Efficacy of empiric antibiotic treatment of late-onset neonatal sepsis caused by Enterobacteriaceae: A systematic review.

Neonatal sepsis is a serious condition, where an adequate empiric antibiotic treatment is crucial. The objective of this systematic review is to assess whether the World Health Organization's recommended treatment regime remains applicable for late-onset neonatal sepsis caused by Enterobacteriaceae, in the time of increased antimicrobial resistance. PubMed was searched for articles from 2009 to 2020. A total of 49 articles were eligible for inclusion. The review was carried out in accordance with PRISMA guidelines. For Klebsiella spp. 100, 68 and 63% of the studies found sensitivity to ampicil…

Maternal Obesity and Risk of Early-onset Neonatal Bacterial Sepsis: Nationwide Cohort and Sibling-controlled Studies.

BACKGROUND: Maternal overweight and obesity are related to risks of pregnancy and delivery complications that, in turn, are associated with newborn infections. We examined the associations between early pregnancy body mass index (BMI; kg/m2) and risk of early-onset neonatal bacterial sepsis (EOS). METHODS: We conducted a nationwide population-based retrospective cohort study of 1 971 346 live singleton infants born in Sweden between 1997 and 2016. Outcome was a culture-confirmed EOS diagnosis. We estimated hazard ratios (HR) of EOS according to BMI using proportional hazard models, and identi

A study of red cell distribution width and neonatal sepsis at Soba University Hospital, Khartoum, Sudan.

Neonatal sepsis is one of the most critical illnesses in newborns with significant morbidity and mortality, particularly in developing countries. The present, cross-sectional, hospital-based, study was conducted to evaluate the association of red cell distribution width (RDW) with neonatal sepsis and its role as a predictive marker in the diagnosis of neonatal sepsis at Soba University Hospital, during the period July 2018 to April 2019. The study population was term neonates, aged 1-28 days, who were diagnosed with neonatal sepsis, with positive blood culture. A total of 111 full-term neonate…

Pattern of bacterial profile and antibiotic susceptibility among neonatal sepsis cases at Cairo University Children Hospital.

OBJECTIVES: Neonatal sepsis is the third leading contributor to mortality and morbidity. Emanating resistance to antibiotics in neonatal intensive care units (NICUs) is considered a major burden. In this study, we aimed to investigate the bacterial prevalence and antibiotic profile among patients admitted with sepsis in the NICU of Cairo University Children Hospital. METHODS: Neonates with suspected sepsis were evaluated for bacterial sepsis in their blood cultures. The neonates with positive bacterial blood culture were included in this study, whereas neonates with negative culture were excl…

Antibiotic Treatment of Suspected and Confirmed Neonatal Sepsis Within 28 Days of Birth: A Retrospective Analysis.

Neonatal sepsis causes significant mortality and morbidity worldwide. Diagnosis is usually confirmed via blood culture results. Blood culture sepsis confirmation can take days and suffer from contamination and false negatives. Empiric therapy with antibiotics is common. This study aims to retrospectively describe and compare treatments of blood culture-confirmed and unconfirmed, but suspected, sepsis within the University of Utah Hospital system. Electronic health records were obtained from 1,248 neonates from January 1, 2006, to December 31, 2017. Sepsis was categorized into early-onset (≤3 d…

Vaginal dysbiosis increases risk of preterm fetal membrane rupture, neonatal sepsis and is exacerbated by erythromycin.

BACKGROUND: Preterm prelabour rupture of the fetal membranes (PPROM) precedes 30% of preterm births and is a risk factor for early onset neonatal sepsis. As PPROM is strongly associated with ascending vaginal infection, prophylactic antibiotics are widely used. The evolution of vaginal microbiota compositions associated with PPROM and the impact of antibiotics on bacterial compositions are unknown. METHODS: We prospectively assessed vaginal microbiota prior to and following PPROM using MiSeq-based sequencing of 16S rRNA gene amplicons and examined the impact of erythromycin prophylaxis on bac…

Incidence, clinical features, and implications on outcomes of neonatal late-onset sepsis with concurrent infectious focus.

BACKGROUND: Neonatal bloodstream infection (BSI) is the most important cause of morbidity and mortality in the neonatal intensive care unit (NICU). Although most neonatal BSIs are primary bacteremia, some are associated with a focus of infection. This distinction is not well characterized. METHODS: All patients with neonatal late-onset sepsis (LOS) between January 2006 and December 2013 were enrolled. LOS was categorized as a BSI with a concurrent focus of infection if LOS occurred before or within 24 h after the diagnosis of a specific infectious entity, and as "primary bacteremia" if no con

Frequent nasopharyngeal suctioning as a risk factor associated with neonatal coagulase-negative staphylococcal colonisation and sepsis.

INTRODUCTION: This case-control study aimed to determine whether catheter use was significantly associated with coagulase-negative staphylococci (CoNS) colonisation and/or sepsis in neonates. METHODS: Weekly swabs of the nose, umbilicus, rectum, wounds, eye discharge and intravenous catheter tips (after removal) of infants admitted to the neonatal intensive care unit of Universiti Kebangsaan Malaysia Medical Centre, Malaysia, were cultured. CoNS sepsis was diagnosed if pure growth of CoNS was cultured from the peripheral blood specimen of symptomatic infants. For each infant with CoNS colonis

Gram stain-specific-probe-based real-time PCR for diagnosis and discrimination of bacterial neonatal sepsis.

Sepsis is a serious disease with high mortality in newborns. It is very important to have a convenient and accurate method for pathogenic diagnosis of neonatal sepsis. We developed a method of simultaneous detection and Gram classification of clinically relevant bacterial pathogens causing sepsis directly from blood samples with Gram stain-specific-probe-based real-time PCR (GSPBRT-PCR). With GSPBRT-PCR, 53 clinically important strains representing 25 gram-positive and 28 gram-negative bacterial species were identified correctly with the corresponding Gram probe. The limits of the GSPBRT-PCR a…

Comparative study of virulence traits of Escherichia coli clinical isolates causing early and late neonatal sepsis.

Neonatal meningitis and septicemia caused by Escherichia coli are still major health problems in industrialized countries. Forty-seven E. coli strains causing neonatal sepsis were analyzed. Twenty-two and 25 strains caused early (detected from 0 to 3 days after birth) and late (detected from 4 to 28 days after birth) infections, respectively. Only the ibeA gene was significantly more prevalent in the strains causing early infections.

Identification of Streptococcus agalactiae virulence genes in the neonatal rat sepsis model using signature-tagged mutagenesis.

Group B streptococcal (GBS) infections are the most common cause of bacterial sepsis in the immediate newborn period. Apart from the capsule, the factors required for survival of GBS in the host are not well defined. In this study, signature-tagged transposon mutagenesis (STM) was used to identify genes required for growth and survival of GBS in a neonatal rat sepsis infection model. Approximately 1600 transposon mutants were screened in pools of 80 mutants, and approximately 120 mutants defective for survival in the animal host were identified. We successfully cloned and sequenced DNA flankin

Obstetric observations in eleven cases of neonatal sepsis due to the group B beta hemolytic streptococcus.

Neonatal sepsis due to Group B streptococcus is reported to be increasing, but the reasons are unclear. Eleven cases from a single hospital were reviewed for common obstetric factors. Frequent observations associated with 8 surviving infants were maternal infection, early evaluation and treatment of the newborn, and 4+ or 5+ growth on maternal cultures. In contrast, mothers of the 3 infants who died had no clinical signs of infection, and cultures were not obtained. All 11 infants had direct monitoring during labor but the frequency of infection, 1/1600 live births, was lower than usually repo…

Pulmonary changes in neonatal sepsis to group B beta-hemolytic Streptococcus: relation of hyaline membrane disease.

The radiographic changes of hyaline membrane disease were seen in seven of eight infants with group B beta-hemolytic streptococcal sepsis. Hyaline membranes were found throughout the lungs of all six infants who were examined after death. The most striking finding was the presence of group B streptococci within the membranes of five infants. Streptococci were so numerous in one instance that they comprised the bulk of the membrane. Another infant uas thought to have classical hyaline membrane disease until numerous group B streptococci were found within the membranes when lung sections were ex
