Locked-in syndrome: a catastrophic complication after surgery.

We describe the locked-in syndrome in a 31-yr-old patient after right upper lobectomy for suspected metastasis. After surgery, vertical eye movement was her only means of communication. She remained in a 'locked-in' state for 7 months before dying. Post-mortem examination showed extensive metastatic tumour deposition in the ventral pons. We discuss the clinical features of this syndrome and factors that may affect onset, diagnosis and management.

[MR tomography and computer tomography of alcohol-induced brain tissue changes].

Results of MRI in five alcoholics suffering from acute neurologic disorders such as ataxia, ophthalmoplegia and confusion are presented. The detection of focal cerebral lesions of high signal intensity in T2-weighted Spin-Echo- and Gradient-Echo-images in periventricular white matter, thalamus and pons combined with patients history and clinical presentation led to the diagnosis of Wernicke-Encephalopathia and Pontine Myelinolysis. MRI was far more sensitive compared with CT.

MR imaging of central pontine myelinolysis.

Central pontine myelinolysis is a demyelinating disorder with characteristic magnetic resonance features. Findings on T1-weighted scans include a symmetric region of low signal involving the basilar pons with sparing of the descending corticospinal tracts as well as the peripheral pontine tissues. The T2-weighted images show an area of high signal corresponding to the abnormal region identified on T1-weighted scans. Associated extrapontine demyelinating lesions are also demonstrated.

Systematic approach to intradural tumors ventral to the brain stem.

Intradural tumors that are situated anterior to the midbrain, pons, and medulla have historically been among the most inaccessible of all intracranial lesions. The classic approaches to the posterior fossa (e.g., suboccipital, retrosigmoid) provide only limited access to the anterior midline, primarily due to interposition of the cerebellum, brain stem, and numerous cranial nerves between the tumor and the viewpoint of the surgeon. A variety of techniques have been developed in recent years that create a craniotomy by removal of a portion of the lateral skull base. These procedures enhance exp…

Isolated Compressive Sixth Nerve Palsy due to Multiple Intracranial Cavernomas.

Cavernomas or cavernous malformations are the most common clinically significant vascular anomalies, accounting for 8-15% of all brain and spinal vascular malformations. While there are several articles in the literature on cavernomas, most cases report haemorrhage from these lesions as the cause of cranial nerve palsies. We report a rare case of multiple intracranial cavernomas in the brain and pons causing an isolated compressive sixth nerve palsy.

Pontine gaze paresis due to traumatic craniocervical hyperextension. Report of two cases.

Pontine gaze paresis is frequently due to tumor of the brain stem. Occasionally it may be caused by inflammation or ischemia. Two cases are reported, each with pontine gaze paresis and other signs of lower brain-stem injury, basal skull fractures, and second cervical vertebra fractures. This pattern of injuries is believed to be the result of craniocervical hyperextension with stretch injury to the brain stem at the junction of the medulla and pons.

Brainstem hemorrhage presented as audiovestibular syndromes.

Brainstem hemorrhage usually presented with acute multiple neurologic dysfunction, and the prognosis was poor. Rarely, it can manifest with audiovestibular symptoms only. Here, we report a case of brainstem hemorrhage involving the right middle cerebellar peduncle and dorsal lateral pons presented with constant nonpulsatile tinnitus and rotatory vertigo. We believed that rotatory nystagmus should be regarded as a central sign until proven otherwise even if the neurologic signs are subtle.

Parkinsonism and recovery in central and extrapontine myelinolysis.

Central pontine myelinolysis is a demyelinating affection of central pons diagnosed on the basis of characteristic MRI finding in an appropriate clinical setting. The condition has been described as universally fatal; however, recent reports of recovery have been documented. We report a case of central pontine and extra pontine myelinolysis, which presented with parkinsonian features apart from bulbar symptoms and made a remarkable recovery. A short review of the literature follows.

Giant fusiform aneurysm presenting as fatal basilar artery occlusion.

Fusiform aneurysms are often asymptomatic, and therefore their actual incidence is unknown. Ischemic stroke has been linked to dislodgment of small thrombi formed inside the aneurysm but is uncommon. This article describes two patients with rapidly fatal infarction in the entire pons, both cerebellar hemispheres, thalamus, and occipital lobes, findings consistent with occlusion of the vertebrobasilar artery complex. In both patients, autopsies revealed a giant fusiform aneurysm of the basilar artery.

Pontine lesion in opsoclonus-myoclonus syndrome shown by MRI.

Two patients with opsoclonus-myoclonus syndrome are reported whose magnetic resonance imaging (MRI) showed brain stem lesions. Both patients developed the opsoclonus-myoclonus syndrome after an upper respiratory illness. One case had visual hallucinations during the course of illness and MRI revealed a focal lesion in the pons involving the junction of basis and tegmentum. MRI of the second case showed a focal lesion at the upper pontine tegmentum.

Stretch-evoked motor responses in the brainstem are modulated by task instructions.

Reliable noninvasive measurement of human brainstem activity during motor control remains challenging due to small anatomical structures and physiological noise, yet it is essential for understanding descending contributions to rapid feedback control. We used brainstem-optimized whole brain functional magnetic resonance imaging (fMRI) to examine whether task-dependent modulation of stretch-evoked motor responses in humans are associated with changes in activation within reticulospinal regions of the human brainstem. A behavioral validation experiment (N = 10), in which participants were instru…
