Acute post traumatic portal venous thrombosis associated with shattered spleen: A case report.

Post-traumatic portal venous thrombosis is a rare event, and is usually seen in association with penetrating injuries. Portal venous thrombosis following blunt abdominal trauma is extremely rare with only few reports in the literature, some associated with underlying coagulation disorders. We report multidetector computed tomography findings in a case of blunt abdominal trauma with otherwise normal coagulation profile, which showed shattered spleen and MDCT evidence of acute thrombosis in the right branch of the portal vein. To the best of our knowledge, this is the first documented report of …

A Rare Case of Vesicourachal Diverticulum with Calculus in a 24-Year-Old Man.

BACKGROUND: During embryonal period, complete obliteration of the urachus at the umbilicus and incomplete closure at the bladder level are the cause of vesicourachal diverticulum. This abnormality is a rare finding that is usually discovered incidentally during radiological evaluation. Occasionally, stones have been detected within the diverticulum. CASE REPORT: We present a case of a vesicourachal diverticulum with calculus diagnosed by multidetector computed tomography and confirmed surgically and histopathologically in a 24-year-old man. CONCLUSIONS: During the radiological differential d…

Imaging of pannus formation in patients with mechanical heart valves.

Patient-prosthesis mismatch (PPM) should be recognized in patients with elevated transprosthetic gradients but without leaflet immobility, since the treatment strategy may differ in either etiology. However, thrombus and/or pannus formation should be excluded before a diagnosis of PPM is made. Particularly, pannus formation may not be diagnosed with 2-dimensional transesophageal echocardiography. Electrocardiographically gated 64-section multidetector computed tomography (MDCT) may be a promising tool in diagnosing or excluding pannus formation. Our report underlines the utility of MDCT in thi…

3D MDCT angiography for the preoperative assessment of truncus arteriosus.

Truncus arteriosus is an uncommon conotruncal anomaly, which is amenable to surgical correction. The systemic, pulmonary, and coronary circulations originate from a single arterial vessel that arises from the ventricular part of the heart and overrides the ventricular septum. The value of multidetector computed tomography with three-dimensional postprocessing lies in the detailed assessment of the presence or absence of the pulmonary arteries, including the number, origin, and branching patterns of major aortopulmonary collateral arteries, and extracardiac abnormalities such as aortic arch int…

Imaging of Orbital Trauma and Emergent Non-traumatic Conditions.

Diagnostic imaging has become critical in the care of patients suffering from traumatic or nontraumatic emergent orbital conditions. Multidetector computed tomography (MDCT) has become the standard imaging modality for assessing orbital trauma because of its accurate assessment of orbital skeletal and soft tissues injuries. Contrast-enhanced MR imaging is the first-line examination in the assessment of nontraumatic orbital conditions given its excellent evaluation of the orbital soft tissues. Conventional angiography is necessary in some vascular orbital complications and allows for endovascul…

Off-pump coronary artery bypass grafting via median sternotomy in a patient with a history of esophagectomy with substernal gastric tube reconstruction: report of a case.

We performed successful off-pump coronary artery bypass grafting (OPCAB) in a patient with unstable angina caused by triple-vessel disease, who had undergone esophagectomy with substernal gastric tube reconstruction for esophageal cancer 40 years earlier. Preoperative multidetector-row computed tomography was done to evaluate the course of the gastric tube and also that of the gastroepiploic artery, which feeds the gastric tube. Following careful sternal re-entry and adequate dissection, OPCAB was performed safely via a median sternotomy without injuring the gastric tube or causing hemodynamic

Acute coronary syndrome caused by left main coronary artery compression between the aortic root and massive dilation of the left atrium.

Left main compression by pulmonary artery dilation has been reported previously. Percutaneous or surgical interventions are treatment options. We report on a 78-year-old man with exertional chest pain and dyspnea. Coronary angiography revealed significant ostial left main artery stenosis. Intravascular ultrasound showed no atherosclerotic plaque but did reveal a rhythmic external compression between the aortic root and another lucent structure. The patient underwent successful stenting of the lesion. Subsequently, 64-slice multidetector coronary computed tomography found the source of compress

Obturator hernia of the fallopian tube.

Obturator hernia of the fallopian tube is extremely rare. Multidetector computed tomography of a 43-year-old nulliparous woman with sudden onset lower right abdominal pain showed a low-density mass in the right obturator canal suspected of being an obturator hernia of the uterine adnexa. She was diagnosed as having an incarcerated obturator hernia of the fallopian tube at operation and treated with prosthetic mesh. Obturator hernia of the fallopian tube is very rare, and all cases reported in the literature were localized on the right side, perhaps due to the lesser mobility of the left than t…

Early non-aneurysmal infectious aortitis: Cross-sectional imaging diagnosis.

In patients without history of vascular surgery, infectious aortitis is a very uncommon, life-threatening condition with nonspecific clinical manifestations, which exposes the patient to uncontrolled sepsis and to the risk of retroperitoneal rupture. State-of-the-art cross-sectional imaging with contrast-enhanced multidetector computed tomography and magnetic resonance imaging allows confident diagnosis and characterization of unsuspected aortitis in septic patients at an early stage before the development of aneurysmal dilatation. The asymmetric distribution of periaortic inflammatory tissue …

Anomalous "High Take-Off" of the right coronary artery evaluated by coronary CT angiography.

Congenital coronary anomalies are uncommon but can cause sudden cardiac death or myocardial ischemia. Conventional coronary angiography is an invasive and expensive modality, sometimes unable to delineate the exact origin and course of an anomalous artery. In this case report, we describe an "extremely rare" anomaly of the right coronary artery where a 64-slice multidetector computed tomography provided valuable information regarding its exact site of the aortocoronary take-off and its spatial relationship with the great vessels. Knowledge of CT appearances and an understanding of the clinical…

CT angiography of the hepatic and pancreatic circulation.

Multidetector computed tomography angiography (MDCTA) is an established, noninvasive, and effective imaging method to evaluate the liver and the pancreas primarily for neoplasm staging and presurgical planning. However, its role has also extended into a variety of other clinical indications. Technological advances in MDCT scanners and post processing now offer new opportunities with CTA, but the challenges of protocol optimization should be confronted appropriately to meet the new expectations. In this review, we focus on the technical details with MDCTA protocols for liver and pancreas and br…

Vascular malformation of the jejunum presenting as obscure gastrointestinal haemorrhage: detection with multidetector CT angiography.

Vascular malformation of the small bowel is an uncommon cause of gastrointestinal haemorrhage. Phlebectasia or venous ectasia is a rare benign vascular anomaly of the gastrointestinal tract. We report a 39-year-old Egyptian man presenting with multiple jejunal phlebectasia, liver haemangioma and port-wine naevus. Despite recurrent melaena, the results of various routine investigations, including repeated endoscopic procedure, were negative. The site and aetiology of bleeding was detected using multidetector computed tomography (MDCT) angiography and was further confirmed by double-balloon ente…

MDCT features of cardiothoracic sources of stroke.

Multidetector computed tomography (MDCT) is widely used in the assessment of cardiothoracic disease and provides high-resolution images of the heart, great vessels, and lungs. A range of cardiothoracic conditions can precipitate stroke, including intracardiac thrombus, right-to-left shunts, and diseases of the thoracic aorta. Many of these conditions may be identified on non-electrocardiogram (ECG)-gated studies, but the advent of high temporal resolution ECG-gated MDCT provides superior anatomical delineation. Radiologists should be familiar with the pathogenesis and CT features of cardiothor…

Emergency transcatheter embolisation of superior mesenteric arteriovenous fistula complicated by recurrent haematemesis.

Arterioportal fistulas are rare. Superior mesenteric arteriovenous fistula is uncommon and usually observed in patients who have abdominal trauma or have undergone abdominal surgery. If untreated, mesenteric arteriovenous fistula is potentially fatal due to portal hypertension with potential complications such as massive variceal bleeding or progressive liver failure. We report a 50-year-old Chinese man who had a history of abdominal surgery and presented with recurrent haematemesis. He was diagnosed by multidetector computed tomography to have a superior mesenteric arteriovenous fistula. Subs…

Double left anterior descending artery arising from right and left sinus of Valsalva in patient with acute coronary syndrome.

An aberrant origin of left anterior descending artery (LAD) from right and left sinus of Valsalva is a rare anomaly but clinically important. Coronary angiography (CAG) was performed in patient with acute coronary syndrome, and revealed severe stenosis at the mid of LAD. The distal of LAD was likely to be a total occlusion. It was proved to be a double left anterior descending artery arising from right and left sinus of Valsalva by multidetector-row computed tomography (MDCT) and following CAG. We describe here congenital coronary anomaly case which would be misleading as a total occlusion.

Arteriovenous malformation arising on persistent sciatic vessels.

BACKGROUND: A persistent sciatic artery is a rare congenital anomaly, and an arteriovenous malformation arising on persistent sciatic vessels is extremely rare. METHODS: This report presents the case of a 30-year-old female with persistent sciatic vessels complicated with an arteriovenous malformation in the right buttock. It was surgically inaccessible, and a three-staged transcatheter embolization using 20% N-butyl-cyanoacrylate/80% lipiodol was performed. RESULTS: The arteriovenous malformation was shown to have been extinguished by multidetector computed tomography. CONCLUSIONS: This re…

Ectopic pancreaticobiliary drainage accompanied by proximal jejunal adenoma: a case report.

A patient with obstructive jaundice was examined by multidetector row helical computed tomography (MDCT) and magnetic resonance imaging (MRI), and his common bile duct was observed to be leading into the distal portion of the horizontal duodenum with a pancreaticobiliary union outside the duodenal wall. A mass was also found in the proximal jejunum. All the above findings were confirmed by subsequent surgery, thus contrast-enhanced MDCT and MRI with appropriate image post-processing could provide non-invasive and accurate information regarding anatomy and lesions of the pancreaticobiliary duct
