A Retrospective Machine Learning Analysis to Predict 3-Month Nonunion of Unstable Distal Clavicle Fracture Patients Treated with Open Reduction and Internal Fixation.

BACKGROUND: This retrospective study aims to predict the risk of 3-month nonunion in patients with unstable distal clavicle fractures (UDCFs) treated with open reduction and internal fixation (ORIF) using machine learning (ML) methods. ML was chosen over traditional statistical approaches because of its superior ability to capture complex nonlinear interactions and to handle imbalanced datasets. METHODS: We collected UDCFs patients at Nanjing Luhe People's Hospital (China) between January 2015 and May 2023. The unfavorable outcome was defined as 3-month nonunion, as represented by disappeared

The Assessment of Intermaxillary Fixation and Open Reduction Using Skeletal Anchorage System Compared With Arch Bar in Mandible Fracture Based on CT Image.

BACKGROUND: There are many different techniques to achieve intermaxillary fixation (IMF) for open reduction of mandible fractures. The arch bar has long been used as the gold standard of IMF to assist open reduction. However, owing to its long operating time, risk of needle stick injury, and gingival trauma, surgeons looked into different treatment options for IMF, such as the skeletal anchorage system (SAS). Therefore, this study aimed to compare the stability between IMF with arch bar and IMF with SAS based on computed tomography image. MATERIALS AND METHODS: In this retrospective study, po

Single stage open reduction, intramedullary rod, bone grafting, and plate fixation for managing adolescent midshaft femoral fracture non-union, report of two cases.

Although rare, long bones fracture non-union in pediatric and adolescent patients is still being reported. It poses a challenge for the trauma surgeons, although principles for adult patient management apply to pediatrics and adolescents; however, there is no standard protocol to deal with such situations. We report two male adolescent patients, 11 and 12 years old, presented with non-united midshaft femoral fracture non-union after being multiply operated on. One patient was diagnosed with a septic non-union, while the other had an atrophic type. Both were treated following the same technique

Arrested root growth and concomitant failure of eruption of a developing tooth following open reduction and internal fixation of a pediatric mandibular fracture.

Mandible is one of the commonly fractured bone in children during maxillofacial trauma. These factures are usually managed conservatively but sometimes require open reduction and internal fixation (ORIF). Management of mandibular fracture in children is influenced by the presence of developing permanent tooth buds, minimal anchorage from primary teeth and facial growth and development. Although, there have been technical and material advancements with ORIF, it is still associated with complications related to growth and damage to developing teeth. This case report describes a case of mandibula

A Meta-Analysis for Postoperative Complications in Tibial Plafond Fracture: Open Reduction and Internal Fixation Versus Limited Internal Fixation Combined With External Fixator.

The treatment of tibial plafond fractures is challenging to foot and ankle surgeons. Open reduction and internal fixation and limited internal fixation combined with an external fixator are 2 of the most commonly used methods of tibial plafond fracture repair. However, conclusions regarding the superior choice remain controversial. The present meta-analysis aimed to quantitatively compare the postoperative complications between open reduction and internal fixation and limited internal fixation combined with an external fixator for tibial plafond fractures. Nine studies with 498 fractures in 49

Preserving the Bone Envelope: A Prospective Observational Study of Closed Reduction and Ilizarov Circular Fixation for Open and Closed Distal Tibial Fractures.

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

Risk Factors for Surgical Site Infection After Open Reduction and Internal Fixation in Overweight and Obese Patients With Tibial Plateau Fractures.

AIM: Overweight and obese patients present unique pathophysiological characteristics that may increase the risk of surgical site infection (SSI) following orthopedic surgery. However, the specific risk factors for SSI in overweight and obese patients with tibial plateau fractures remain unclear. This study aimed to identify the independent risk factors for SSI after open reduction and internal fixation (ORIF) in this high-risk population. METHODS: A total of 300 patients with tibial plateau fractures who underwent ORIF were retrospectively analyzed. Patients were divided into three groups acc

Excellent functional outcomes and low complication rate following open reduction for severe pediatric radial neck fractures: A retrospective analysis.

BACKGROUND: Pediatric radial neck fractures, though rare, pose significant management challenges with several treatment options. Open reduction is typically used for more complex cases, where greater fracture severity may contribute to higher complication rates and poorer outcomes. This study aims to evaluate functional outcomes, complications, and risk factors in a large cohort of pediatric patients treated with open reduction, performed after unsuccessful closed reduction. METHODS: A retrospective cohort study was conducted on 53 pediatric patients (mean age 8 years) with radial neck fractu

Early Functional Outcomes of Autologous Fibular Strut Graft Augmentation in Open Reduction and Internal Fixation (ORIF) of Proximal Humerus Fractures.

Background Displaced proximal humerus fractures in elderly patients are associated with high rates of fixation failure due to poor bone quality and medial column comminution. Augmentation of locking plate fixation with an intramedullary fibular strut autograft has been proposed to enhance medial support and improve outcomes. This study evaluated early radiological and functional outcomes following open reduction and internal fixation (ORIF) using a proximal humerus internal locking system (PHILOS) plate augmented with a fibular strut autograft. Objectives The objectives of this study are to ev

Open reduction and internal fixation vs acute total hip arthroplasty for geriatric acetabular fractures: A multicenter matched cohort study.

BACKGROUND: Optimal management of acetabular fractures remains controversial. Open reduction and internal fixation (ORIF) may be followed by post-traumatic degeneration and late conversion arthroplasty, whereas acute total hip arthroplasty (THA) may introduce implant-related risks. We compared short- and long-term outcomes after ORIF versus acute THA in a large, multicenter electronic health record cohort. METHODS: We performed a retrospective cohort study using the TriNetX Network. Adults with isolated, closed, acute acetabular fractures treated with either ORIF or acute primary THA were ide

Tibiotalocalcaneal arthrodesis versus open reduction and internal fixation in treatment of trimalleolar ankle fractures: A propensity-matched TriNetX database study.

BACKGROUND: Trimalleolar ankle fractures are complex injuries associated with high complication rates and poorer outcomes, particularly in elderly and medically comorbid patients. While open reduction and internal fixation (ORIF) remains the standard treatment, alternative strategies such as tibiotalocalcaneal arthrodesis (TTCA) have been increasingly explored for high-risk populations. METHODS: A retrospective cohort study was performed using the TriNetX Research Network. Adult patients undergoing operative treatment for trimalleolar fractures were identified and stratified by procedure (TTC
