w…

Pregnancy-related risk factors and receipt of postpartum care among Texas Medicaid pregnant enrollees: Opportunities for pharmacist services.

BACKGROUND: The United States (US) experiences the highest rate of maternal mortality of similar countries. Postpartum care (PPC) focused on chronic disease management is potentially lifesaving, especially among pregnancies complicated by risk factors such as diabetes, hypertension, and mental health conditions (MHCs), which are conditions in which pharmacists can have an impact. OBJECTIVE: To evaluate the prevalence of maternal mortality risk factors and their relationships with receipt of PPC among Texas Medicaid enrollees. METHODS: A retrospective study included women with a delivery betw…
