Microbacterium resistens Peritoneal Dialysis-Associated Peritonitis: A Case Report.

Peritoneal dialysis (PD)-associated peritonitis is most commonly caused by Gram-positive organisms; however, infections due to unusual pathogens can present important diagnostic and management challenges. Microbacterium resistens is an environmental Gram-positive bacillus rarely implicated in human infections, and PD-associated peritonitis due to this organism has been reported only once previously, to our knowledge. We report a case of persistent PD-associated peritonitis caused by M resistens in a 65-year-old man receiving automated PD combined with once-weekly hemodialysis. The initial epis…

Nontuberculous Mycobacterium Peritonitis in Patients on Peritoneal Dialysis: A Scoping Review.

Early and accurate identification of causative microorganisms is essential for improving outcomes in peritoneal dialysis (PD)-associated peritonitis. However, nontuberculous mycobacterial (NTM) peritonitis remains difficult to diagnose and manage, often resulting in delayed treatment and unfavorable clinical outcomes. We conducted a scoping review to summarize the clinical features, microbiological profiles, treatment strategies, and outcomes of PD-associated NTM peritonitis. A total of 107 patients from 81 published reports were identified, including one patient treated at our institution. Th…

Intramuscular fat content and the risk of peritonitis in peritoneal dialysis patients.

BACKGROUND: Sarcopenia is a common and serious complication of dialysis patients. In addition to the reduction in total muscle mass, fat inf … . The relation with functional assessment, bioimpedance spectroscopy (BIS) parameters, survival and peritonitis rate were analyzed. RESULTS: L3 total muscle area and IMAT% had opposite but both significant correlation with functional scores (Morse Fall Scale and Norton Scale) and lean tissue mass measured by BIS, but IMAT% did not have correlation with adipo …

Fungal peritonitis in peritoneal dialysis patients: a nationwide cohort study.

BACKGROUND: Fungal peritonitis is a rare but serious complication of peritoneal dialysis (PD), associated with high morbidity, mortality, and technique failure. This study aimed to evaluate the clinical course, risk factors, and outcomes of fungal peritonitis in adult PD patients. METHODS: We conducted a nationwide retrospective study including all cases of PD-associated fungal peritonitis identified over the past decade in medical centers across Israel. Clinical, microbiological, and outcome data were analyzed. RESULTS: Forty patients were identified, with a median age of 72.5 years. The mo…

Recurrent peritonitis due to Shewanella putrefaciens in a peritoneal dialysis patient.

Shewanella putrefaciens is a gram-negative, nonfermenting, and motile bacillus. It can be found in freshwater, saltwater, and soil. This case report presents a 36-year-old man referred from the nephrology service with recurrent secondary peritonitis associated with peritoneal dialysis. Multidrug-resistant S. putrefaciens was isolated from peritoneal fluid. The patient showed improvement after Piperacillin/tazobactam treatment. Preventive measures against nosocomial infections should be comprehensive, including aseptic techniques, regular monitoring, appropriate antibiotic treatment, and timely…

Prescript peritoneal dialysis: Patterns of iron prescribing in peritoneal dialysis.

BackgroundIron deficiency and anaemia are prevalent in people undergoing peritoneal dialysis (PD), necessitating effective iron supplementation. While oral iron is often preferred for its accessibility and cost, it may be insufficient or poorly tolerated. Intravenous (IV) iron is generally well tolerated and associated with improved haemoglobin response and reduced erythropoiesis-stimulating agent requirements, yet optimal dosing, administration intervals, and haematological targets remain under-researched, particularly in PD populations. Current prescribing practices vary significantly, refle

Peritoneal Immunosurgery: Immunotherapy Augmented Surgery for the Treatment of Peritoneal Cancers.

Peritoneal malignancy often indicates disruptions in multiple physiological systems resulting from widespread cancer. The heterogenous origin and dynamic nature of peritoneal cancer make it difficult to treat with standard approaches that fit into guidelines. We describe how successful treatment should address the underlying pathology, the systemic response to surgical treatments and target the immune perturbations that facilitate the establishment and propagation of this multifaceted disease.

Factors influencing diagnostic imaging in peritoneal dialysis-associated peritonitis.

BackgroundPeritoneal dialysis (PD)-associated peritonitis is a major complication in PD and may require abdominal imaging to identify the intra-abdominal pathology, though its clinical utility remains unclear.MethodsThis retrospective, single-center study included all episodes of PD-associated peritonitis that occurred between January 2013 and July 2024. The primary objective was to identify factors predicting the use of abdominal imaging during peritonitis episodes.ResultsA total of 691 episodes of peritonitis occurred in 376 PD patients during the study period. Of these, 354 episodes (51%) w…
