Eligibility for Early Mobilization in Mechanically Ventilated Adults: A Multicenter Study.

RATIONALE: Early mobilization of mechanically ventilated patients within 72 hours of intubation is underutilized despite its known clinical benefits. The most cited barrier to implementation is physiologic instability. OBJECTIVES: To quantify eligibility for early mobilization across established physiologic criteria in a multicenter cohort of mechanically ventilated adults and estimate the relative impact of structural and process implementation barriers. METHODS: Retrospective cohort of adults receiving invasive mechanical ventilation for ≥4 hours across nine CLIF Consortium health systems

HRV-Based Surrogate Classification in Surgical Environments Using Personalised Labelling and Sequence Modelling.

Accurate monitoring of stress and workload in surgical environments is essential for improving performance and patient safety. Physiological signals, particularly heart rate variability (HRV), offer a promising approach for objective and continuous stress assessment. However, existing studies often rely on population-level models, limited temporal analysis, and evaluation strategies that may not reflect real-world generalisation. This study proposes an end-to-end pipeline for HRV-based classification of HRV-derived surrogate stress-like states in surgical settings, combining personalised featu

The FreeD module's lateral translation timing in the gait robot Lokomat: a manual adaptation is necessary.

BACKGROUND: Pelvic and trunk movements are often restricted in stationary robotic gait trainers. The optional FreeD module of the driven gait orthosis Lokomat offers a combined, guided lateral translation and transverse rotation of the pelvis and may therefore support weight shifting during walking. However, from clinical experience, it seems that the default setting of this timing does not correspond well with the timing of the physiological pelvic movement during the gait cycle. In the software, a manual adaptation of the lateral translation's timing with respect to the gait cycle is possibl

Neurophysiological markers associated with heterogeneity in conduct problems, callous unemotional traits, and anxiety: Comparing children to young adults.

Evidence from physiological studies has been integral in many causal theories of behavioral and emotional problems. However, this evidence is hampered by the heterogeneity characterizing these problems. The current study adds to prior work by identifying neuro-physiological markers associated with heterogeneity in conduct problems (CP), callous-unemotional (CU) traits, and anxiety. Participants were classified into the following groups: (a) low risk, (b) anxious (predominately high anxiety), (c) primary (scored high on CP and CU traits but low on anxiety), and (d) secondary (high anxiety, CU t

Coronary artery disease management in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: The optimal management of coronary artery disease in patients with severe aortic stenosis undergoing transcatheter aortic valve implantation (TAVI) remains uncertain. AIMS: We aimed to compare clinical outcomes between routine coronary revascularisation and conservative management in patients undergoing TAVI and to assess the relative effectiveness of individual revascularisation strategies by performing a meta-analysis of randomised trials. METHODS: We conducted a pairwise meta-analysis comparing percutaneous coronary intervention (PCI) with conservative treatment and a network

Comparative effects of amylin and cholecystokinin on food intake and gastric emptying in rats.

CCK is a physiological inhibitor of gastric emptying and food intake. The pancreatic peptide amylin exerts similar actions, yet its physiological importance is uncertain. Objectives were to compare the dose-dependent effects of intravenous infusion of amylin and CCK-8 on gastric emptying and food intake in rats, and to assess whether physiological doses of amylin are effective. Amylin and CCK-8 inhibited gastric emptying with mean effective doses (ED(50)s) of 3 and 35 pmol x kg(-1) x min(-1) and maximal inhibitions of 60 and 65%, respectively. Amylin and CCK-8 inhibited food intake with ED(50)

[High-dose thalamonal-rohypnol for premedication. A randomized double-blind study].

The combination of fentanyl and droperidol, Innovar, was compared to flunitrazepam (1-2 mg) in a higher dosage (2.5-5 ml) for i.m. premedication. We measured psychological (ESB) and physiological (blood pressure, heart rate, p.cortisol) stress parameters before and after premedication. Side effects were registered. The day after surgery the patients were asked about the quality of premedication. Flunitrazepam reduced anxiety; Innovar did not. After Innovar the feeling of weakness increased significantly compared to flunitrazepam. Two patients panicked and refused operation. Other severe side e

[Physiology and pathophysiology of subarachnoid and epidural block (author's transl)].

The most important patho-physiological changes following subdural or epidural blockade are seen in the central nervous system and cardiovascular system. Preganglionic sympathetic blockade is the main reason for these changes. Sympathetic blockade is followed by a decrease in peripheral vascular resistence and alterations in the arterial and venous circulation. Hypotension occurs frequently and is thus of great interest in clinical anaesthesia. Differences in the degree of nerve block are not only seen when comparing spinal and peridural blockade but also relate to the different pharmacodynamic

Effects of physiologic pacing versus ventricular pacing on the risk of stroke and death due to cardiovascular causes. Canadian Trial of Physiologic Pacing Investigators.

BACKGROUND: Evidence suggests that physiologic pacing (dual-chamber or atrial) may be superior to single-chamber (ventricular) pacing because it is associated with lower risks of atrial fibrillation, stroke, and death. These benefits have not been evaluated in a large, randomized, controlled trial. METHODS: At 32 Canadian centers, patients without chronic atrial fibrillation who were scheduled for a first implantation of a pacemaker to treat symptomatic bradycardia were eligible for enrollment. We randomly assigned patients to receive either a ventricular pacemaker or a physiologic pacemaker

The Relationship between Population T4/TSH Set Point Data and T4/TSH Physiology.

Context. Population studies of the distribution of T4/TSH set points suggest a more complex inverse relationship between T4 and TSH than that suggested by physiological studies. The reasons for the similarities and differences between the curves describing these relationships are unresolved. Methods. We subjected the curve, derived from empiric data, describing the TSH suppression response to T4, and the more mathematically derived curve describing the T4 response to TSH, to the different possible models of population variation. The implied consequences of these in terms of generating a popula

Birth interventions and outcomes among first time mothers: A population-based register study in four Nordic countries.

INTRODUCTION: Although the Nordic countries generally report lower caesarean section rates compared with middle- and high-income settings, caesarean section represents only one dimension of obstetric intervention. Limited evidence exists regarding the prevalence of physiological birth across the Nordic region. We aimed to describe and compare cross-country variations in intrapartum intervention rates and birth-related complications among nulliparous women in Finland, Iceland, Norway, and Sweden. METHODS: This Nordic observational study used data from the Finnish, Icelandic, Norwegian, and Swe

Comparison of Efficacy and Willingness to a 5% Gluconolactone-Based Topical Serum and Intense Pulsed Light in Mild Erythema of Rosacea: A Paired Control Study.

INTRODUCTION: Mild erythema is a common affliction of rosacea. Many methods including drugs and phototherapy (e.g., Intense Pulse Light, IPL) have been tried to alleviate symptoms. Topical applications for enhancing moisturization have also been thought to be of some benefit. In this study, the improvement of erythema and skin physiology parameters were assessed after a topical serum (5% gluconolactone-based serum) usage or a single IPL treatment, and the differences in improvement between the two groups were compared. The treatment willingness of the subjects was also performed. METHODS: In

Detection and cross-organ characterization of physiological response to microplastic stress in Panax ginseng based on hyperspectral imaging assisted with machine learning.

Microplastic pollution can affect growth and quality of medicinal plants, yet rapid detection of microplastic stress responses remains underexplored. We treated ginseng with polyethylene microplastics, acquired leaf hyperspectral images (HSI) on day 23, and constructed machine learning models for identifying stress levels and predicting physiological indicators. Furthermore, the applicability of successive projections algorithm (SPA) and competitive adaptive reweighted sampling (CARS) for characteristic wavelength selection was compared. Results showed that polyethylene stress significantly af
