Preferential production of latent transforming growth factor beta-2 by primary prostatic epithelial cells and its activation by prostate-specific antigen.

Three mammalian isoforms of transforming growth factor-beta (TGFbeta) are known, TGFbeta1, 2, and 3, that have non-overlapping functions during development. However, their specific roles in cancers such as prostate cancer are less clear. Here we show that primary cultures of prostatic epithelial cells preferentially produce and activate the latent TGFbeta2 isoform. Paired cultures of normal and malignant prostate cells from prostate cancer patients produced predominantly the TGFbeta2 isoform, with 30- to 70-fold less TGFbeta1. By mono-Q ion exchange chromatography, three major peaks of latent

Prognostic Impact of Initial Prostate Specific Antigen Half-Life After Abiraterone Acetate in High-Volume Metastatic Hormone-Sensitive Prostate Cancer: Who May Need Triplet Therapy?

BACKGROUND: As no randomized study has compared abiraterone acetate (ABI) doublet and triplet therapy, the optimal target for the add-on of docetaxel (DTX) to ABI doublet therapy remains unclear. The present study explored patients who may benefit from add-on DTX using initial prostate-specific antigen (PSA) reduction after ABI doublet therapy. METHODS: We retrospectively reviewed 233 patients with CHAATED high-volume metastatic castration-sensitive prostate cancer (mCSPC) treated with ABI doublet therapy. Using the initial PSA half-life calculated by PSA reduction within 6 weeks of treatment…

Alpha-Particle Therapy with an Actinium-225 Labeled Bivalent Inhibitor of Prostate-Specific Membrane Antigen is Therapeutically Efficacious in a Mouse Model of Prostate Cancer.

Prostate-Specific Membrane Antigen (PSMA) is overexpressed in some prostate cancers and is a viable candidate for targeted radionuclide therapy. PSMA targeted delivery of α2+ particle-emitting actinium-225 to tumors is a potential therapeutic option for prostate cancer. A substituted diazacrown ether macrocycle (Macropa) forms stable complexes with [225Ac]Ac3+. In this work, two PSMA-targeting lysine-ureido-glutamic acid pharmacophores are attached to a single Macropa macrocycle to give MacropaBisPSMA. The new conjugate was radiolabeled with [225Ac]Ac3+ to give [225Ac]AcMacropaBisPSMA. The the

Predictive Value of [-2]proPSA-Related Indices for Prostate Cancer Detection Years Before Prostate-Specific Antigen Elevation: Insights From a Five-Year Longitudinal Screening Cohort.

BACKGROUND: [-2]proPSA(p2PSA)-related indices have demonstrated diagnostic value for detecting prostate cancer (PC) in males with modest increases in prostate-specific antigen levels (PSA) of < 10 ng/mL. However, the "leak point" of p2PSA level in the blood circulation before developing screen-detectable PC remains uncertain. Therefore, this study evaluated the ability of p2PSA-related indices in the years before considering conventional biopsy indications. METHODS: This case-control study analyzed database and serum bank information from a population-based screening cohort in Gunma Prefectur…

False-Positive Prostate-Specific Membrane Antigen (PSMA) PET Uptake in the Vocal Cords Mimicking Metastatic Disease in Prostate Cancer: A Case Report Highlighting Diagnostic Pitfalls.

Prostate-specific membrane antigen positron emission tomography/computed tomography (PSMA PET/CT) is increasingly used in prostate cancer imaging because of its high sensitivity for detecting metastatic disease, even at low prostate-specific antigen (PSA) levels. However, false-positive uptake may occur in benign or inflammatory conditions, creating important diagnostic pitfalls. We report the case of a patient with oligometastatic low-volume prostate adenocarcinoma who presented with biochemical progression and a PSA level of 10.85 ng/mL. Histopathology confirmed a high-grade adenocarcinoma (…

Spurious Uptake on 68Ga-Prostate-Specific Membrane Antigen PET/CT Due to Ankylosing Spondylitis; A Rare Pitfall in Imaging of Biochemical Recurrence of Prostate Cancer.

Bone is one of the most common sites of prostate cancer recurrence, and 68Ga-prostate-specific membrane antigen (PSMA) uptake by benign bone entities poses a diagnostic dilemma. We describe the case of a 60-year-old man with recurrence in a small presacral node on 68Ga-PSMA PET/CT. Of note, the images also demonstrated bilateral asymmetrical sacroiliac joint uptake. A history of ankylosing spondylitis was subsequently elicited, confirming the radiographic suspicion of sacroiliitis, therefore confirming the nonmalignant nature of 68Ga-PSMA uptake related to sacroiliitis rather than osseous recu

Bivalent Inhibitors of Prostate-Specific Membrane Antigen Conjugated to Desferrioxamine B Squaramide Labeled with Zirconium-89 or Gallium-68 for Diagnostic Imaging of Prostate Cancer.

Prostate-specific membrane antigen (PSMA) is a carboxypeptidase that is overexpressed in prostate cancer and is an excellent candidate for targeted diagnostic imaging and therapy. Lysine-ureido-glutamate inhibitors of PSMA radiolabeled with positron-emitting radionuclides can be used for diagnostic imaging with positron emission tomography (PET). A squaramide ester derivative of desferrioxamine B (H3DFOSq) was used to prepare four new agents with either one or two lysine-ureido-glutamate pharmacophores. The H3DFOSq ligand can be used to form stable complexes with either of the positron-emittin

Prostate-specific membrane antigen PET-CT in patients with high-risk prostate cancer before curative-intent surgery or radiotherapy (proPSMA): a prospective, randomised, multicentre study.

BACKGROUND: Conventional imaging using CT and bone scan has insufficient sensitivity when staging men with high-risk localised prostate cancer. We aimed to investigate whether novel imaging using prostate-specific membrane antigen (PSMA) PET-CT might improve accuracy and affect management. METHODS: In this multicentre, two-arm, randomised study, we recruited men with biopsy-proven prostate cancer and high-risk features at ten hospitals in Australia. Patients were randomly assigned to conventional imaging with CT and bone scanning or gallium-68 PSMA-11 PET-CT. First-line imaging was done withi

Five-year trends of bone scan and prostate-specific membrane antigen positron emission tomography utilization in prostate cancer: A retrospective review in a private centre.

INTRODUCTION: In the last 5 years, there has been a significant focus on the use of positron emission tomography (PET) for primary and secondary staging of prostate cancer. In this study, we aim to describe the trends of use between Gallium-68 prostate-specific membrane antigen ligand (Ga-68 PSMA) PET/computed tomography (PSMA PET/CT) and nuclear medicine bone scan (NMBS) for prostate cancer staging in the first institution in Australia to offer both modalities. METHODS: We evaluated trends in prostate cancer staging/restaging imaging modalities at our facility between the time period January

Clinical efficacy of transrectal ultrasound-guided prostate biopsy in men younger than 50 years old with an elevated prostate-specific antigen concentration (>4.0 ng/mL).

BACKGROUND: Prostate cancer (PCa) is not commonly found in men younger than 50 years of age. However, serum prostate-specific antigen (PSA) concentration has been examined more frequently at a younger age in Asia partially due to an increased awareness of prostate cancer. The purpose of our study was to investigate the efficacy and complication of PSA-triggered transrectal ultrasonography-guided prostate (TRUSP) biopsies. We retrospectively reviewed TRUSP biopsies in young men with elevated PSA concentration in Taipei Veterans General Hospital. METHODS: We reviewed the cases of patients young…

The role of choline positron emission tomography/computed tomography in the management of patients with prostate-specific antigen progression after radical treatment of prostate cancer.

CONTEXT: Choline positron emission tomography (PET)/computed tomography (CT) is a currently used diagnostic tool in restaging prostate cancer (PCa) patients with increasing prostate-specific antigen (PSA) after either radical prostatectomy (RP) or external-beam radiation therapy (EBRT). However, no final recommendations have been made on the use of this modality for patient management. OBJECTIVE: To critically analyse the current evidence for the use of choline PET/CT scanning in the management of patients with a progressive increase in PSA after radical treatment for PCa, evaluating its diag…

Delays in cancer detection using 2 and 4-year screening intervals for prostate cancer screening with initial prostate specific antigen less than 2 ng/ml.

PURPOSE: The American Urological Association and American Cancer Society advocate annual screening with serum prostate specific antigen (PSA) and digital rectal examination starting at age 50 years in the general population and earlier in men at high risk. Some groups have suggested that screening at 2 or 4-year intervals may be sufficient in men with initial PSA 2 ng/ml or less. We reviewed the records of men enrolled in a PSA and digital rectal examination based prostate cancer screening study to determine the extent to which the diagnosis of cancer would have been delayed using a 2 or 4-yea
