Incidence and Predictors of Readmissions to Non-Index Hospitals After Transcatheter Aortic Valve Implantation in the Contemporary Era in New South Wales, Australia.

BACKGROUND: In Australia, transcatheter aortic valve implantation (TAVI) is only performed in a limited number of specialised metropolitan centres, many of which are private hospitals, making it likely that TAVI patients who require readmission will present to another (non-index) hospital. It is important to understand the impact of non-index readmission on patient outcomes and healthcare resource utilisation. METHOD: We analysed linked hospital and death records for residents of New South Wales, Australia, aged ≥18 years, who had an emergency readmission within 90 days following a TAVI proce

Risk Factors for Readmission Following Same-Day Discharge in Primary Total Hip Arthroplasty.

BACKGROUND: Same-day discharge (SDD) following primary total hip arthroplasty (THA) is increasingly utilized and has been shown to be safe in appropriately selected patients. However, limited data compare whether specific patient risk factors for postoperative readmission exert different magnitudes of influence in the SDD versus in the non-same-day discharge (NSDD) cohorts. Identifying risk factors that disproportionately affect SDD patients may refine SDD selection criteria and improve postoperative safety. METHODS: This retrospective cohort study evaluated 11,245 patients who underwent prim

30-Day Readmission Following Outpatient Transmetatarsal Amputation in the Geriatric Population: An ACS NSQIP Analysis.

Whether secondary to deformity, traumatic injury, infection, neoplasm, or ischemic disease, the transmetatarsal amputation provides a functional means of limb preservation prior to major proximal amputation. With similar readmission rates following inpatient and outpatient surgery, prevention of an unnecessary admission among vulnerable patients, specifically geriatrics, proves beneficial. This investigation examines differences among geriatric patients admitted and not requiring readmission following outpatient transmetatarsal amputation. An ACS NSQIP database analysis following filtering for

Perspectives of Post-Acute Transition of Care for Cardiac Surgery Patients.

Post-acute care (PAC) facilities improve patient recovery, as measured by activities of daily living, rehabilitation, hospital readmission, and survival rates. Seamless transitions between discharge and PAC settings continue to be challenges that hamper patient outcomes, specifically problems with effective communication and coordination between hospitals and PAC facilities at patient discharge, patient adherence and access to cardiac rehabilitation (CR) services, caregiver burden, and the financial impact of care. The objective of this review is to examine existing models of cardiac transitio

Factors associated with readmissions in psychiatric inpatient care: a prospective cohort study based on hospital registers.

BACKGROUND: Readmissions in psychiatric inpatient care may reflect problems in the provision of care, but the underlying factors are not well known. We examined the associations of patient characteristics (sociodemographic factors, diagnoses), treatment (duration, previous episodes, neuromodulation) and ward overload with psychiatric inpatient readmissions and multiple readmissions in Finland. METHODS: We used a routinely collected data pool from one hospital district and followed all 2052 hospitalizations that started in 2018. The outcomes were readmission within 30 days and one year, and am…

National Rates, Causes, Risk Factors, and Outcomes Associated With 30-Day and 90-Day Readmissions Following Degenerative Posterior Cervical Spine Surgery Utilizing the Nationwide Readmissions Database.

BACKGROUND: Hospital readmissions have profound financial and clinical impacts. Analyses of 30-day readmissions following spine surgery have been previously reported utilizing administrative databases. However, time periods outside the initial 30 days have not been well studied. Furthermore, these databases have limitations regarding coding and institutional crossover. OBJECTIVE: The authors sought to analyze 30-day and 90-day readmission rates and risk factors using the Nationwide Readmissions Database (NRD) in a retrospective cohort receiving elective, posterior cervical spine surgery for d

Characterizing the Cost Trends of Readmissions After Aseptic Revision Total Knee Arthroplasty.

BACKGROUND: A rise in total knee arthroplasty (TKA) volume has led to an increased incidence of aseptic revision TKA (rTKA), resulting in major associated health care costs. This study evaluated the economic impact and clinical factors associated with readmission following aseptic rTKA, aiming to identify key drivers of cost and risk factors for readmission. METHODS: This retrospective analysis used data from a large national database from January 1, 2009, to June 30, 2022. Patients undergoing aseptic rTKA were identified via Current Procedural Terminology codes. Readmissions within 90 days p

Factors Affecting Length of Stay, Readmission, and Revision After Shoulder Arthroplasty: A Population-Based Study.

BACKGROUND: Increased length of hospital stay, hospital readmission, and revision surgery are adverse outcomes that increase the cost of elective orthopaedic procedures, such as shoulder arthroplasty. Awareness of the factors related to these adverse outcomes will help surgeons and medical centers design strategies for minimizing their occurrence and for managing their associated costs. METHODS: We analyzed data from the New York Statewide Planning and Research Cooperative System on 17,311 primary shoulder arthroplasties performed from 1998 to 2011 to identify factors associated with extended

Diverticulitis in Pregnancy Is Associated with High Complication Rates.

BACKGROUND: National trends toward earlier onset of diverticulitis combined with more advanced maternal age may converge, making diverticulitis an increasingly important etiology of obstetric abdominal infection. However, there is little data on this topic, and no specific guidelines on how to treat pregnant patients with the disease. The purpose of this study was to identify the frequency of hospitalization for diverticulitis in pregnant patients and the common complications associated with this disease process. METHODS: We conducted a retrospective cohort study using the National Readmissio

A review regarding the article 'clinical and echocardiographic outcomes of patients undergoing transcatheter edge-to-edge repair for functional vs degenerative mitral valve regurgitation'.

Echocardiography is widely available in most cardiac centers, plays a key role in both the diagnosis and management of IE, is often the first-hand imaging modality, and should be performed immediately when mitral regurgitation is suspected. In addition, it is helpful during therapy and after surgery to assess valve morphology and function, complications, and heart function. In particular, transthoracic echocardiography is useful to detect new silent complications, monitor vegetation size, assess perivalvular abscess formation, pseudoaneurysm, intracardiac fistula and valvular perforation, as w

Randomised controlled trial of a prognostic assessment and management pathway to reduce the length of hospital stay in normotensive patients with acute pulmonary embolism.

BACKGROUND: The length of hospital stay (LOS) for acute pulmonary embolism (PE) varies considerably. Whether the upfront use of a PE prognostic assessment and management pathway is effective in reducing the LOS remains unknown. METHODS: We conducted a randomised controlled trial of adults hospitalised for acute PE: patients were assigned either to a prognostic assessment and management pathway involving risk stratification followed by predefined criteria for mobilisation and discharge (intervention group) or to usual care (control group). The primary end-point was LOS. The secondary end-point
