Respiratory muscle involvement in sarcoidosis.

In sarcoidosis, muscle involvement is common, but mostly asymptomatic. Currently, little is known about respiratory muscle and diaphragm involvement and function in patients with sarcoidosis. Reduced inspiratory muscle strength and/or a reduced diaphragm function may contribute to exertional dyspnea, fatigue and reduced health-related quality of life. Previous studies using volitional and non-volitional tests demonstrated a reduced inspiratory muscle strength in sarcoidosis compared to control subjects, and also showed that respiratory muscle function may even be significantly impaired in a su

Fundamentals of Embouchure in Brass Players: Towards a Definition and Clinical Assessment.

UNLABELLED: Brass players may experience problems producing an optimal sound (or range of sounds) in their instrument. Assessing and treating dysfunctional embouchure requires knowledge of functional embouchure, but peer-reviewed literature on dysfunctional and functional embouchure is scarce. OBJECTIVE: This study aimed to provide a narrative overview of embouchure based on information from different scientific and clinical fields. This should be regarded as a first step in constructing a reliable, valid, and practical multi-item method to assess embouchure for brass players. METHODS: Liter

Cycling cadence selections at different saddle heights minimize muscle activation rather than energy cost.

Unlike walking and running, people do not consistently choose cadences that minimize energy consumption when cycling. This suggests either that the neural control system for locomotion relies on indirect sensorimotor cues to energetic cost that are approximately accurate during walking but not cycling, or that an alternative objective function applies that correlates with energy expenditure in walking but not cycling. This study compared how objective functions derived as proxies to (i) energy cost or (ii) an avoidance of muscle fatigue predicted self-selected cycling cadences (SSC) at differe…

Patients' Lived Experiences of Spasticity.

Spasticity is an individual, sensory-motor experience that is correlated with nonmotor symptoms of pain, fatigue, and dysesthesia. Patients and clinicians may have different understandings of spasticity. This discrepancy is complicated because spasticity is highly variable depending on the individual and the unique stressors in their environment. Listening to how patients explain their symptoms and what factors make their spasticity better or worse is key to effective symptom management. Good patient-clinician communication leads to applying the correct treatment of each symptom. The use of or

The Effect of Skeletal Muscle-Specific Creatine Treatment on ALS NMJ Integrity and Function.

Although skeletal muscle (hSKM) has been proven to be actively involved in Amyotrophic Lateral Sclerosis (ALS) neuromuscular junction (NMJ) dysfunction, it is rarely considered as a pharmacological target in preclinical drug discovery. This project investigated how improving ALS hSKM viability and function effects NMJ integrity. Phenotypic ALS NMJ human-on-a-chip models developed from patient-derived induced pluripotent stem cells (iPSCs) were used to study the effect of hSKM-specific creatine treatment on clinically relevant functional ALS NMJ parameters, such as NMJ numbers, fidelity, stabil

Low-energy laser therapy application on knee joints as an auxiliary treatment in patients with polyarticular juvenile idiopathic arthritis: a dual-arm randomized clinical trial.

Patients with juvenile idiopathic arthritis (JIA) always experience persistent pain and stiffness which induces muscle weakness, fatigue, and functional limitations. This study evaluated whether applying low-energy laser therapy (LLT) on the knee joint could be an effective adjuvant intervention for patients with JIA. Sixty children with polyarticular JIA participated and were randomly allocated to receive either LLT (wavelength λ = 903 nm; power output of 50 mW; and energy of 1.5 J) plus exercises (LLT group) or exercises alone (control group). Pain, peak concentric torque of quadriceps muscl

Changes in biomechanics of skiing at maximal velocity caused by simulated 20-km skiing race using V2 skating technique.

This study investigated how the fatigue caused by a 20-km simulated skating cross-country skiing race on snow affects the final spurt performance from a biomechanical perspective. Subjects performed a 100-m maximal skiing trial before and at the end of the simulated race. Cycle characteristics, ground reaction forces from skis and poles, and muscle activity from eight muscles were recorded during each trial. Results showed that subjects were in a fatigued state after the simulated race manifested by 11.6% lower skiing speed (P<.01). The lower skiing speed was related to an 8.0% decrease in cyc

Association of Muscle Endurance, Fatigability, and Strength With Functional Limitation and Mortality in the Health Aging and Body Composition Study.

BACKGROUND: Mobility limitation is highly prevalent among older adults and is central to the loss of functional independence. Dynamic isokinetic muscle fatigue testing may reveal increased vulnerability to disability and mortality beyond strength testing. METHODS: We studied community-dwelling older adults enrolled in the Health Aging and Body Composition study (age range: 71-82) free of mobility disability and who underwent isokinetic muscle fatigue testing in 1999-2000 (n = 1,963). Isokinetic quadriceps work and fatigue index was determined over 30 repetitions and compared with isometric qu…

Noninvasive optical characterization of muscle blood flow, oxygenation, and metabolism in women with fibromyalgia.

INTRODUCTION: Women with fibromyalgia (FM) have symptoms of increased muscular fatigue and reduced exercise tolerance, which may be associated with alterations in muscle microcirculation and oxygen metabolism. This study used near-infrared diffuse optical spectroscopies to noninvasively evaluate muscle blood flow, blood oxygenation and oxygen metabolism during leg fatiguing exercise and during arm arterial cuff occlusion in post-menopausal women with and without FM. METHODS: Fourteen women with FM and twenty-three well-matched healthy controls participated in this study. For the fatiguing exe

Fatigue increases ankle sprain risk in badminton players: A biomechanical study.

Ankle sprains are the most common injury in regular badminton players and usually occur at the end of a match or training. The purpose of the present study was to examine the influence of fatigue produced by badminton practice on the lower limb biomechanics of badminton players. It was hypothesized that fatigue induces ankle kinematic and lower leg muscle activity changes which may increase the risk of ankle sprain. Ankle kinematics, ankle kinetics and muscles activities of 17 regular badminton players were recorded during lateral jumps before and after an intense badminton practice session. P

Evaluation of minimum pinch force while holding in fibromyalgia patients.

OBJECTIVE: This study aimed to investigate the relationship between the minimum pinch force applied during object carrying and physical fatigue in patients with fibromyalgia. For this purpose, the study evaluated the association between the minimum and maximum pinch forces exerted while carrying a weight and both isokinetic muscle strength and the isokinetic fatigue index. METHODS: One hundred eight (54 FMS/54 healthy) women participated. Pinch force was measured with a force sensor, and wrist flexor/extensor strength and fatigue index were evaluated using an isokinetic dynamometer at 180°/s.

The role of the nervous system in neuromuscular fatigue induced by ultra-endurance exercise.

Ultra-endurance events are not a recent development but they have only become very popular in the last 2 decades, particularly ultramarathons run on trails. The present paper reviews the role of the central nervous system in neuromuscular fatigue induced by ultra-endurance exercise. Large decreases in voluntary activation are systematically found in ultra-endurance running but are attenuated in ultra-endurance cycling for comparable intensity and duration. This indirectly suggests that afferent feedback, rather than neurobiological changes within the central nervous system, is determinant in t
