["O how frightfully..." - Anxiety (not only) around christmas].

Despite its cultural association with peace and joy, the Christmas season can act as a psychosocial stressor, triggering or exacerbating various fears or anxiety-related disorders. This article reviews the psychological and somatic dimensions of anxiety in the context of the holiday period, drawing from literature, art, and clinical observations. Specific phobias (e.g., santaclausophobia, decidophobia, dendrophobia), social anxiety disorder, and generalized anxiety disorder are discussed related to seasonal triggers. From a pathophysiological perspective, anxiety can induce dyspnea, tachycardi

Threat Acquisition and Extinction Differences Between Patients With Panic Disorder or Specific Phobia and Non-Clinical Controls: A Systematic Review.

The study of threat conditioning and extinction processes in anxiety disorders (AD) may further our understanding of the genesis, maintenance, and treatment of these conditions. As it stands, there have been multiple systematic reviews carried out in this area. Patient-control differences in threat acquisition and extinction have been investigated in relation to ADs, obsessive-compulsive disorder (OCD), and social anxiety disorder (SAD). However, this remains to be investigated in either panic disorder (PD) or specific phobia (SP). In this paper, a narrative systematic review was carried out t

Is disgust more resistant to extinction than fear? A meta-analytic review of laboratory paradigms.

Disgust can be acquired via evaluative conditioning; a process by which a neutral stimulus (conditioned stimulus; CS) comes to be evaluated as disgusting due to its pairing with an inherently disgusting stimulus (unconditioned stimulus; US). Research has shown that conditioned disgust responses are resistant to extinction which may have implications for disorders (i.e., contamination-based obsessive-compulsive disorder, specific phobias, and post-traumatic stress disorder) in which heightened disgust has been implicated. Importantly, extinction is the primary mechanism by which exposure therap

State of the Science: Disgust and the Anxiety Disorders.

Anxiety disorders have long been conceptualized as disorders of fear, while other emotions have largely been overlooked. However, an emerging literature has increasingly implicated disgust in certain anxiety-related disorders, including obsessive-compulsive disorder, specific phobias (e.g., spider phobia), health anxiety, and posttraumatic stress disorder. Roughly two decades of research has accumulated evidence identifying various mechanisms linking disgust-related phenomena to these disorders. In the present "State of the Science" review, we sought to summarize the current state of the liter

β-Blockers for the Treatment of Misophonia and Misokinesia.

Misophonia is an adverse physical and emotional reaction to certain repetitive trigger sounds, usually generated by other people. Misokinesia refers to visual triggers that are sometimes (but not always) related to trigger sounds. Despite how common and disabling these conditions can be, medication treatment of misophonia and misokinesia is largely unexplored. We present the first case of using a β-blocker (propranolol) to successfully treat a patient experiencing misophonia and misokinesia. A moderate dose (60 mg) of propranolol completely eliminated multiple auditory and visual trigger sympt

Mood and anxiety profiles differentially associate with physical conditions in US adolescents.

BACKGROUND: Mood and anxiety are widely associated with physical conditions, but research and treatment are complicated by their overlap, clinical heterogeneity, and manifestation on a spectrum rather than as discrete disorders. In contrast to previous work relying on threshold-level disorders, we examined the association between empirically-derived profiles of mood and anxiety syndromes with physical conditions in a nationally-representative sample of US adolescents. METHODS: Participants were 2,911 adolescents (aged 13-18) from the National Comorbidity Survey-Adolescent Supplement who provi

Virtual Reality-Enhanced Extinction of Phobias and Post-Traumatic Stress.

Virtual reality (VR) refers to an advanced technological communication interface in which the user is actively participating in a computer-generated 3-dimensional virtual world that includes computer sensory input devices used to simulate real-world interactive experiences. VR has been used within psychiatric treatment for anxiety disorders, particularly specific phobias and post-traumatic stress disorder, given several advantages that VR provides for use within treatment for these disorders. Exposure therapy for anxiety disorder is grounded in fear-conditioning models, in which extinction lea

Specific phobia: a review of DSM-IV specific phobia and preliminary recommendations for DSM-V.

The present review was conducted in order to evaluate the current diagnostic criteria for specific phobia (SP) in light of the empirical evidence gathered since DSM-IV and to propose changes to DSM-V where change is clearly and reliably indicated by the evidence. In response to questions put forth by the DSM-V Anxiety, OC Spectrum, Posttraumatic, and Dissociative Disorder Work Group, four primary areas were determined for this review: the accuracy and utility of the current SP type classification system, the validity of test anxiety as a type of SP, the boundary between agoraphobia and SP, and

A contemporary learning theory perspective on the etiology of anxiety disorders: it's not what you thought it was.

The authors describe how contemporary learning theory and research provide the basis for perspectives on the etiology and maintenance of anxiety disorders that capture the complexity associated with individual differences in the development and course of these disorders. These insights from modern research on learning overcome the shortcomings of earlier overly simplistic behavioral approaches, which sometimes have been justifiably criticized. The authors show how considerations of early learning histories and temperamental vulnerabilities affect the short- and long-term outcomes of experience

Imagined halitosis: a social phobia symptom?

Imagined halitosis is poorly documented in the psychiatric literature. It is perhaps best thought of as a symptom rather than as a specific syndrome (a collection of symptoms that co-vary together). Many of the cases with imagined halitosis described in the literature resemble the psychiatric syndrome of social phobia.

Natural history of phobic anxiety.

The characteristics of those with phobic anxiety--anxiety with or without avoidance of specific fear-provoking situations--seen in family practice have not been previously described. However, patients with agoraphobia--extreme avoidance of fear-provoking situations--are frequently seen in the general health sector. In addition, panic attacks, which are spontaneous episodes of intense anxiety associated with at least four autonomic symptoms that build rapidly in intensity, are believed to be antecedent to phobic anxiety. This secondary analysis used 29 patients with panic attacks. During a stru

Cognitive therapy for anxiety disorders.

In summary, we conclude that current evidence supports cognitive therapy as a promising approach to the treatment of anxiety. However, definitive conclusions regarding its efficacy must await large-scale, well-conducted, controlled trials that address the methodological shortcomings previously addressed. Furthermore, although there are many components to Beck's version of cognitive therapy, the critical ingredients are not known at this time. Despite the absence of data regarding this issue, we believe that future trials of cognitive therapy should include the following as core components: (a)

Marital therapy and spouse involvement in the treatment of depression, agoraphobia, and alcoholism.

This article examines the literature on marital therapy and spouse involvement as treatments for major psychopathology. The focus is on three disorders that have attracted a sufficient number of empirical findings: depression, agoraphobia, and alcoholism. For each of these disorders, we first examine the relation between marital dynamics and the disorder and next discuss empirical efforts to evaluate the impact of marital therapy or spouse involvement on the disorder. In addition, we summarize trends and suggest methodological refinements for future research.

[Is anxiety hereditary?].

Familial aggregation of anxiety disorders has repeatedly been reported: the estimated risk among the first degree relatives is of 15% to 18% compared to 3% in control groups. The most recent studies are focused on more homogeneous clinical subgroups. Thus, among agoraphobics first degree relatives, the risk is elevated not only for agoraphobia but also for panic attacks and other phobias. In families of panic attacks, the risk of panic attacks in the first degree relatives is specifically elevated. Thanks to the extremely fast development of genetic linkage and molecular genetics, progress is
