Microchimerism in the transfused obstetric population.

Microchimerism (MC), the coexistence of allogeneic populations of cells within a host, is well described in pregnancy and blood transfusion. To date, transfusion-associated MC (TA-MC) appears unique to patients transfused after severe traumatic injury. We sought to determine whether transfusion in the peripartum period results in enduring, high-level TA-MC. We conducted a prospective cohort study of 22 women who were newly transfused within 48 h of delivery. Two subjects showed evidence of transient TA-MC; however, MC was not detected at 6 weeks and 6 months. The negative findings suggest that

Obstetrical trauma--therapeutic dilemmas?

UNLABELLED: Obstetrical trauma is frequent among newborns alive and could represent a cause of perinatal death. The aim of this paper consists of presenting the authors experience regarding the diagnosis and treatment of skeletal and thoracic-abdominal organs related to traumatic birth. MATERIAL AND METHODS: Between 2000 and 2010, 33 patients with trauma at birth were included in the study. The type of the lesion and the therapeutical approach and results were analyzed. RESULTS: Two categories of results were clearly differentiated: excellent for obstetric trauma involving limbs and soft par

Obstetric anaesthesia and peripartum management.

Anaesthetists play a key role in the management of parturients with cardiac disease. Pregnant women with cardiac disease should be seen antenatally in a high-risk obstetric anaesthesia clinic, and a comprehensive management plan formulated. Most women with cardiac disease can have a vaginal delivery, and early epidural analgesia is frequently considered beneficial. No evidence exists to definitively support either regional or general anaesthesia for caesarean section, and much depends on the urgency of the procedure and the severity of disease. Women with the most severe forms of cardiac disea

Obstetrical brachial plexus palsy.

Obstetrical brachial plexus palsy is considered to be the result of a trauma during the delivery, even if there remains some controversy surrounding the causes. Although most babies recover spontaneously in the first 3 months of life, a small number remains with poor recovery which requires surgical brachial plexus exploration. Surgical indications depend on the type of lesion (producing total or partial palsy) and particularly the nonrecovery of biceps function by the age of 3 months. In a global palsy, microsurgery will be mandatory and the strategy for restoration will focus first on hand r

[Surgical management of obstetric fistulae].

With three decades of experience in the treatment of obstetric fistulae (OF), the authors discuss surgical management of OF secondary to blocked labor (> 48 hours), the main cause of OF in poor countries. Clinical classification is mandatory to ensure the correct indication, with three situations of increasing difficulty. The technical principles are simple and few instruments are required. Surgical difficulty increases in case of tissue sclerosis, difficult locations, and associated lesions (sphincter destruction, recto-anal fistulae, etc.). Group I OF can be cured in a single operation, whil

Non-obstetric vulval trauma.

OBJECTIVE: To describe the mechanism, injury pattern and management of women who present to the ED with non-obstetric vulval trauma. METHODS: A retrospective, single-institution case series was carried out. Data were sourced from medical records of women who presented to the ED and Royal Brisbane and Women's Hospital between 2007 and 2011. Records of possible injuries to the vulva were assessed to determine incidence, age, site, type of injury, mechanism of injury and whether urinary retention required treatment. This information was analysed using the computer software Statistical Package fo

[Shoulder dystocia: an obstetrical emergency].

Shoulder dystocia is one of the most feared obstetric emergencies due to related maternal and neonatal complications and therefore, the growing of medico-legal litigation that it entails. Although associated with risk factors such as fetal macrossomia, gestacional diabetes and instrumented delivery, the majority of cases are unpredictable. The lack of a consensus on shoulder dystocia diagnosis causes variations on its incidence and hampers a more comprehensive analysis. Management guidelines described for its resolution include several manoeuvres but the ideal sequence of procedures is not cle

Obstetric brachial plexus injury.

Obstetric brachial plexus injury (OBPI), also known as birth brachial plexus injury (BBPI), is unfortunately a rather common injury in newborn children. Incidence varies between 0.15 and 3 per 1000 live births in various series and countries. Although spontaneous recovery is known, there is a large subset which does not recover and needs primary or secondary surgical intervention. An extensive review of peer-reviewed publications has been done in this study, including clinical papers, review articles and systematic review of the subject. In addition, the authors' experience of several hundred

Angiogenic imbalances: the obstetric perspective.

Clinical and experimental evidence indicates that angiogenic imbalances may participate in the mechanisms of disease of several pregnancy complications, some of which may be life threatening. This article reviews current evidence in support of this view and the possibility that the fetus may play a central role in these imbalances; it also reviews recent experimental observations that modulation of angiogenic imbalances during pregnancy may have prophylactic and/or therapeutic value.

The preoperative assessment of obstetric patients.

The importance of early identification and management of the high-risk obstetric patient is emphasised in the Confidential Enquiry into Maternal and Child Health (CEMACH) report. High-risk patients who need anaesthetic input include those with airway problems, cardiorespiratory disease and rare genetic conditions, such as malignant hyperthermia and suxamethonium apnoea. Anaesthetic options for labour analgesia as well as anaesthesia for operative delivery will need to be discussed in detail with the patient if a delivery management plan is to be constructed. Input from other medical teams, suc

Labour analgesia and obstetric outcomes.

Neuraxial analgesic techniques are the gold standards for pain relief during labour and delivery. Despite the increased use and known benefits of neuraxial labour analgesia, there has been significant controversy regarding the impact of neuraxial analgesia on labour outcomes. Review of the evidence suggests that effective neuraxial labour analgesia does not increase the rate of Caesarean delivery, even when administered early in the course of labour; however, its use is associated with a prolonged second stage of labour. Effective second-stage analgesia might also be associated with an increas

Obstetric management of obesity in pregnancy.

Rates of obesity among the pregnant population have increased substantially and adiposity has a damaging effect on every aspect of female reproductive life. This review summarises epidemiological data concerning obesity-related complications of pregnancy. Obesity is linked to a number of adverse obstetric outcomes as well as increased maternal and neonatal morbidity and mortality. These complications include miscarriage, congenital abnormalities, pre-eclampsia, gestational diabetes mellitus, iatrogenic preterm delivery, postdates pregnancy with increased rates of induction of labour, caesarean

Obstetric brachial plexus injuries.

Obstetric brachial plexus lesions (OBPLs) are typically caused by traction to the brachial plexus during labor. The incidence of OBPL is about 2 per 1000 births. Most commonly, the C5 and C6 spinal nerves are affected. The prognosis is generally considered to be good, but the percentage of children who have residual deficits may be as high as 20% to 30%. Surgery should be restricted to severe cases in which spontaneous restoration of function is not likely to occur (ie, in neurotmesis or root avulsions). In this article, the authors present an overview of our current understanding of the neuro

Teenage pregnancies and their obstetric outcomes.

This study was conducted to evaluate the obstetric performance of teenage women in India. In total, 13,210 women were included in the study, of whom 840 were teenagers (<19 years) and 12,370 were > or =20 years. Antepartum, intrapartum and postpartum events were recorded and comparative analysis was done. We found that teenage women were at a significantly higher risk for development of severe anaemia (relative risk [RR] 1.61, P value <0.02), eclampsia (RR 1.95, P value <0.05), preterm labour (RR 1.25, P value <0.001), intrauterine growth retardation (RR 2.29, P value <0.001) and low birth wei
