Management of anxiety using eye movement desensitization and reprocessing therapy in children undergoing extraction: a randomized controlled pilot study.

BACKGROUND: Eye movement desensitization and reprocessing (EMDR) therapy has been reported to be very efficacious for treating post-traumatic stress disorder (PTSD) and other anxiety-related conditions. However, a review of the literature reveals the sparse use of this therapy in the field of pediatric dentistry. This study aimed to evaluate anxiety trends in pediatric dental patients during local anesthesia and extraction with and without EMDR therapy. METHODS: Children in the age range of 8-12 years who required dental extractions were assigned randomly into two groups: an EMDR group (group

Influence of hospital and patient location on early postoperative outcomes after appendectomy and pyloromyotomy.

BACKGROUND: The effects of hospital location and designation on postoperative pediatric outcomes remain unclear. We hypothesized that urban hospital outcomes would be superior to rural hospitals, and that outcomes at urban centers would differ for children from rural versus urban counties. METHODS: Retrospective cohort study of children undergoing appendectomy (n=129,507) and pyloromyotomy (n=13,452) using the 2006/2009 KID databases. Hospitals were characterized by specialty designation and classified as urban/rural. County of residence was classified as urban/rural. Outcomes included compli

The role of capnography in improving respiratory safety during pediatric endoscopic procedures: An observational study.

Endoscopic procedures in pediatric patients are increasingly performed under sedation in non-operating room anesthesia settings, where early detection of respiratory compromise is essential. While capnography is recommended for identifying hypoventilation, its clinical impact remains debated. This study aimed to evaluate whether end-tidal carbon dioxide (ETCO2) monitoring reduces the incidence of hypoxia during sedated pediatric endoscopic procedures. This observational study included 135 children aged 1 to 17 years undergoing elective upper gastrointestinal endoscopy or combined endoscopy and

[The potentials of virtual reality in pediatric perioperative care].

Each year, millions of children worldwide undergo general anesthesia for various surgical or diagnostic procedures. In Hungary, approximately 20,000 to 30,000 children are treated in pediatric surgical centers annually, and additional 5,000 to 10,000 children undergo operation in adult otolaryngology departments. Treating children is a challenge, as their cognitive and emotional development levels may pose difficulties for them in understanding the procedures ahead and may make the processing of the stressful experiences of the intervention uncertain. Preoperative anxiety can often lead to inc

Risk factors for pediatric emergence delirium: a systematic review.

PURPOSE: Emergence delirium (ED) impacts approximately 25% of pediatric patients undergoing general anesthesia and is associated with with concerning consequences, including injury to patients and health care personnel. Risk prediction models with good usability for pediatric ED are lacking. We aimed to identify published risk factors in this systematic review. METHODS: We conducted a systematic literature search of prospective observational studies and clinical trials through eight major databases from inception to 11 January 2023. We included prospective studies published in English that ex

Incidence of Unintentional Intraoperative Hypothermia in Pediatric Scoliosis Surgery and Associated Preoperative Risk Factors.

INTRODUCTION: Intraoperative hypothermia is associated with perioperative complications such as blood loss and wound infection. Thus, perioperative heat retention methods to prevent perioperative hypothermia such as providing a warmed blanket and active patients' warming are important. Although major surgery and pediatric patient age are noted as risk factors, only a few studies focus on hypothermia as an intraoperative complication in pediatric scoliosis surgery. The aim of this study is to investigate the incidence of intraoperative hypothermia in pediatric scoliosis surgery and the associat

Pediatric Perioperative Pain Management.

Effective perioperative pain control in pediatric patients undergoing orthopedic surgery remains a challenge. Developing a successful pain control regimen begins preoperatively with assessment of the patient and discussion with the patient and family regarding expectations. Perioperative pain control regimens are customized based on the type of surgery, patient characteristics, and anticipated severity and duration of the postoperative pain. Recent study focuses on multimodal strategies and regional anesthesia options, allowing for decreased opioid use. This article provides an evidence-based

[The history of pediatric plastic surgery].

The history of pediatric plastic surgery is linked to that of paediatrics. Until the early 19th century, there was no children's hospital. Only some operations were performed before the discovery of anesthesia, aseptic and antisepsis: cleft lip repair, amputation for polydactyly. Many operations were described in the 19th century for cleft lip and palate repair, hypospadias, syndactylies. The first operation for protruding ears was performed in 1881. Pediatric plastic surgery is diversified in the 2nd half of the 20th century: cleft lip and palate, burns, craniofacial surgery, hand surgery bec

Opinion: No Longer Children, Not Quite Typical Adults-Building a Unified, Collaborative Model of Perioperative Care for Patients With Adult Congenital Heart Disease.

Advances in congenital heart disease care have resulted in a rapidly expanding population of adolescents and adults with adult congenital heart disease (ACHD) who increasingly present for noncardiac surgery. These patients often possess complex physiology that does not align neatly with traditional pediatric or adult perioperative frameworks, creating challenges in care continuity, risk stratification, and perioperative planning. Although guidelines recommend involvement of anesthesiologists with ACHD expertise for higher-risk patients, access to specialized centers and dual-trained providers

Magnet-Assisted Transanal Removal of a Rectal Foreign Body in a Child: A Case Report.

Management of rectal foreign bodies, uncommon in children, aims to achieve safe, minimally invasive removal while avoiding anesthesia or surgery whenever possible. We report a rare pediatric case of a rectal foreign body successfully removed using a magnet-assisted transanal technique. To our knowledge, this technique has not been previously reported in pediatric patients. A 10-year-old girl with a developmental disability inserted an AAA battery into her rectum. She was asymptomatic on arrival. Pelvic radiography revealed a single cylindrical rectal foreign body. Considering the object type,

Weighing the Risk Profile of Cervical Spine MRI in Evaluating Pediatric Cervical Spine Injuries.

OBJECTIVE: Screening for cervical spine injury after blunt trauma is common, but there remains varied practice patterns and clinical uncertainty regarding adequate radiographic evaluation. An oft-cited downside of MRI is the added risk compared to CT in the pediatric population; however, these specific risks have not yet been reported. This study examines the risks of cervical spine MRI in pediatric trauma patients in the context of what value MRI adds. METHODS: This was a retrospective observational study of all pediatric blunt trauma patients who were evaluated with a cervical spine MRI ove
