Safely treating hypokalaemia in high dependency cardiac surgical patients.

In Australia, there were national issues on the use of potassium ampoules (resulting in patient deaths), which led to the removal of the ampoules from clinical areas. A decision was made by the Medication Safety Committee at a metropolitan Melbourne hospital to remove potassium ampoules from ward areas as part of the establishment of a hospital-wide potassium guideline. As a result, the nurses in the cardiothoracic ward Practice Review Committee identified the need to review the proposed practice of treating hypokalaemia with 30 mmol of potassium chloride (KCL) in 1000 mL over an extended peri…

Pharmacologic Approaches to Electrolyte Abnormalities in Heart Failure.

Electrolyte abnormalities are common in heart failure and can arise from a variety of etiologies. Neurohormonal activation from ventricular dysfunction, renal dysfunction, and heart failure medications can perturb electrolyte homeostasis which impact both heart failure-related morbidity and mortality. These include disturbances in serum sodium, chloride, acid-base, and potassium homeostasis. Pharmacological treatments differ for each electrolyte abnormality and vary from older, established treatments like the vaptans or acetazolamide, to experimental or theoretical treatments like hypertonic s

Aldosterone--a hormone of cardiovascular adaptation and maladaptation.

Aldosterone stimulates reabsorption of sodium, sustaining blood volume and pressure in the face of salt deprivation or extracellular fluid depletion. The steroid also stimulates excretion of potassium, protecting extracellular fluid from excessive levels of that ion. These two actions are relatively rapid and clearly adaptive when appropriately initiated and terminated, but maladaptive when prolonged or excessive, causing hypertension and electrolyte imbalance. Aldosterone and other mineralocorticoids exert slower, direct effects on cells in the heart, kidneys, and vessels, leading to hypertro

Periodic paralysis with cardiac arrhythmia.

In 1963, Klein et al. first described two girls with normokalemic periodic paralysis and cardiac arrhythmia. We have observed a 15-year-old … glucose followed by 10 units of regular insulin. Muscle weakness was evoked by the ingestion of 4 g potassium chloride. During the provoked muscle weakness, the ECG showed normal sinus rhythm temporarily.

Plasma potassium may protect sodium pumps of toad hearts from an endogenous inhibitor.

Resibufogenin (3-hydroxy-14,15-epoxy-20,22-dienolide glycoside) is a potent sodium pump inhibitor present in toad toxin. It is present in the skin of the cane toad (Bufo marinus) at a concentration equivalent to ouabain of approximately 1 mM. Because toads, like other amphibians, have permeable skin, resibufogenin is also found in high concentrations in the blood. In the cane toad the blood concentration is estimated to be 1 microM (D. Bartter syndrome: an overview.

The term Bartter syndrome denotes a group of renal diseases which share a common denominator of hypokalaemia and metabolic alkalosis. The patch-clamp technique has made possible the analysis of single ion channels, improving our understanding of the molecular physiopathology of all the 'Bartter-like' syndromes. Genetic mapping of each defect has further clarified the mutations involved and the possible modes of inheritance. This improved understanding has opened new avenues for therapy, improving mortality and morbidity in these patients. Another group of illnesses, the 'pseudo-Bartter syndrom

Refinement of Replacement Fluid Using Human Serum Albumin Preparation to Enhance Biocompatibility in Therapeutic Plasma Exchange.

Human serum albumin (HSA) solution is widely used as a replacement fluid during therapeutic plasma exchange (TPE). However, the ionic concentrations in commercially available 5% HSA solutions differ significantly from physiological levels. In this study, we developed a simple method for preparing a replacement fluid with ion concentrations closer to physiological norms by mixing 25% HSA with lactated Ringer's solution, sodium chloride, and calcium gluconate hydrate. We prepared three types of replacement fluids, each with a 4.0% albumin concentration and total volumes of 1.83, 2.43, and 3.04 L

Chronic metabolic alkalosis in an infant with cystic fibrosis.

A 6-month-old infant suffering from cystic fibrosis is reported. In spite of an apparently appropriate treatment and in absence of respiratory infection, the patient showed progressive anorexia, intermittent vomiting and weight loss. These non-specific signs and symptoms could all be explained by metabolic alkalosis and disappeared immediately after oral supplementation with sodium and potassium chloride. This unusual metabolic complication should be searched for in every cystic fibrosis infant with unexplained anorexia and failure to thrive.

Effect of somatostatin on exocrine pancreas evaluated on a total external pancreatic fistula of neoplastic origin.

A patient with a pancreatic fistula, with a defunctionalized duodenum (subsequent to a previously performed Billroth II gastrectomy for cancer and choledochojejunostomy for metastatic infiltration in the bile duct) was submitted to prolonged synthetic somatostatin infusion (48 h). There was a mean reduction of 80% in pancreatic output, with no changes in the composition of sodium, potassium, and calcium, an increase in chloride and a decrease in bicarbonate. An increase in amylase was noted. The unique clinical features of this case have provided direct confirmation of the inhibiting action of

Paradoxic aciduria in bovine metabolic alkalosis.

Cows with metabolic alkalosis secondary to abomasal displacement and other abomasal disorders were often found to excrete acidic urine. This paradoxic aciduria contradictsthe classical view that the pH of the urine may be used to estimate the acid-base status of the body. Data from bovine clinical patients with metabolic alkalosis, serum electrolyte changes, and paradoxic aciduria suggested that the balance of sodium potassium, and chloride in the body places limits on the kidneys' ability to regulate the acid-basebalance.

Vascular Functional Effect Mechanisms of Elabela in Rat Thoracic Aorta.

BACKGROUND: Elabela is a recently discovered peptide hormone. The present study aims to investigate the vasorelaxant effect mechanisms of elabela in the rat thoracic aorta. METHODS: The vascular rings obtained from the thoracic aortas of the male Wistar albino rats were placed in the isolated tissue bath system. Resting tension was set to 1 gram. After the equilibration period, the vessel rings were contracted with phenylephrine or potassium chloride. Once a stable contraction was achieved, elabela-32 was applied cumulatively (10-9-10-6 molar) to the vascular rings. The experimental protocol …

Strong monovalent electrolyte imbalances in serum of dogs infected with Babesia canis.

Canine babesiosis is a systemic tick-borne protozoan disease caused by infection with parasites of the genus Babesia. Acid-base disorders and ion imbalances have been described in dogs infected with Babesia rossi in South Africa. In this paper, the authors describe changes to monovalent ion concentrations and calculated parameters of monovalent ions in 70 dogs naturally infected with B. canis, a species occurring in Europe. Hyponatraemia, hypokalaemia, hyperchloraemia, decrease of chloride gap, strong ion gap, difference between sodium and chloride concentrations, and an increase of chloride-t

Self-induced vomiting presenting as Bartter's syndrome.

The physiologic effects of habitual vomiting mimic the metabolic abnormalities seen in Bartter's syndrome, ie, hypokalemic metabolic alkalosis and increased plasma renin and aldosterone levels with kaliuresis. However, in habitual vomiting, the 24-hour urine chloride level will be lss than 10 mEq/liter owing to gastrointestinal loss and in Bartter's syndrome it will be greater than 20 mEq/liter owing to defective chloride reabsorption. The case is reported of a young woman whose self-induced vomiting caused metabolic abnormalities consistent with a diagnosis of Bartter's syndrome. The absence

Unusual potassium channels mediate nonadrenergic noncholinergic nerve-mediated inhibition in opossum esophagus.

Field stimulation of the circular muscle of the opossum esophagus produces a transient hyperpolarization (inhibitory junction potential, IJP) followed by an "off" depolarization. A similar nonadrenergic, noncholinergic (NANC) response in guinea pig taenia caecum has been shown to be due to an increase in the potassium ion permeability of the smooth muscle cell membrane. Double sucrose gap studies showed a decrease in resistance during the IJP, and a reversal at an estimated membrane potential of about -90 mV (4 mM K+). The reversal potential was dependent on the extracellular potassium concent

[Role of distal nephron in the control of extracellular volume in physiology and in nephrotic syndrome].

The kidney plays a major role to maintain the constancy of the "milieu intérieur" by adjusting the urinary excretion of water and solutes to the requirement of the body balance. This function is coordinated with elimination of waste products generated among others by the catabolism of proteins and nucleic acids. To cope with these two major functions, the human kidneys generate each day about 180 L of ultrafiltrate from plasma and reabsorbs the vast majority of filtered water and solutes to excrete daily about one-two liter(s) of urine containing concentrations of sodium, potassium and chlorid
