[Activity of vestibular organs in women with postmenopausal osteoporosis].

UNLABELLED: Osteoporosis is a systemic disease characterized by low bone mass and alterations in the microstructure of the skeleton. It is a worldwide disease, most prevalent in women over the age of 70. It is estimated that approximately 15-18% of women over 50 years of age are affected by osteoporosis and further 37-50% of women have decreased bone mass. The percentage of men suffering from this disease is about 40-50% lower. Untreated osteoporosis causes pathologic bone fractures, in particular fractures of the femoral neck, deformations and pain. THE AIM OF THIS WORK: was to evaluate the

Cost-effectiveness of Denosumab for the treatment of postmenopausal osteoporosis.

UNLABELLED: Denosumab is an injectable drug that reduces the risk of fractures. The objective was to estimate the cost-effectiveness of denosumab in a Swedish setting, also accounting for poor adherence to treatment. Denosumab is cost-effective, particularly for patients at high risk of fracture and low adherence to oral treatments. INTRODUCTION: Denosumab is a novel biologic agent developed for the treatment of osteoporosis and osteoporotic fractures that has been shown to reduce the risk of fractures in a phase III trial. The objective of this study was to estimate the cost-effectiveness of

Clinical use of denosumab for the treatment for postmenopausal osteoporosis.

Denosumab is a fully human monoclonal antibody with high affinity and specificity for human receptor activator of nuclear factor kappa B ligand (RANKL), the principal regulator of osteoclastic bone resorption. By binding to RANKL, denosumab prevents it from binding to its receptor on the cell surface of pre-osteoclasts and mature osteoclasts, thereby reducing the formation, activity, and survival of osteoclasts and inhibiting osteoclastic bone resorption. In a large, randomized, placebo-controlled clinical trial in postmenopausal women with osteoporosis, denosumab 60 mg administered subcutaneo

Development and validation of a disease model for postmenopausal osteoporosis.

UNLABELLED: This article describes the development of a model for postmenopausal osteoporosis (PMO) based on Swedish data that is easily adaptable to other countries. INTRODUCTION: The aims of the study were to develop and validate a model to describe the current/future burden of PMO in different national settings. METHODS: For validation purposes, the model was developed using Swedish data and provides estimates from 1990. For each year of the study, the "incident cohort" (women experiencing a first osteoporotic fracture) was identified and run through a Markov model using 1-year cycles unt

[Genital tuberculosis: a rare cause of postmenopausal bleeding].

BACKGROUND: Tuberculosis remains a global health problem, being the genitourinary tract the second most common site for tuberculosis infection after the lungs. Genital tuberculosis is now undergoing a worrying recrudescence. CASE REPORT: We report two cases of postmenopausal women who presented with vaginal bleeding. General physical and gynecological examination detected no abnormality. Transvaginal pelvic ultrasound showed fluid in the endometrial cavity in both cases. The office hysteroscopy was suggestive of focal endometrial thickness and an endometrial biopsy was performed. The histopat

[Ureteral endometriosis in estrogen-treated postmenopausal woman].

A 56-year-old woman was admitted to our hospital due to gross hematuria. Cystoscopy and contrast-enhanced abdominal CT scan revealed a solid tumor at the right terminal ureter. She underwent transurethral resection of the right ureteral tumor. The ureteral tumor included proliferative endometrial stroma and glands under urothelial cells, and the histopathological diagnosis was ectopic endometriosis. Before the surgery, the patient had underwent hormone replacement therapy using estrogen patches to treat menopausal disorders, however, the dose of estrogen was five times higher than regular dose

Bisphosphonate dose and incidence of fractures in postmenopausal osteoporosis.

INTRODUCTION: The specific pharmacological properties of bisphosphonates have raised concerns about their long-term effects on skeletal fragility that may be related to the total dose of bisphosphonate given. However, the effect of different doses on the incidence of osteoporotic fractures has not been adequately studied. METHODS: In this retrospective analysis, we investigated the effect of different doses of intravenous pamidronate given at 3-monthly intervals on the incidence of fractures in 92 women with severe postmenopausal osteoporosis. RESULTS: The risk of sustaining a new vertebral

Skeletal health: primate model of postmenopausal osteoporosis.

Currently, the nonhuman primate is the most widely used large animal model to evaluate the safety and efficacy of new drug entities to treat or prevent estrogen-deficiency-induced bone loss and osteoporosis. Surgical ovariectomy (OVX) induces a state of high bone turnover and rapid bone loss establishing a new steady-state bone mass within 8-9 months. Many systems in the monkey are similar to humans, including skeletal and reproductive physiology and the immune system, making this a plausible model suitable to evaluate the effects of new bone drugs. The long-term sequelae following OVX and wit

Paradigm shift in the pathophysiology of postmenopausal and thyrotoxic osteoporosis.

Bone loss is an inevitable result of getting older in both sexes. This loss is accelerated in women in the early years following menopause and in both sexes with drugs and diseases, prominently hyperthyroidism. Postmenopausal osteoporosis and hyperthyroid osteoporosis, both of which are widely recognized clinical entities, have been thought to arise from low estrogen levels and high thyroid hormone levels, respectively. However, when estrogen declines, follicle-stimulating hormone, an anterior pituitary hormone, rises. Likewise, when thyroid hormone levels rise in hyperthyroidism, thyroid-stim

Denosumab for prevention of fractures in postmenopausal women with osteoporosis.

BACKGROUND: Denosumab is a fully human monoclonal antibody to the receptor activator of nuclear factor-kappaB ligand (RANKL) that blocks its binding to RANK, inhibiting the development and activity of osteoclasts, decreasing bone resorption, and increasing bone density. Given its unique actions, denosumab may be useful in the treatment of osteoporosis. METHODS: We enrolled 7868 women between the ages of 60 and 90 years who had a bone mineral density T score of less than -2.5 but not less than -4.0 at the lumbar spine or total hip. Subjects were randomly assigned to receive either 60 mg of den

Obesity and risk of cancer in postmenopausal Korean women.

PURPOSE: To evaluate an association between obesity, measured by body mass index (BMI; kg/m(2)), and risk of cancer at individual and all sites in postmenopausal women. METHODS: A cohort of 170,481 postmenopausal Korean women who were age 40 to 64 years at baseline measurement of BMI was observed prospectively from 1994 to 2003 for cancer incidence. Multivariable adjusted proportional hazard models were used for evaluating the association. RESULTS: Women with a BMI of 30 kg/m(2) or higher had a 23% higher risk of cancer than women with a BMI between 21.0 and 22.9 kg/m(2) (hazard ratio = 1.23

Identification of a role for the ARHGEF3 gene in postmenopausal osteoporosis.

Osteoporosis is a common and debilitating bone disease characterized by low bone mineral density (BMD), a highly heritable and polygenic trait. Genome-wide linkage studies have identified 3p14-p21 as a quantitative trait locus for BMD. The ARHGEF3 gene is situated within this region and was identified as a strong positional candidate. The aim of this study was to evaluate the role of variation in ARHGEF3 on bone density in women. Sequence variation within ARHGEF3 was analyzed with 17 single-nucleotide polymorphisms (SNPs) in a discovery cohort of 769 female sibs. Significant associations were

Treatment of postmenopausal osteoporosis in women: a systematic review.

Osteoporosis, a typical disease of the elderly, has become a frequent and relevant public health problem. Several drugs are available for treatment of osteoporosis, some of which are currently dispensed by the Brazilian Unified National Health System. The objective of this study was to present a systematic review of drugs for treatment of osteoporosis, focusing on the adequacy of clinical protocols based on existing evidence in the scientific literature. We conducted a search for randomized clinical trials in PubMed and LILACS that presented results for bone mineral density, incidence of verte

Denosumab: an investigational drug for the management of postmenopausal osteoporosis.

Denosumab (AMG 162) is an investigational fully human monoclonal antibody with a high affinity and specificity for receptor activator of nuclear factor-kappaB ligand (RANKL), a cytokine member of the tumor necrosis factor family. RANKL, the principal mediator of osteoclastic bone resorption, plays a major role in the pathogenesis of postmenopausal osteoporosis and other skeletal disorders associated with bone loss. Denosumab inhibits the action of RANKL, thereby reducing the differentiation, activity, and survival of osteoclasts, and lowering the rate of bone resorption. Clinical trials have s

Drospirenone and estradiol: a new option for the postmenopausal woman.

The efficacy of estrogen with or without a progestogen as hormone replacement therapy (HRT) for menopausal symptoms is well-established. Recent large-scale randomized studies with combined estrogen/progestogen therapy (EPT) have raised a number of safety issues, specifically the potential risk for coronary heart disease. Subsequent analyses and other studies have indicated that HRT may be cardioprotective in younger postmenopausal women. A new continuous EPT combines natural 17beta-estradiol (E2) 1 mg with the novel progestin, drospirenone (DRSP) either 0.5 or 2 mg. DRSP has a physiological pr

[Effectiveness of anti-resorption therapies of postmenopausal osteoporosis].

The aim of this article is to justify a need for evaluation of the absolute risk of fractures, to compare efficacy of antiresorptive therapies as indicated by large clinical trials, and to review currently available methods to assess efficacy of therapies in individual patients. It is emphasized that adherence to therapies is influenced not only by the individual bone efficacy and safety of the drug but also by beneficial impacts of the treatment on tissues other than bone.
