BACKGROUND: Priapism can be categorized as low flow or high flow. Low flow priapism is a compartment syndrome and requires immediate treatment to avoid long-term ischemic damage. Alternatively, high flow priapism is not an emergent condition and can be managed as an outpatient. The diagnosis has traditionally been made via cavernosal blood gas analysis; however, this is painful and can cause iatrogenic harm.

Homocystinuria is a rare, autosomal recessive, inherited disorder of metabolism that is caused by a deficiency of cystathionine synthase, leading to marked hyperhomocysteinemia. High-flow priapism due to pseudoaneurysm is a relatively rare urologic condition. Clear anatomic delineation of the number and origin of feeding vessels facilitates pre-embolization planning. Computed tomographic angiography can afford a three-dimensional display of the feeding vessels. We present a 26-year-old man with post-traumatic high-flow priapism, which is the first case studied with computed tomographic angiography.

Priapism in children after a black widow spider bite is a rare phenomenon with only a few case reports noted in the literature. The use of drugs with α-adrenergic antagonistic effect is one of the most prominent etiologies of priapism. We report a 32-year-old schizophrenic male in treatment with risperidone who consulted in the emergency room for a painful priapism. A low flow priapism was diagnosed. Medical treatment was unsuccessful and the patient was subjected to a proximal corporo-spongiosal shunt (Quackels technique), with good results. The patient was discharged in good conditions.

PURPOSE: To present three cases of arterial high flow priapism (HFP) and propose a management algorithm for this condition.
MATERIALS AND METHODS: We studied three children with post-traumatic arterial HFP (two patients with perineal trauma and one with penis trauma).

A 51-year-old man presented with 4 days of idiopathic ischemic priapism. Partial detumescence was achieved with aspiration and injection with intracorporeal adrenaline.
