Medical Device Regulation: Requirements for Dental Professionals Who Prescribe and Manufacture Custom-Made Devices.

A custom-made device (CMD) is a medical device intended for the sole use of a particular patient. In a dental setting, CMDs include prosthodontic devices, orthodontic appliances, bruxism splints, speech prostheses and devices for the treatment of obstructive sleep apnoea, trauma prevention and orthognathic surgery facilitation (arch bars and interocclusal wafers). Since 1993, the production and provision of CMDs have been subject to European Union (EU) Directive 93/42/EEC (Medical Device Directive, MDD) given effect in the UK by The Medical Devices Regulations 2002 (Statutory Instrument 2002/6

Orthodontic intrusion of periodontally-compromised maxillary incisors: 3-dimensional finite element method analysis.

BACKGROUND: Loading of the compromised periodontium with orthodontic forces produces different results than those achieved in patients with healthy periodontal support. Determining the force value at a level preventing further deterioration of the patient's periodontal status, thus delivering the most precisely individualized "dose" of loading, seems to be crucial for the successful intrusion of teeth with reduced periodontal support. OBJECTIVES: The aim of the study was to determine the range of force values efficiently intruding maxillary incisors without further compromising the initially-

The effects of micro-implant assisted rapid palatal expansion (MARPE) on the nasomaxillary complex--a finite element method (FEM) analysis.

BACKGROUND: Orthodontic palatal expansion appliances have been widely used with satisfactory and, most often, predictable clinical results. Recently, clinicians have successfully utilized micro-implants with palatal expander designs to work as anchors to the palate to achieve more efficient skeletal expansion and to decrease undesired dental effects. The purpose of the study was to use finite element method (FEM) to determine the stress distribution and displacement within the craniofacial complex when simulated conventional and micro-implant-assisted rapid palatal expansion (MARPE) expansion

[The bony deficit in cleft lip and palate: review of procedures. Experience with the tibial periosteal graft].

Surgeons have long been preoccupied with continuity of the bone in the repair of cleft lips and palates. It is no longer necessary to demonstrate the deleterious effect of employing osteoplasty in a first stage procedure; very few practitioners still rely upon it. On the other hand, a great number of authors now advocate a bone graft as a secondary operation, although they have not been able as yet to reach a consensus on its timing, nor upon exactly what its objectives should be. An early bone graft, carried out at the time of the primary dentition, would have as its goal stabilization of the

Restorative pulpal and repair responses.

BACKGROUND: Each year, about 90 million new restorations are placed in the United States and 200 million are replaced. Controversy surrounds the pulpal reactions and frequency of bacterial microleakage associated with common restorative materials. The authors investigated and compared pulpal reactions to different types of restorative materials. METHODS: Two hundred seventy-two teeth with standardized rectangular Class V unexposed cavities were restored with resin-based composite bonded to dentin; resin-based composite bonded to enamel; resin-modified glass ionomers, or RMGI; amalgam lined wi

Evaluation of Maxillary Stability in Simultaneous Downward Repositioning and Expansion With Miniplates, Screws, and Allograft.

Maxillary expansion and downward repositioning are among the least stable movements in orthognathic surgery. However, they are often necessary for functional and aesthetic purposes. Although combining these movements can reduce the need for multiple surgical procedures, it increases the risk of instability and requires stabilization techniques to prevent relapse. A multitude of approaches have been described, yet the literature remains limited in optimal strategies to enhance long-term outcomes. This retrospective study evaluated patients who underwent simultaneous maxillary expansion and down

An innovative method for three-dimensional bone reconstruction of the anterior mandible with preserved dentition using an allogeneic bone block: A 6-month follow-up.

BACKGROUND: Bone defects around the teeth affect a large portion of the population. Bone regeneration in the area of existing teeth is completely different from that in an edentulous area. To date, no method has been developed for three-dimensional (3D) bone reconstruction in regions with preserved teeth. OBJECTIVES: This study aimed to radiologically evaluate the results of the new method of 3D mandibular bone reconstruction in preserved dentition using a custom-made allogeneic bone block with a 6-month follow-up. MATERIAL AND METHODS: Alveolar ridge dimensions were radiographically assesse

Age dependence of the maturation of the midpalatal suture in the stability of orthodontic anchoring screws.

INTRODUCTION: Orthodontic anchoring screws (OASs) have been placed around midpalatal sutures in patients of various ages. Our previous study found that OAS placement more than 1.5 mm from midpalatal suture was more successful than placement directly at the suture. This study aimed to investigate the relationship between age and midpalatal suture maturation, considering factors affecting the failure of OASs using cone-beam computed tomography. METHODS: In total, 150 patients who underwent cone-beam computed tomography were selected. The total depth and sutured depth of the midpalatal suture co
