BACKGROUND AND HYPOTHESIS: Platelet inhibitor therapy is a relative contraindication to percutaneous kidney biopsies due to concerns about bleeding risk. Here we present an analysis of a prospective observational study on the risk of complications after native and transplant kidney biopsy with and without platelet inhibition.
METHODS: 1103 consecutive biopsies between 2020 and 2025 at a single tertiary academic care center were included (26.0% in transplanted kidneys). Complications after biopsy were assessed and recorded before discharge, which was the day after the biopsy unless complicatio

In the ED, the patient requested administration of platelet transfusion but was refused due to a normal platelet count. During the course of his hospital stay, he complained of epistaxis and was noted to have a downtrending hemoglobin from 11.0 g/dl to 9.0 g/dl. The patient and his family were not comfortable when the d …

Glanzmann thrombasthenia (GT) is a rare autosomal recessive platelet function disorder resulting from qualitative or quantitative defects in the GPIIb/IIIa (integrin αIIbβ3) complex. Patients typically present with mucocutaneous bleeding. Standard treatment involves antifibrinolytic agents and platelet transfusions; however, repeated transfusions may lead to alloimmunization and platelet refractoriness. Recombinant activated factor VIIa (rFVIIa; NovoSeven) offers an alternative hemostatic approach. We present two pediatric cases of GT managed with rFVIIa. The first case involves a 12-year-old …

Background/Objectives: Thrombocytopenia increases bleeding tendency and may complicate perioperative management in total knee arthroplasty (TKA), yet data on both in-hospital burden and post-discharge adverse events are limited. This study evaluated the association between preoperative platelet count and outcomes after primary TKA. Methods: In total, 3474 primary TKAs performed for osteoarthritis at a single institution (2008-2023) were retrospectively reviewed. Patients were classified as Normal, Mild, Moderate, or Severe by preoperative platelet count; the Severe group (N = 6) was excluded f

BACKGROUND: The timely administration of platelet transfusions is critical for patient survival, and the clinical demand for platelet transfusions has been steadily increasing. However, platelet storage lesions (PSLs) that develop during in vitro preservation exacerbate these shortages. The PSL is significantly influenced by various factors, including temperature, gas composition, and buffering systems. Strategies to mitigate PSLs and improve platelet storage have been actively explored in recent years.
OBJECTIVES: This study aimed to investigate whether elevated carbon dioxide (CO2) levels i…

BACKGROUND: Obstetric hemorrhage is the leading cause of maternal mortality globally, especially in China. The key to a successful rescue is immediate and rapid blood transfusion. Autotransfusion has become an integral part of clinical blood transfusion, with intraoperative cell salvage (IOCS) being the most widely used.
AIM: To investigate the application of IOCS in cesarean section.
METHODS: A total of 87 patients who underwent cesarean section and blood transfusion in our hospital from March 2015 to June 2020 were included in this prospective controlled study. They were divided into the o

Glanzmann's thrombasthenia (GT) is an autosomal recessive disorder of platelet function. Conventional management is by platelet transfusion, given before invasive interventions. Alloimmunization resulting in platelet refractoriness and an unpredictable response to platelet infusion have provided particular management difficulties in the past. More recently recombinant (r)VIIa (Novoseven) has a valuable role in the treatment of platelet function disorders. Treatment of a patient with GT during two pregnancies and spinal surgery is reported. An algorithm is presented to provide a structured and …

Platelet transfusion is indispensable in clinical care for hemostatic disorders. During storage, platelets undergo progressive physiological and biochemical deterioration known as platelet storage lesions (PSL), which compromise quality, shelf life, and transfusion effectiveness. This study aimed to profile long noncoding RNA (lncRNA) expression in stored platelets and evaluate their associations with storage duration, donor gender, blood type, and lifetime apheresis donation frequency, to identify candidate biomarkers for PSL. We examined expression of GAS5, LIPCAR, MALAT1, and SNHG9 and thei…

Platelet transfusion requirements are higher among patients with veno-occlusive disease (VOD), compared to patients without VOD. One possible explanation is inadequate production of thrombopoietin (TPO), a protein synthesized in the liver. We prospectively studied 28 patients to test the hypothesis that plasma TPO levels were decreased in patients who developed VOD. Plasma TPO levels to day +30 were measured by ELISA (normal, 0.36+/-0.15 fmol/ml). VOD developed in 18/28 patients. Platelet transfusion requirements were significantly different in patients with and without VOD (97+/-46.1 units vs…

BACKGROUND AND OBJECTIVES: Even with the introduction of specific risk-reduction strategies, transfusion-related acute lung injury (TRALI) continues to be a leading cause of transfusion-related morbidity and mortality. Existing small animal models have not yet investigated TRALI resulting from the infusion of heat-treated supernatant from whole blood platelet concentrates. In this study, our objective was the development of a novel in vivo two-event model of TRALI in sheep.

A 68-year-old man with Glanzmann's thrombasthenia suffered from recurrent cryptogenic bleeding originating in the upper duodenal flexure. Extensive endoscopic procedures and medical treatments were unsuccessful and proximal duodenectomy was proposed. In preceding years platelet transfusions had often had a suboptimal result and were complicated by allergic reactions. Surgery was carried out while he was being treated with recombinant Factor VIIa. Neither major blood loss nor other complications occurred. Histological examination of the bleeding site failed to show abnormalities and the nature …

PURPOSE OF REVIEW: Blood transfusion is ubiquitous in major burn injury. The present article describes recent research findings directly impacting blood transfusion strategies in major burn injury both in the operating room and the ICU.
RECENT FINDINGS: Transfusion strategies have been the focus of recent burn investigations. First, a randomized prospective trial encompassing both the ICU and operating room reported that a restrictive red blood cell transfusion threshold (7 g/dl) had equivalent outcomes to a traditional threshold (10 g/dl) for burns more than 20% in terms of mortality, infect

Patients with liver or kidney disease exhibit complex hemostatic changes, including abnormalities in platelet count and function. Although patients frequently bleed, bleeding is often unrelated to and not predicted by platelet abnormalities. Likewise, routine platelet transfusion to correct platelet abnormalities does not appear to prevent bleeding. Patients also exhibit prohemostatic abnormalities such as increased von Willebrand factor levels and platelet activation, which may compensate for antihemostatic changes. Notably, thrombosis is a frequent complication in these patients. Here, we na…
