[Maxillofacial fractures: midface and internal orbit. Part 2: therapeutic options].

While the state of the art of clinical assessment and imaging techniques were described in Part 1, this contribution presents a systematic review of the surgical treatment principles in the management of midface and internal orbit fractures from initial care to definitive treatment, including illustrative case examples. New developments and advances are characterized by limited versus extended surgical approaches, by standardization of osteosynthesis principles with regard to three-dimensional buttress reconstruction, by newly developed individualized implants such as titanium meshes and, espe…

Clostridium septicum gas gangrene in the orbit: a case report.

Our report presents a case of Clostridium septicum gas gangrene in an unusual, orbital localization. The predisposing factors are typical: colon tumour and lymphatic malignancy. Most probably bacteria from the intestinal flora entered the bloodstream through the compromised intestinal wall and settled in the orbit resulting in the development of an abscess containing gas. At the site of the gas gangrene, an indolent B cell lymphoma was present. After surgery and antibiotic treatment, the patient healed from the C. septicum infection; but subsequently died as a consequence of the tumour.

Aneurysmal bone cyst secondary to ossifying fibroma in the orbit.

A 28-year-old man presented with proptosis and medial displacement of the right eye, intermittent pain, and diplopia of 1 month's duration. Radiologic examination demonstrated a mass in the right lateral orbit resembling an aneurysmal bone cyst (ABC). Histopathologic evaluation of the surgically excised lesion via orbitotomy confirmed the radiologic diagnosis. Two months later, the patient presented again with a mass at the same location as the previous one, with a major component that was solid. The recurrent lesion was excised via frontal craniotomy. Histopathologic examination of the surgic…

Computer-navigated contouring of craniofacial fibrous dysplasia involving the orbit.

Virtual surgical planning and computer-aided surgery were used to treat a mono-ostotic fibrous dysplasia of the right zygoma. Mirroring of the contralateral zygoma sets the target for the contouring of the affected zygomatic bone. An optical system for computer-guided surgery was used. Instruments were calibrated and visualized in real time on screen. Achievement of the virtually set target for the orbitozygomatic anatomy was assessed during surgery. Postoperative computed tomography and clinical follow-up confirmed an excellent result with regard to facial symmetry and eye bulb position. The …

Incidence of calcification of the trochlear apparatus in the orbit.

PURPOSE: To quantify the incidence of asymptomatic incidental trochlear calcifications and to describe their clinical features. METHODS: We retrospectively reviewed orbital computed tomography (CT) scans of 216 patients to identify the presence of trochlear calcifications. We analyzed the prevalence, age distribution, and gender preponderance of trochlear calcifications. We also examined age-specific prevalence rates for trochlear calcifications, as well as their relationship to systemic disease. RESULTS: The mean age of patients was 26.8 years. Trochlear calcifications were observed in 35 (…

Myoepithelial carcinoma of the orbit: a clinicopathological and histopathological study.

Two cases of invasive myoepithelial carcinoma arising from the paranasal sinuses and invading the orbit are presented. Patient 1, a 53-year-old man, had a 3-month history of proptosis, pain and epiphora of the right eye. The second patient, a 24-year-old man, had for a week been complaining of protrusion of his left eye and of orbital pain. Computed tomography scan and magnetic resonance imaging revealed tumour masses in the frontal, ethmoidal and maxillary sinuses with invasion of the orbit and the frontal lobe. Biopsies from both cases showed spindle and epithelioid tumour cells. Mitotic fig…

An unusual presentation of alveolar type of rhabdomyosarcoma of orbit.

Orbital rhabdomyosarcoma is the commonest primary orbital malignancy in children. A case of alveolar type of orbital rhabdomyosarcoma with unusual presentation in a three and half years male child presented with painless nodular swelling in the left lower lid of one and half years duration and gradual protrusion of the left eyeball. Examination of the patient revealed a mass in the left lower lid region extending to other parts of orbit and lymphnode metastasis in the neck region along with non axial proptosis, rest of the systemic examination being normal. Patient underwent incisional biopsy …

Sudoriferous cyst of the orbit of adult origin after trauma.

A rare case of sudoriferous cyst of the orbit occurring in an adult, who had facial trauma, is reported. Several factors suggest its adult onset. The only other case reported in an adult is of presumed childhood origin. Very few congenital cases have been reported. A 65-year-old lady presented with recent onset of left-sided ptosis and a painless mass below the left supraorbital margin. The patient had traumatic ptosis after a road traffic accident 13 years ago. The ptosis was surgically repaired, which resulted in symmetrical palpebral apertures. Computed tomographic scan revealed a well-defi…

Renal medullary carcinoma metastatic to the orbit: a clinicopathologic report.

A 39-year-old black man with sickle cell trait presented with a rapidly progressive, painful proptosis of the left eye. A computed tomographic scan was interpreted as revealing a cavernous hemangioma. Medial orbitotomy revealed a hard, gray mass supranasal to the optic nerve and invading the medial rectus muscle. Intraoperative frozen section specimens were read initially by the pathologist as metastatic adenocarcinoma. On gross examination, the conspicuous hemorrhage, necrosis, and bright yellow color characteristic of renal cell carcinoma were not present. Final pathologic analysis of the or…

[A rare cause of exophthalmos, hydatid cyst of the orbit].

INTRODUCTION: The orbital hydatid cyst is a rare (1 to 2%) localization of the Echinococcus granulosus parasite. OBSERVATIONS: An intra-orbital localization of hydatidosis was revealed in two patients by the occurrence of unilateral exophthalmia. After tumoral resection the diagnosis was confirmed by histological examination. COMMENTS: The principle clinical sign of an orbital hydatid cyst is exophthalmia. Imaging is essential for pre-operative diagnosis. Serology is insufficient and treatment is surgical.

Castleman's disease in the orbit. A 20-year follow-up.

PURPOSE: To report a case of localized orbital Castleman's disease of mixed cell type with a follow-up of 20 years. METHODS: A female patient presented at the age of 12 years with constitutional symptoms and left-sided proptosis. Laboratory tests revealed marked hypergammaglobulinaemia and high erythrocyte sedimentation rate (ESR), suggesting an immunological disturbance. A CT scan and MRI showed an infiltrating orbital mass lateral to and behind the eye. RESULTS: Histological examination of orbital biopsies showed a lymphoid lesion consistent with Castleman's disease of the mixed cell type.…

A conceptual design for cosmo-biology experiments in Earth's Orbit.

A conceptual design was developed for a cosmo-biology experiment. It is intended to expose simulated interstellar ice materials deposited on dust grains to the space environment. The experimental system consists of a cryogenic system to keep solidified gas sample, and an optical device to select and amplify the ultraviolet part of the solar light for irradiation. By this approach, the long lasting chemical evolution of icy species could be examined in a much shorter time of exposure by amplification of light intensity. The removal of light at longer wavelength, which is ineffective to induce p

Radiographic features of an aneurysmal bone cyst of the orbit.

Aneurysmal bone cyst is an unusual benign solitary lesion of bone occurring rarely in the orbit. In this report, we present computed tomography and magnetic resonance images of an orbital aneurysmal bone cyst involving the frontal bone in a 16-month-old boy. The aneurysmal bone cyst exhibited bone destruction, new bone formation of the orbital roof and fluid-fluid levels in the lesion following hemorrhage. These radiographic features may be useful for diagnosing orbital aneurysmal bone cysts.

Hepatocellular carcinoma metastatic to the orbit in an African patient.

Primary hepatocellular carcinoma rarely metastasizes to the orbit. We report a 19-year-old black man from Zimbabwe who had hepatomegaly and an orbital mass. An orbital biopsy suggested a diagnosis of hepatocellular carcinoma. Immunohistochemical stains supported the diagnosis. To our knowledge, this is only the second reported case of hepatocellular carcinoma metastasizing to the orbit and the first such reported case from Africa.

Idiopathic sclerosing inflammation of the orbit. A distinct clinicopathologic entity.

BACKGROUND: Idiopathic sclerosing inflammation of the orbit is a poorly delineated, fibrosing, immune-mediated entity resulting in significant ocular disability. To characterize this process and propose more specific and effective therapy, clinical and pathologic findings in 16 cases are reviewed. METHODS: The clinical records of 16 patients with biopsy-proven disease were retrospectively reviewed to determine demographic and clinical features, radiologic features, course, management, and outcome. These findings were correlated with pathologic features to describe this unique entity. Immunohi…
