Impact of IV metoclopramide + diphenhydramine on post-concussion symptoms. A secondary analysis of data from an ED-based randomized study.

BACKGROUND: In a randomized, placebo-controlled study, intravenous metoclopramide 20 mg + diphenhydramine 25 mg (M+D) improved post-concussion symptoms 1 h later among patients seeking care in an emergency department (ED) following head trauma. We performed a secondary hypothesis-generating analysis of those data to determine whether M+D is effective against cognitive and affective symptoms in addition to somatic symptoms, and whether post-treatment symptomatology is associated with 1-week and 1-month outcomes. METHODS: We enrolled patients in two urban EDs in the Bronx, NY. Enrollment commen

"It feels like a loss of one's own identity" - Work identity in persons suffering persisting post-concussion symptoms: A qualitative study.

PURPOSE: To examine the impact of persisting post-concussion symptoms (PPCS) on work identity, and to explore how work identity relates to the acceptance and coping mechanisms in individuals experiencing PPCS. METHODS: Twenty-three in-depth semi-structured interviews with persons suffering from PPCS for > 6 months, who attempted to return to work within the last 6 months. The data were analyzed using thematic analysis combined with hermeneutical phenomenological approach. RESULTS: We identified one global theme "Me and my work identity". Participants expressed a profound connection between w

Cause of Concussion With Persisting Symptoms Is Associated With Long-Term Recovery and Symptom Type, Duration, and Number in a Longitudinal Cohort of 600 Patients.

It is important for patients and clinicians to know the potential for recovery from concussion as soon as possible after injury, especially in patients who do not recover completely in the first month and have concussion with persisting concussion symptoms (C+PCS). We assessed the association between the causes of concussion and recovery from C+PCS in a consecutive retrospective and prospective cohort of 600 patients referred to the Canadian Concussion Center (CCC) at Toronto Western Hospital. Data were obtained from clinical records and follow-up questionnaires and not from a standardized dat

Sex-based differences in outcomes for adult patients presenting to the emergency department with a concussion.

OBJECTIVE: Patients with concussion frequently present to the emergency department (ED). Studies of athletes and children indicate that concussion symptoms are often more severe and prolonged in females compared with males. Given infrequent study of concussion symptoms in the general adult population, the authors conducted a sex-based comparison of patients with concussion. METHODS: Adults (≥ 17 years of age) presenting with concussion to one of three urban Canadian EDs were recruited. Discharged patients were contacted by telephone 30 and 90 days later to capture the extent of persistent pos

Prediction of Global Functional Outcome and Post-Concussive Symptoms after Mild Traumatic Brain Injury: External Validation of Prognostic Models in the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) Study.

The majority of traumatic brain injuries (TBIs) are categorized as mild, according to a baseline Glasgow Coma Scale (GCS) score of 13-15. Prognostic models that were developed to predict functional outcome and persistent post-concussive symptoms (PPCS) after mild TBI have rarely been externally validated. We aimed to externally validate models predicting 3-12-month Glasgow Outcome Scale Extended (GOSE) or PPCS in adults with mild TBI. We analyzed data from the Collaborative European NeuroTrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) project, which included 2862 adults wi

Subtle ocular motor deficits in people with chronic whiplash associated disorder compared to healthy controls.

INTRODUCTION: More than 50% of people who are diagnosed with whiplash-associated disorder (WAD) will report symptoms 12 months and beyond after their initial onset. However, many signs and symptoms, such as dizziness, emotional lability, confusion, ocular movement abnormalities, and balance deficits, may not be directly attributed to the cervical spine and may be more consistent with post-concussive syndrome (PCS). METHODS: A total of 15 people with chronic (>3 months) WAD and 15 age-sex match controls were recruited. They were evaluated on clinical tools commonly used to assess signs and sym

Case Report: Virtual Reality Neurofeedback Therapy as a Novel Modality for Sustained Analgesia in Centralized Pain Syndromes.

Neurofeedback (NFB) Therapy is a form of biofeedback, using the electroencephalogram (EEG) that has been in use since the 1970s, serving as a non-pharmacological intervention for epilepsy and psychiatric conditions such as anxiety, depression, insomnia, PTSD, post-concussive syndrome, and now, centralized pain. Chronic pain can increase neuronal activity and eventually causes poor modulation of pain messages. With the emergence of Virtual Reality (VR) in acute pain management, and the contraindications of opioids in chronic pain, applying novel biotechnologies seems like the next frontier in m

Personal technology for self-management after mild traumatic brain injury: a scoping review.

PURPOSE: Mild traumatic brain injuries (mTBI) account for 70-90% of TBIs worldwide. Persistent symptoms (fatigue, depression, cognitive problems) impact daily activities and participation. Self-management interventions have shown effectiveness in reducing symptoms. Personal digital technology (e.g. mobile phones) might offer effective and efficient options for supporting self-management following mTBI, but it remains unclear if/how personal digital technology is used to deliver self-management interventions following mTBI. This scoping review aimed to map evidence for digital self-management d

Do Initial Symptom Factor Scores Predict Subsequent Impairment Following Concussion?

OBJECTIVE: Symptom factors present during the first week following concussion may predict subsequent concussion outcomes and recovery duration. We hypothesized that a high loading on cognitive-fatigue-migraine and somatic factors would be predictive of neurocognitive impairment following concussion. We also hypothesized that the affective factor would be related to vestibular symptoms and impairment. DESIGN: Prospective repeated measures. SETTING: Concussion specialty clinic. PARTICIPANTS: Athletes aged 13 to 20 years diagnosed with a concussion within the past 7 days. INDEPENDENT VARIABLE

Efficacy of Melatonin in Children With Postconcussive Symptoms: A Randomized Clinical Trial.

BACKGROUND: Approximately 25% of children with concussion have persistent postconcussive symptoms (PPCS) with resultant significant impacts on quality of life. Melatonin has significant neuroprotective properties, and promising preclinical data suggest its potential to improve outcomes after traumatic brain injury. We hypothesized that treatment with melatonin would result in a greater decrease in PPCS symptoms when compared with a placebo. METHODS: We conducted a randomized, double-blind trial of 3 or 10 mg of melatonin compared with a placebo (NCT01874847). We included youth (ages 8-18 year

Neuropsychiatric complications of traumatic brain injury.

Traumatic brain injury (TBI) may be defined as any extracranial mechanical force to the brain that results in any period of loss of consciousness, any loss of memory for events immediately before or after the event, or any alteration in mental status at the time of the event. The major causes are automobile accidents, falls, sporting injuries, and assaults. Many soldiers returning from combat in Afghanistan and Iraq have also experienced TBI. This article provides an overview of the neuropsychiatric complications of TBI, including impairment of consciousness, posttraumatic amnesia, cognitive d

Neuroendocrine Whiplash: Slamming the Breaks on Anabolic-Androgenic Steroids Following Repetitive Mild Traumatic Brain Injury in Rats May Worsen Outcomes.

Sport-related concussion is an increasingly common injury among adolescents, with repetitive mild traumatic brain injuries (RmTBI) being a significant risk factor for long-term neurobiological and psychological consequences. It is not uncommon for younger professional athletes to consume anabolic-androgenic steroids (AAS) in an attempt to enhance their performance, subjecting their hormonally sensitive brains to potential impairment during neurodevelopment. Furthermore, RmTBI produces acute neuroendocrine dysfunction, specifically in the anterior pituitary, disrupting the hypothalamic-pituitar

Neurovascular coupling dysfunction of the frontal‒thalamic circuit in postconcussion syndrome.

Postconcussion syndrome (PCS) is characterized by alterations in perfusion and neural activity within specific regions, such as the frontal‒thalamic circuit. Despite extensive research on the neurovascular coupling (NVC) function of the PCS, how the involved brain regions interact as a dynamic network in the processing of cognitive and behavioral functions is still elusive. We recruited 38 participant with PCS and 38 healthy controls for magnetic resonance imaging examinations. We subsequently investigated four types of NVC, namely, amplitude of low-frequency fluctuation (ALFF), fractional ALF

Do fear and catastrophizing about mental activities relate to fear-avoidance behavior in a community sample? An experimental study.

Introduction: Healthy people often experience headache, cognitive failures, or mental fatigue. Some people even experience these symptoms on a level comparable to patients with mild spectrum brain injuries. In these individuals, the fear-avoidance model explains symptoms as a consequence of catastrophizing and fear-avoidance toward mental activities. This experimental study investigated in healthy adults whether fear-avoidance and catastrophizing about mental activities are related to fear-avoidance behavior (i.e., behavioral avoidance of mental activities) according to the fear-avoidance mode
