The incidence of spondylolysis and spondylolisthesis in children with osteogenesis imperfecta.

BACKGROUND: Spondylolysis and spondylolisthesis are common abnormalities of the lumbar spine. The incidence of these diagnoses is recognized in the healthy population. However, their incidence in osteogenesis imperfecta (OI) patients is less well defined. METHODS: This is a retrospective radiographic review of patients treated in the OI clinic from a single institution. Lateral radiographs were reviewed on all available patients to assess the incidence of spondylolysis and spondylolisthesis in this patient population. The morphology of the pedicle and pars interarticularis was also evaluated …

[Effects of zoledronic acid in adults with osteogenesis imperfecta].

BACKGROUND AND OBJECTIVE: Osteogenesis imperfecta (OI) is a genetic disorder that results in bone fragility. Several studies have demonstrated the effectiveness of bisphosphonate therapy. The aim of this study was to evaluate the effects of intravenous zoledronic acid on bone mineral density (BMD) and biochemical markers of bone turnover in adults with OI. MATERIAL AND METHODS: We carried out a prospective non-randomized study in patients with osteoporosis or severe osteopenia (T score < -2) related to OI and intolerance or contraindication to oral bisphosphonates. The patients were treated w…

Potential implications of cell therapy for osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a brittle-bone disease whose hallmark is bone fragility. Since the disease is genetic, there is currently no available cure. Several pharmacological agents have been tried with not much success, except the recent use of bisphosphonates. Stem cells have been suggested as an alternative OI treatment, but many hurdles remain before this technology can be applied for treating patients with OI. This review summarizes what is known at present regarding the application of stem cells to treat OI using animal models, clinical trials using mesenchymal stem cells to treat …

Intramedullary osteosynthesis for fracture associated with osteogenesis imperfecta.

Our aim was to analyse the complications associated with intramedullary stabilisation of the bone fractures which are a common complication of osteogenesis imperfecta. A total of 12 fractures among six individuals with osteogenesis imperfecta were treated by intramedullary stabilisation. The mean age of the patients (three male and three female) was 19.4 years, range 7-42 years. The most common fracture site was the femoral midshaft (seven fractures). After implant removal, one new fracture and one re-fracture occurred. Operative stabilisation of fractures is a safe treatment option for osteog…

Osteogenesis imperfecta in a Nigerian neonate: a case report.

BACKGROUND: Osteogenesis imperfecta is an inherited disease where the basic pathology is of defective collagen. It is a rare disorder with a reported incidence of 1/25000-1/30000. Four variants are recognized clinically, of which type II is the most severe form. Clinically the hall marks of the disease are multiple fractures and osteopenia. OBJECTIVE: This case is being presented because of its rare incidence, and to highlight the radiological features distinguishing it from battered baby syndrome occurring from child abuse. CASE REPORTS: An eight day old Nigeria neonate, with clinico radiol…

Interlocking telescopic rod for patients with osteogenesis imperfecta.

BACKGROUND: Intramedullary fixation with use of a telescopic rod with a T-piece is one of the standard methods for long-bone stabilization in growing children with osteogenesis imperfecta. However, installation and removal of this device can cause substantial damage to the distal joint, which limits its use, especially in the tibia. We devised a modification of the telescopic rod system--the interlocking telescopic rod--in which the obturator is a simple rod with a hole, instead of a T-piece, at its distal end. METHODS: The clinical and radiographic outcomes were evaluated more than two years…

[Anesthetic management of an obese patient with osteogenesis imperfecta].

Osteogenesis imperfecta (OI) is a rare hereditary disorder characterized by an excessive tendency to bone fractures and retarded growth, as well as variable involvement of other connective tissues such as teeth, sclerae, auditory bones and ligaments. A 30-year-old woman (body mass index of 39.6) with OI was scheduled for appendectomy. Because of obesity, abnormal cervical spine mobility and fragile teeth, in addition to the risk of mandibular and facial fractures, general anesthesia including airway management was considered to be difficult and, thus, avoided. Instead, neuraxial blocks were ch…

Two doses of pamidronate in infants with osteogenesis imperfecta.

INTRODUCTION: Current regimens of intravenous pamidronate for infants and children with osteogenesis imperfecta (OI) typically deliver 3-12 mg/kg/year of drug. We wished to ascertain the effect of pamidronate at 6 or 12 mg/kg/year on skeletal health in infants with OI. METHODS: We recruited 12 infants over a period of 4 years. Infants received either 6 or 12 mg/kg/year of pamidronate. Bone outcomes were assessed by skeletal surveys and DXA bone density measurements at baseline and at 12 months. RESULTS: Bone mass increased in both groups. Infants receiving 12 as opposed to 6 mg/kg/year pamid…

Radial head dislocation and subluxation in osteogenesis imperfecta.

BACKGROUND: Upper limb deformity in children with osteogenesis imperfecta may substantially impair function. The aims of this retrospective work were to study the prevalence of radial head malalignment (dislocation or subluxation) in different types of osteogenesis imperfecta and to identify factors linked to it. METHODS: We assessed 489 upper limbs from 254 patients (with a mean age of 9.6 years and including 130 female patients) who had various types of osteogenesis imperfecta. Radiographs representing a single time-point for each patient were assessed for the presence and direction of radi…

Stapes surgery in Japanese patients with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a heterogenous connective tissue disorder. The classical triad of symptoms involves a conductive and/or sensorineural hearing impairment together with a tendency to spontaneous bone fractures and blue sclerae. Between 1993-2004, primary stapes surgery was performed on 14 ears of 11 OI patients who presented with conductive and/or mixed hearing loss. Pathological findings included atrophy and/or fractures of the stapedial crura in combination with thickening and fixation of the stapes footplate and hypervascularity of the promontory mucosa. All the patients with …

Craniofacial features in osteogenesis imperfecta: a cephalometric study.

Osteogenesis imperfecta (OI) is a heterogeneous group of connective tissue diseases that mainly manifest as bone fragility and skeletal deformity. In most families it segregates as a dominant trait and results from mutations in type I collagen genes. In this study we analyzed the size and form of the bony structures in heads of 59 consecutive patients with OI types I, III, or IV (Sillence classification), using lateral radiographs. Paired controls were matched for gender and age. The purpose was to obtain baseline information of craniofacial development in OI patients that have not received bi…

Subdural hematoma as clinical presentation of osteogenesis imperfecta.

Osteogenesis imperfecta is an inherited collagenous disease. The mildest form may present with less severe findings, for example blue sclera, but can also lead to significant handicap such as deafness or multiple bone fractures. We describe an 11-month-old female in whom bilateral chronic subdural hematoma was the leading clinical presentation of osteogenesis imperfecta type I. She was hospitalized due to epileptic seizures caused by these bilateral subdural hematomas without preceding trauma. Osteogenesis imperfecta type I was diagnosed on the basis of clinical and radiologic findings. This c…

Bone turnover markers in patients with osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a heterologous group of rare inherited bone disorders resulting from defect in collagen synthesis or function. In previous studies, bone turnover has been found either increased or low-normal. These contradictory results might result from the study population made of children with prior recent fractures. We measured serum total and bone alkaline phosphatase (total and bone AP) serum osteocalcin (sOC), serum type I collagen C-telopeptide breakdown products (sCTX), urinary free-deoxypyridinoline (ufDPD), and urinary cross-linked N-telopeptides of type I collagen (…

Abnormalities in the cerebral arterial system in osteogenesis imperfecta.

A 9-year-old girl with osteogenesis imperfecta (OI) type 4 was admitted to the hospital because of choreatic movement. Cerebral angiography demonstrated bilateral symmetrical stenosis of the proximal cavernous segment of internal carotid artery, prethrombotic occlusion of the M1 segment of left middle cerebral artery, and moyamoya-like collateral vascular structure arising from the right lenticulostriate arteries. After a literature review, these findings were interpreted as vasculopathic changes secondary to vascular fragility caused by the collagen abnormality in OI.

Vestibular dysfunction in adult patients with osteogenesis imperfecta.

Progressive hearing loss is a major symptom in osteogenesis imperfecta (OI), a genetic brittle bone disease. Vertigo is frequently associated with otosclerosis in which the hearing loss clinically resembles that in OI. Vertigo is also common in basilar impression (BI) found in up to 25% of adult OI patients. In order to evaluate the cause, frequency, and characteristics of vertigo in OI, 42 patients were studied by interview, clinical examination, and audiological examination supplemented with electronystagmography (ENG) and lateral skull radiography. Audiometry showed hearing loss in 25 patie…
