Paget's disease of bone and genetic disorders of RANKL/OPG/RANK/NF-kappaB signaling.

Identification of the RANKL/OPG/RANK/NF-kB (receptor activator of nuclear factor kappa-B ligand / osteoprotegerin) signaling pathway as the … indicate rapid skeletal remodeling. In FEO, osteolysis progresses to fat-filled bone rather than to osteosclerosis. Antiresorptive therapy with bisphosphonates can be effective for each disorder.

The application of bone cement to fill the surgical defect after resection of a giant-cell tumor: A case report.

Background. A case of a large bone defect treated with bone cement filler is discussed, and the usefulness and advantages of this treatment … ring which time no clinical or radiological signs of tumor recurrence were found. There was visible osteosclerosis surrounding the bone cement filler, which is believed to be a prognostically favorable finding, associated with a low risk of recurrence. The functional outcome of treatment was assessed with the Knee Rating Score. The patient gained a knee score of …

Mutation of TBCE causes hypoparathyroidism-retardation-dysmorphism and autosomal recessive Kenny-Caffey syndrome.

The syndrome of congenital hypoparathyroidism, mental retardation, facial dysmorphism and extreme growth failure (HRD or Sanjad-Sakati syndrome; OMIM 241410) is an autosomal recessive disorder reported almost exclusively in Middle Eastern populations. A similar syndrome with the additional features of osteosclerosis and recurrent bacterial infections has been classified as autosomal recessive Kenny-Caffey syndrome (AR-KCS; OMIM 244460). Both traits have previously been mapped to chromosome 1q43-44 (refs 5,6) and, despite the observed clinical variability, share an ancestral haplotype, suggesti…

Measurement of tartrate-resistant acid phosphatase and the brain isoenzyme of creatine kinase accurately diagnoses type II autosomal dominant osteopetrosis but does not identify gene carriers.

Autosomal dominant osteopetrosis type II (ADO2) is typically diagnosed from radiographs, which demonstrate the pathognomonic findings of osteosclerosis and endobone formation. Individuals with ADO2 also have elevated serum levels of tartrate-resistant acid phosphatase (TRAP) and the BB isoenzyme of creatine kinase (CK-BB). In the current study, we tested the utility of these enzymes in making or refuting a diagnosis of ADO2. Furthermore, because ADO2 has incomplete penetrance, we examined whether TRAP and CK-BB were helpful in identifying gene carriers. We studied eight families, measured seru…

Bi-allelic EPHA2 mutations cause dysosteosclerosis by inhibiting osteoclastogenesis and enhancing osteoblastogenesis.

Erythropoietin-producing hepatocellular (EPH) receptors, the largest receptor tyrosine kinase family, have been shown to play a crucial role in normal physiological processes and pathologies. EPHA2, encoded by EPHA2 gene, a member of the EPH receptor family, has been implicated in cataracts and various bone tumors, including osteosarcoma, chondrosarcoma, and bone metastases of breast cancer. However, no association between the EPHA2 gene and genetic skeletal disorders has been reported to date. Here, we identified a Chinese family with a diagnosis of dysosteosclerosis, presenting with signific…

Sclerosing bone dysplasias: leads toward novel osteoporosis treatments.

Sclerosing bone dysplasias are a group of rare, monogenic disorders characterized by increased bone density resulting from the disturbance in the fragile equilibrium between bone formation and resorption. Over the last decade, major contributions have been made toward better understanding of the pathogenesis of these conditions. These studies provided us with important insights into the bone biology and yielded the identification of numerous drug targets for the prevention and treatment of osteoporosis. Here, we review this heterogeneous group of disorders focusing on their utility in the deve

[Bone and joint diseases in children. Abnormal bone mineral density in children].

In recent years, the problem of osteoporosis is increasingly raised in children as well as adults both as a primary disorder and as secondary results to various diseases, medications, and lifestyle issues. On the other hand, molecular mechanisms of pathogenesis have been elucidated in several sclerosing disorders contributing to our understanding of basic biology of bone metabolism. In this chapter, we reviewed the recent progress on the method to evaluate BMD in growing period and etiologies of abnormal BMD in children.

Periostitis ossificans (Garrè's osteomyelitis) radiographic study of two cases.

BACKGROUND: Periostitis Ossificans (PO) is a non-suppurative type of Osteomyelitis, commonly occurring in children and young adults, in mandible. The most common cause for PO is periapical infection of mandibular first molar. Radiographically PO is characterized by the presence of lamellae of newly formed periosteal bone outside the cortex, giving the characteristic appearance of "onion skin". CASE REPORTS: Two male children 11 years of age reported to the Department of Oral Medicine with a painless and persistent bony hard swelling in the mandible, with a short duration (Figs 1, 5). Intraspinal posterior epidural cysts associated with Baastrup's disease: report of 10 patients.

OBJECTIVE: This study was performed to describe the association of posterocentral epidural cysts with interspinous bursal fluid in Baastrup's disease using MRI and interspinous bursography. CONCLUSION: Interspinous bursal fluid in Baastrup's disease can extend into the posterocentral epidural space and cause various degrees of central canal stenosis. Bursography alone or combined with CT allows documentation of abnormal communicating channels between the interspinous bursa and epidural cyst.

Osteopathia striata cranial sclerosis: non-random X-inactivation suggestive of X-linked dominant inheritance.

Osteopathia striata with cranial sclerosis (OS-CS) is a rare syndrome comprising macrocephaly, minor anomalies, conductive hearing loss, and mild mental retardation. The diagnosis is based on radiological findings, including cranial sclerosis and longitudinal striations of metaphyses of long bones. Here we report on 10 new cases of OS-CS, including two sporadic cases and three families, with an excess of affected females (9F/1M). Phenotypic variability was observed in our patients as well as several unusual findings. Hirschsprung disease, Pierre Robin sequence, coronal craniostenosis, and lary

Erdheim-chester disease.

Erdheim-Chester disease (ECD) is a rare histiocytic infiltrative disease, characterized by long bone sclerosis. We describe two confirmed cases of ECD, both of which demonstrate non-malignant retroperitoneal and peri-renal infiltration causing dilatation of the upper renal tracts. The cases are illustrated with contrast studies, computed tomography (CT) and magnetic resonance imaging (MRI). Typical sclerosis of the long bones was apparent on radiography. Both cases have been treated conservatively to date.A brief review of the literature regarding the manifestations of ECD is included. In case

[Nonbacterial osteomyelitis of the clavicle].

Osteomyelitis can be subdivided into a bacterial exogenic (post-traumatic/postoperative), bacterial endogenic (hematogenous) and an abacterial type, including the rare group containing primary chronic sclerosing osteomyelitis, which is typically localized in the clavicle. In a review of the literature, the criteria for the various types of chronic sclerosing osteomyelitis are analyzed. Out of a group of 17 patients with osteomyelitis of the clavicle treated between 1978 and 1996, three cases of primary chronic sclerosing osteomyelitis are demonstrated. In the differential diagnosis, primary ch

Reversibility of skeletal fluorosis.

At two x ray examinations in 1957 and 1967, 17 cases of skeletal fluorosis were identified among long term cryolite workers in Copenhagen. In 1982 four of these patients were alive, eight to 15 years after exposure had ended. Radiographs were obtained, and the urinary fluoride excretion was measured. A similar picture emerged in all four cases: extensive fading of the sclerosis of trabecular bone in ribs, vertebral bodies, and pelvis, whereas cortical bone thickening and calcification of muscle insertions and ligaments remained virtually unchanged. The fluoride excretion was increased in three

X-linked dysosteosclerosis. Four familial cases.

Four Spanish children with clinical and radiological features corresponding to dysosteosclerosis are presented. All the children were male and belonged to the same family. The first known carrier is the maternal grandmother, who had three daughters by three different fathers who in turn had one or more sons with the disease. The four carriers were normal. Consanguinity did not occur in any case and the disorder was transmitted as a sex-linked recessive condition.
