A physicians's guide to preventing tobacco use during childhood and adolescence.

Physicians who care for children can and should help patients avoid the use of tobacco. Physicians are well aware of the health hazards associated with tobacco use, inasmuch as smoking is the chief, single cause of premature mortality in this country. Each day, more than 3000 children in the United States begin to use tobacco. Physicians who care for children have patients at vastly different stages of intellectual and social maturity. Both the theory and practical details of tobacco-related interventions differ among infants, children, and adolescents. The physician is in a unique position to

The relevance of a staff-oriented approach in consultation psychiatry: a preliminary study.

According to the literature, a patient-staff conflict or intra-staff conflict is often the hidden reason for requesting a psychiatric consultation. This study is specifically directed at determining the percentage of consultations in which such "staff problems" play a clinically relevant role. Indications of staff problems were found in one-third of 313 consultations investigated. These problems occurred significantly more frequently in patients admitted to surgical wards and in patients referred because of psychological disturbances related to their physical disorder, with a diagnosis of "tra

The meaning of higher education for people diagnosed with a mental illness: four students share their experiences.

In this qualitative study, four participants diagnosed with a mental illness were interviewed to explore their experiences while attending a post-secondary school. Each participant described how education helped them to find a sense of purpose in their lives. Education is also described as a means of transition from the patient role to other roles such as student or worker. However, the symptoms and stigma associated with their mental illness has created additional challenges for them while in a school setting. Supportive professors and counselors were viewed as helpful in overcoming these bar

The role of nurses and dietitians in managing paediatric coeliac disease.

Coeliac disease (CD) is an immune-mediated genetic condition elicited by the ingestion of gluten, leading to proximal small bowel enteropathy. It affects around 1% of the population, although only a small proportion of cases are actually diagnosed. It is a multisystem disorder presenting with both gastrointestinal and extra-intestinal manifestations such as diarrhoea, abdominal pain, constipation, vomiting, iron deficiency anaemia, faltering growth, dental enamel defects, short stature, liver disease, arthropathy and recurrent aphthous ulcers. Nurses, working in different clinical settings, ar

The role of teachers in the prevention and management of childhood obesity among school-aged children in Ghana: a cross-sectional study.

BACKGROUND: School-based interventions have been widely used in the prevention and control of childhood obesity. Teachers, as pivotal figures in child development, are strategically situated to aid in this process. Nonetheless, there is limited evidence regarding their role, awareness, and contribution to childhood obesity in Ghana. This study evaluated the role of teachers in the prevention and management of childhood obesity in Ghana. MATERIALS AND METHODS: This quantitative, cross-sectional study included 74 teachers selected through convenience sampling from public schools in the Oforikro

Evaluating the implementation of a rapid access atrial fibrillation clinic utilising a pharmacist-physician model of care.

BACKGROUND: Time to assessment and treatment of atrial fibrillation (AF) is critical for reducing stroke risk. However, Australian data have shown low uptake of timely anticoagulation, with people in regional areas having a greater risk of low uptake compared to people in metropolitan areas. OBJECTIVE: To conduct a retrospective, mixed methods evaluation of a pharmacist-physician model of care for a rapid access atrial fibrillation (RAAF) clinic in a large regional centre in Victoria, Australia. METHODS: The RAAF clinic involved telehealth pharmacist appointments and face-to-face physician c

Quantitative Assessment of Occupational and Leisure-Time Physical Activity and Sedentary Behavior in Registered Nurses and Certified Nursing Assistants Using Wrist-Worn Accelerometers.

OBJECTIVE: This study aimed to explore how occupational physical activity (OPA) and leisure-time physical activity (LTPA) interact with nurses assigned to different roles (ie, registered and assistants). METHODS: Twenty-two professional nurses were monitored for 7 days, using a wearable accelerometer. Based on raw data, physical activity (PA) was classified as sedentary behavior, and light-, moderate-, and vigorous-intensity PA. RESULTS: In comparison with registered nurses, certified nursing assistants spent less time in sedentary behavior (29% vs 53%) and more time in moderate-intensity PA

Selected infectious disease topics in the ED: Optimizing logistics to optimize care.

PURPOSE: Emergency medicine pharmacists (EMPs) are involved in many clinical decisions made at the bedside in the emergency department (ED). Along with answering clinical questions, pharmacists are often relied on to optimize logistical issues requiring coordination among multiple departments and clinical applications, such as the electronic medical record and automated dispensing cabinets. This article discusses 3 common infectious disease-related issues that pharmacists can assist with to ensure care is optimized during and after a patient's ED visit. SUMMARY: This article first discusses a

The perceptions of first point of contact roles in primary care by pre-registration students of the allied health professions.

AIM: To develop a better understanding of the perceptions of first point of contact roles within primary care by pre-registration students of the Allied Health Professions (AHPs). BACKGROUND: General practice in the UK is under growing pressure from declining general practitioner (GP) numbers and increased service demand. The National Health Service (NHS) is attempting to mitigate this demand by making more effective use of its highly experienced workforce through the creation of first contact practitioners (FCPs). Working in primary care, FCPs are highly experienced AHPs with three or more y

Gerontological social work and cardiac rehabilitation.

Cardiac rehabilitation is a setting in which integrating social work services can benefit older adults. Many cardiac rehabilitation patients endorse symptoms of stress and depression following a cardiac event, impeding their ability to participate fully in cardiac rehabilitation services or recover from a heart attack. Gerontologically trained social workers can improve the care of older adults with heart disease in a variety of ways and this paper discusses the potential roles social workers can play in enhancing care. Two examples demonstrating how community academic partnerships can lead to

Pharmacist intervention to detect drug adverse events on admission to the emergency department: Two case reports of neuroleptic malignant syndrome.

WHAT IS KNOWN AND OBJECTIVE: Neuroleptic malignant syndrome (NMS) is a rare but severe adverse effect of antipsychotic drugs. CASE DESCRIPTION: We report two cases of NMS highlighted by clinical pharmacists in an emergency unit during summer. One of them was fatal. Medication reconciliation processes performed at admission identified treatment with loxapine for one of them and with loxapine and clozapine for the other. Interview of the patients highlighted clinical symptoms suggesting NMS, allowing the pharmacists to alert the medical team. WHAT IS NEW AND CONCLUSION: Adverse drug events may

Caring for the patient, caring for the record: an ethnographic study of 'back office' work in upholding quality of care in general practice.

BACKGROUND: The quality of information recorded about patient care is considered key to improving the overall quality, safety and efficiency of patient care. Assigning codes to patients' records is an important aspect of this documentation. Current interest in large datasets in which individual patient data are collated (e.g. proposed NHS care.data project) pays little attention to the details of how 'data' get onto the record. This paper explores the work of summarising and coding records, focusing on 'back office' practices, identifying contributors and barriers to quality of care. METHODS:
