Intestinal toxicoinfection by Clostridium botulinum type F in an adult. Case associated with Guillain-Barré syndrome.

A 27-year-old man with type F botulism (classification undetermined) had two episodes of botulinum toxaemia with identification of botulinum toxin and Clostridium botulinum organisms in faecal specimens during a three-month stay in hospital. Between these clinical episodes neither toxin nor Cl botulinum could be demonstrated. The illness was severe with quadriplegia, respiratory insufficiency, and bowel paralysis. In addition the patient had sensory abnormalities and a raised protein in the cerebrospinal fluid. The results demonstrate for the first time in detail an intestinal colonisation wit

Muscle hypertrophy and a syndrome of continuous motor unit activity in prednisone-responsive Guillain-Barré polyneuropathy.

The clinical, electrophysiological and morphological findings (light and electron microscopy of the sural nerve and gastrocnemius muscle) are reported in an unusual case of Guillain-Barré polyneuropathy with an association of muscle hypertrophy and a syndrome of continuous motor unit activity. Fasciculation, muscle stiffness, cramps, myokymia, impaired muscle relaxation and percussion myotonia, with their electromyographic accompaniments, were abolished by peripheral nerve blocking, carbamazepine, valproic acid or prednisone therapy. Muscle hypertrophy, which was confirmed by morphometric data

Differential diagnosis of acute weakness.

Muscle weakness evolving rapidly over hours to days demands prompt diagnosis and proper treatment to prevent life-threatening respiratory insufficiency. Such weakness usually results from disease affecting some portion of the motor unit, such as poliomyelitis, Guillain-Barré syndrome, botulism, and dyskalemic myopathy. The differential diagnosis of these processes is discussed.

[Guillain-Barré syndrome associated to a type B acute hepatitis (author's transl)].

A 42-year-old male patient suffers an acute hepatitis with positive HBs Ag and approximately 2 months after its onset, an acute polyneuritis with lessening of conduction velocity and albumino-cytologic dissociation appeared. Both conditions recuperated synchronously in a few months. This association has been only slightly referred to previously, and the majority of cases lacked facts important to the establishment of a diagnosis. The polyneuritis is possibly secondary to the viral alteration, either directly or due to an ensuing immunological alteration. Besides, there is the possibility that

Glial fibrillary acidic protein as a marker of axonal damage in chronic neuropathies.

We evaluated serum glial fibrillary acidic protein (GFAP) levels by enzyme-linked immunosorbent assay (ELISA) in controls (n = 30) and in patients with chronic sensory-motor axonal neuropathy (CSMAN) (n = 30), chronic inflammatory demyelinating polyneuropathy (CIDP) (n = 30), multifocal motor neuropathy (MMN) (n = 30), and primary muscular spinal atrophy (PMSA) (n = 15). GFAP levels, expressed as optical density, were increased in CSMAN (median = 1.05) compared to controls (median = 0.41; P < 0.05) and CIDP (median = 0.53, P < 0.05). They were also increased in PMSA (median = 0.99) compared to

Malnutrition, contaminated water, and post-infectious neuropathies: lessons from the Guillain-Barré syndrome outbreak in Gaza.

The cases of malnutrition, water insecurity, and the collapse of the health system, turning into a humanitarian crisis, have been illustrate … tion of water, but micronutrient deficiencies (e.g., thiamine) can also contribute to or exacerbate polyradiculoneuropathy. Nutritionally deficient patients cannot regenerate as well due to a reduced immune response that limits the ability to fight infections or repair nerve injuries. It is thus important to integrate nutritional surveillance and rehabilitation with the …

[POEMS syndrome. Report of three cases and literature review].

BACKGROUND: POEMS syndrome is a debilitating paraneoplastic disorder associated with plasma cell neoplasia. Clinically, it is characterized by predominantly demyelinating peripheral neuropathy that often starts in the pelvic limbs, organomegaly, endocrinopathy, skin changes, and elevated serum M protein. Diagnosis requires meeting a set of clinical criteria in which polyradiculoneuropathy and plasma cell disorder are the two mandatory criteria. The aim of this article is to describe 3 clinical cases, their diagnostic approach, and therapeutic management. CLINICAL CASES: In this study, three c…

Neuronal Proteins as Antigenic Targets in Membranous Nephropathy.

The discovery of new target antigens in membranous nephropathy (MN) has revealed new disease phenotypes and, in some cases, has suggested mechanisms of disease shared by two concurrent autoimmune diseases. Subject of Review: Several recent reports and an accompanying editorial describe the association of anti-contactin-1 (CNTN1) autoantibodies of the IgG4 subclass with a novel subtype of MN that co-occurs with a form of chronic inflammatory demyelinating polyradiculoneuropathy caused by anti-CNTN1 antibodies. CNTN1, the cellular source of which is still undetermined, is identified as the targe…

Vaccine-preventable diseases, vaccines and Guillain-Barre' syndrome.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy. Infections and vaccines have been hypothesized to play a role in triggering GBS development. These beliefs can play a role in reducing vaccination coverage. In this report, data concerning this hypothesis are discussed. It is shown that an association between vaccine administration and GBS has never been proven for most of debated vaccines, although it cannot be definitively excluded. The only exception is the influenza vaccine, at least for the preparation used in 1976. For some vaccines, such as measles/mumps/…

Autoantibodies in chronic inflammatory neuropathies: diagnostic and therapeutic implications.

The chronic inflammatory neuropathies (CINs) are rare, very disabling autoimmune disorders that generally respond well to immune therapies such as intravenous immunoglobulin (IVIg). The most common forms of CIN are chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), multifocal motor neuropathy, and polyneuropathy associated with monoclonal gammopathy of unknown significance. The field of CIN has undergone a major advance with the identification of IgG4 autoantibodies directed against paranodal proteins in patients with CIDP. Although these autoantibodies are only found in a small…

European Academy of Neurology/Peripheral Nerve Society Guideline on diagnosis and treatment of Guillain-Barré syndrome.

Guillain-Barré syndrome (GBS) is an acute polyradiculoneuropathy. Symptoms may vary greatly in presentation and severity. Besides weakness and sensory disturbances, patients may have cranial nerve involvement, respiratory insufficiency, autonomic dysfunction and pain. To develop an evidence-based guideline for the diagnosis and treatment of GBS, using Grading of Recommendations, Assessment, Development and Evaluation (GRADE) methodology a Task Force (TF) of the European Academy of Neurology (EAN) and the Peripheral Nerve Society (PNS) constructed 14 Population/Intervention/Comparison/Outcome q…

Guillain-Barré Syndrome Following Respiratory Syncytial Virus (RSV) Infection in the Third Trimester: A Case Report of a Rare Entity.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy affecting the peripheral nervous system, with a range of clinical presentations. GBS in pregnancy is rare and creates diagnostic and management challenges, with limited evidence to guide decisions on the timing and mode of delivery. We report a case of a 37-year-old gravida 4, para 3, who presented at 34+3 weeks of gestation with progressive lower limb weakness, sensory loss, and urinary incontinence. She had received a respiratory syncytial virus (RSV) vaccine shortly before the onset of symptoms. Neurological ev…

Pan-neurofascin autoimmune nodoparanodopathy: A case report and literature review.

RATIONALE: Locked-in syndrome (and its variant, completely locked-in state) generally has a high mortality rate in the acute setting; however, when induced by conditions such as acute inflammatory polyradiculoneuropathy, it may well be curable such that an attempt at cure should be systematically sought by clinicians. PATIENT CONCERNS: A 52-year-old man presented with acute tetraparesia and areflexia, initially diagnosed as Guillain-Barré syndrome. Despite appropriate treatment, his condition deteriorated, evolving into a completely locked-in state. DIAGNOSES: The detection of anti-pan-neuro…

Guillain-Barré syndrome.

Guillain-Barré syndrome (GBS) is a rare immune-mediated polyradiculoneuropathy. Patients typically develop rapidly progressive weakness and sensory deficits that can result in complete paralysis requiring mechanical ventilation. GBS is usually a monophasic disease in which an aberrant immune response to an infection or other trigger damages the peripheral nerves. For example, in patients with preceding Campylobacter jejuni infection, molecular mimicry causes a cross-reactive antibody response to nerve gangliosides. Diagnosis is based on clinical features, supported by cerebrospinal fluid analy…
