Polymyositis is characterized by non-specific inflammatory disease associated with an autoimmune disorder involving muscles of the limbs and neck. We report a case of an 80-year-old man who was referred to our clinic with a chief complaint of dysphagia and muscle weakness in all four limbs. The patient was diagnosed with polymyositis based on pathological findings, muscle weakness, electromyogram findings, and an elevated creatine phosphokinase level. The patient was also positive for HLA-DR3. Intravenous predonine administration was initiated, but dysphagia was not improved. We considered a c…

Although rheumatoid arthritis (RA) and myositis are major autoimmune diseases, co-occurrence of the two is rare. We treated a patient who developed polymyositis (PM) following the treatment of RA with leflunomide. Prednisolone (PSL) in combination with methotrexate (MTX) was effective in managing the PM, but the RA relapsed during the treatment. Based on the clinical course, we suspect that the PM was induced by the leflunomide treatment and suggest that clinicians should consider the possibility of this rare adverse event in cases of cholestyramine-resistant elevation of transaminases.

BACKGROUND: Idiopathic inflammatory myopathies are chronic skeletal diseases with significant mortality and morbidity despite treatment by corticosteroids. Immunosuppressive agents and immunomodulatory therapy are used to improve disease control and reduce the long-term side effects of corticosteroids. While these treatments are used commonly in routine clinical practice, the optimal therapeutic regimen remains unclear.
OBJECTIVES: To systematically review the evidence for the effectiveness of immunosuppressants and immunomodulatory treatments for dermatomyositis and polymyositis.

Pneumomediastinum is a rare complication of dermatomyositis (DM) and Polymiositis (PM). We report here three cases of PM/DM who developed pneumomediastinum. First case was 61 years old woman with amyopathic dermatomyositis (aDM). Her aDM was complicated with skin ulceration due to vasculopathy, but complicated interstitial pneumonia was not severe. She developed subcutaneous emphysema and pneumomediastinum. Second case was 57 years old woman with DM, who had intractable skin phenomena and mild interstitial pneumonia. The patient became subcutaneous emphysema and pneumomediastinum following sev

A nine-year-old, female, neutered golden retriever was referred for investigation of weakness. Immune-mediated polymyositis with mixed mononuclear cell inflammation and fibrosis was diagnosed in muscle biopsy specimens from several sites. A soft tissue opacity was noted radiographically cranial to the cardiac silhouette. The dog failed to respond to treatment with prednisolone and was euthanased. At post-mortem examination, the structure observed radiographically was showed to be an enlarged right atrium. Histopathological examination of samples collected at post-mortem confirmed polymyositis.…

Pneumomediastinum as a complication of interstitial pneumonia with leakage of air into the mediastinum or subcutaneous tissues is a rare complication of dermatomyositis (DM). Herein we report a case of pneumomediastinum complicating polymyositis (PM), which is usually associated with DM. A 61-year-old man was hospitalized in our department because of deterioration of interstitial pneumonia. Treatment with high-dose corticosteroid and cyclosporin A steadily improved his interstitial pneumonia. Two weeks later, he developed subcutaneous emphysema and chest X-ray showed pneumomediastinum. Both su…

Polymyositis/Dermatomyositis (PM/DM) is a chronic inflammatory disorder that culminates in injury to the skin and muscle and, sometimes, is accompanied by interstitial lung disease (ILD). A number of autoantibodies are associated with myositis, including those specific for aminoacyl-tRNA synthetase (anti-ARS), signal recognition particle (anti-SRP), and Mi-2. These autoantibodies have proven to be useful in the diagnosis and classification of the diseases and are predictive of prognosis. It has been known that certain patients may have typical DM skin manifestations without clinical evidence o…

OBJECTIVE: To analyze the clinical features of pneumomediastinum complicated in polymyositis and dermatomyositis (PM/DM) and to study the pathogenesis thereof.
METHODS: The clinical data of 4 patients with pneumomediastinum complicated in dermatomyositis out of 447 PM/DM patients hospitalized in Peking Union Medical College (PUMC) Hospital Jan 1989 to June 2005, were analyzed. The records of patients with PM/DM available in English throughout the world were reviewed to collect those with pneumomediastinum as a complication. And the data of these patients were analyzed, focusing mainly on the …

PURPOSE: The authors report a unique case of multifocal choroidopathy in association with polymyositis.
METHODS: The clinical presentation, laser treatment, and subsequent change in ocular findings are described.
RESULTS: A 65-year-old Japanese man with polymyositis experienced mid acute vision impairment in both eyes during systemic corticosteroid treatment for interstitial pneumonia. Fundus examination revealed a gray-white subretinal exudate with serous retinal detachment. Angiographic examination disclosed partial dilatation of the choroidal vein at the posterior pole and dye leakage poi…

OBJECTIVE: : The objective of this study was to characterize the involvement of costimulatory molecules in patients with polymyositis (PM).
METHODS: : A multiparameter flow cytometry analysis was used to identify peripheral blood mononuclear cell (PBMNC) subsets expressing CD8, CD4, and CD28 in 14 patients with PM, 10 patients with Duchenne muscular dystrophy (DMD), and 10 healthy controls. Patients with PM were divided into the following 2 groups: patients with untreated active PM and patients with posttreated inactive PM.
RESULTS: : The percentage of CD8CD28/CD8 T cells and the absolute nu…

OBJECTIVE: To observe the relation between dermatomyositis/polymyositis and head-neck malignancy.
METHOD: All 136 cases of dermatomyositis/polymyositis admitted to PUMCH during 10 years (1994-2003) were studied. The clinical features of 6 patients with an associated head-neck malignancy were analyzed.
RESULT: There were 17 cases of malignancy in 136 patients with dermatomyositis/polymyositis, of which, 6 cases (6/17, 35.29%) were found to have head-neck malignancy: 4 cases of nasopharyngeal carcinoma, 1 case of non-Hodgkin lymphoma and 1 case of hypopharynx carcinoma.
CONCLUSION: Nasopharyn…

The relationship between polymyositis and malignancy is well known. Several types of tumors can be complicated with myositis and/or more frequently dermatomyositis. It has been suggested that tumors of the large bowel are rarely complicated by myositis. We describe a patient with adenocarcinoma of the colon presenting as polymyositis and review the available literature.

Polymyositis (PM) is associated with an increased risk of malignancy. We report a case in which a 45-year-old patient with PM underwent computed tomography to search for an occult malignant process. The evaluation revealed a left renal mass that was treated successfully by laparoscopic radical nephrectomy. Pathologic evaluation revealed a clear cell renal cell carcinoma. The patient remained free of disease at 2 years postoperatively and required decreasing amounts of medication to manage his PM. This suggests that renal malignancies may contribute to the PM disease process in a paraneoplastic…

Dermatomyositis (DM) and polymyositis (PM) are diseases displaying the symptoms of the idiopathic muscle inflammation (IZM). The skin lesions are visible most often 3 months to 3 years prior to appearance of the muscle inflammatory process. A frequent coexistence of DM with internal organ neoplasms obliges doctors to diagnose their patients quickly. The knowledge of skin lesions allows to begin this diagnosis earlier and to initiate proper therapy. Based on the latest medical literature, the authors present: the actual classification of the idiopathic muscle inflammation, the attempt to differ…
