Neuraxial Anesthesia and Analgesia During Cardiothoracic Surgery: A Narrative Review.

PURPOSE OF REVIEW: The purpose of this review is to synthesize and examine the literature on the use of neuraxial anesthesia and analgesia during cardiothoracic surgery. As cardiothoracic procedures often require systemic anticoagulation, neuraxial techniques are quite often underutilized due to the theoretical risk of epidural hematoma. In this review, we seek to examine the literature to review the indications and contraindications and to explore if neuraxial anesthesia and analgesia has a role in cardiothoracic surgery. RECENT FINDINGS: Neuraxial techniques have multiple advantages during

A Comprehensive Review of the Treatment and Management of Pain in Sickle Cell Disease.

PURPOSE OF REVIEW: Sickle cell disease (SCD) is a hematological disorder which leads to serious complications in multiple organ systems. While significant research has addressed many of the effects of acute pain episodes and end-organ damage connected to this disease, little has approached the chronic pain state associated with this condition. RECENT FINDINGS: Associated chronic pain represents a significant detractor from the quality of life experienced by these patients, affecting over half of those with SCD on more days than not. Current treatment typically is centered upon preventing and

Adjunctive methods of pain control in burns.

Opiates remain the most common form of analgesic therapy in the burn patient today. Because of increased opiate requirements, optimal relief of burn pain continues to be a problem for these patients. The purpose of this article is to summarize those alternative pain control methods that appear in the literature. For instance, in minor burns acetominophen continues to be a useful first line analgesic. Non-steroidal anti-inflammatory drugs (NSAID) and benzodiazepine are generally combined with opiates while entonox seems to be used commonly in the adolescent patients to relieve procedural pain. …

Multicenter study evaluating factors associated with treatment outcome for low back pain injections.

BACKGROUND: There has been a worldwide surge in interventional procedures for low back pain (LBP), with studies yielding mixed results. These data support the need for identifying outcome predictors based on unique characteristics in a pragmatic setting. METHODS: We prospectively evaluated the association between over two dozen demographic, clinical and technical factors on treatment outcomes for three procedures: epidural steroid injections (ESIs) for sciatica, and sacroiliac joint (SIJ) injections and facet interventions for axial LBP. The primary outcome was change in patient-reported aver

The provocative lumbar facet joint.

Low back pain is the most common pain symptom experienced by American adults and is the second most common reason for primary care physician visits. There are many structures in the lumbar spine that can serve as pain generators and often the etiology of low back pain is multifactorial. However, the facet joint has been increasingly recognized as an important cause of low back pain. Facet joint pain can be diagnosed with local anesthetic blocks of the medial branches or of the facet joints themselves. Subsequent radiofrequency lesioning of the medial branches can provide more long-term pain re

Radiofrequency Probe Guidance in Peripheral Nerve Stimulator Placement for Complex Anatomy: A Case Report.

BACKGROUND: Peripheral nerve stimulation (PNS) has been recognized for its efficacy in treating various pain conditions. However, its application in anatomically unique patients, especially those with achondroplasia, is not well established. CASE REPORT: We describe a 30-year-old woman with achondroplasia suffering from neuropathic pain after total knee arthroplasty. Due to inadequate pain control from standard pharmacotherapy, she underwent PNS. Sensory stimulation was performed utilizing a radiofrequency ablation (RFA) probe to precisely locate the target sensory nerve. Once the correct sen

Factors predicting outcomes from chronic pain management interventions.

Chronic pain is the leading cause of years lost to disability worldwide, by a large margin, affecting 20-34% of the world's population. Chronic pain is the target for an increasing number of invasive and expensive treatments, supported by different levels of evidence. At a time when personalised medicine, driven in part by the growth of artificial intelligence, is surging, a scoping review on the factors that affect pain outcomes for procedural interventions is needed. A scoping review is important because placebo controlled trials for the most commonly used treatments consistently show small-

The effectiveness of a pain and anxiety protocol to treat the acute pediatric burn patient.

This retrospective review of 286 acute pediatric burn survivors treated in 2001 evaluated the effectiveness of a pharmacotherapeutic protocol for pain, anxiety, and itching. Background pain, procedural pain, exercise pain, anxiety, incidence of acute stress disorder (ASD), and itch were measured with standardized instruments. When this review was compared to similar reviews done in 1993-1994 and 1998, a steady trend toward using more potent pain medications in this patient population is evident. While the use of acetaminophen alone decreased from 50.6% of patients in 1993-1994 and 26.3% in 199…

The CY 2027 Medicare Physician Fee Schedule and the Continuing Struggle to Balance Cost, Quality, Access, and the Survival of Interventional Pain Management.

BACKGROUND: On July 16, 2026, the Centers for Medicare and Medicaid Services (CMS) released the Calendar Year (CY) 2027 Medicare Physician Fee Schedule (PFS) Proposed Rule (CMS-1848-P). The rule arrives one year after the CY 2026 final rule institutionalized two structural changes of unusual consequence for procedural specialties: a recurring efficiency adjustment that reduces work relative value units (RVUs) for non-time-based services, and a redesign of the indirect practice expense (PE) methodology that shifts payment away from facility-based procedural care toward office-based evaluation a

Absent Left Main Coronary Artery with Separate Origin of Left Anterior Descending Artery and Left Circumflex Artery from Left Coronary Sinus.

We report a case of a 49-year-old female who presented with atypical chest pain. An electrocardiogram-gated coronary computed tomography angiography revealed the absence of the left main coronary artery segment with direct origin of the left anterior descending artery and the left circumflex artery from the left coronary sinus having separate ostia. The case highlights a rare anomaly of coronary artery origin with procedural implications.

[Neurosurgical treatments for pain].

Pain represents the most frequent symptom faced by general practitioners and is associated with 60% of neurological troubles. Pain consists in a conscious, subjective, unpleasant and protective sensory experience transmitted by thermoalgic pathways in the central nervous system (nociceptive pain). Lesioning of peripheral or central sensory pathways can also generate pain associated with hypoesthesia (phantom or neuropathic pain). Since the 1920's, neurosurgeons have attempted to alleviate nociceptive and neuropathic chronic pain by interrupting (irreversible interruptive techniques) thermoalgi

Moderate and deep procedural sedation-the role of proper monitoring and safe techniques in clinical practice.

PURPOSE OF REVIEW: Interventional pain management procedures provide significant improvement to patient quality of life and functionality. In-office procedures are becoming an increasingly more common site of pain management intervention for patients with minimal risk of harm. RECENT FINDINGS: Moderate and deep sedation techniques can be used in patients with high anxiety, complex pharmacotherapy, or a low pain threshold. Proper guidance and oversight by an attending anesthesiologist, in addition to appropriate monitoring, are key. Epidural steroid injection complications rates have been cite
