Differential muscle strength decline in osteoarthritis of the knee. A developing hypothesis.

The ratio of quadriceps to hamstrings muscle strength (Q/H ratio) is important for the stability of the knee and for protection from excessive stress. The purpose of this study was to explore whether the Q/H ratio is altered in patients with osteoarthritis (OA). Subjects were 43 patients with physician-diagnosed OA of the knee (mean age, 65.61 years; SD, 12.74). Isometric knee flexor and extensor strength was measured with a hand-held dynamometer. Both muscle groups were weak, with relatively greater weakness in the quadriceps muscles. The mean Q/H ratio of 1.43 (SD, 0.39) was below ratios rep…

Isokinetic thigh muscle strength after ligament reconstruction in the knee joint: results from a 5-10 year follow-up after reconstructions of the anterior cruciate ligament in the knee joint.

We evaluated the thigh muscle strength using an isokinetic dynamometer in 80 patients 5 to 10 years following anterior cruciate ligament reconstruction. Patients with good results also had good muscle function, both of their flexor and their extensor muscles. Patients with poor results demonstrated reduced muscle strength in both their hamstrings and their quadriceps. Patients with fair functional results demonstrated reduced strength of their extensor muscles compared to their normal leg. However, strength in their operated leg's flexor muscles was unchanged compared to their normal leg. This

Knee proprioception after exercise-induced muscle damage.

The purpose of the present study was to investigate whether exercise-induced quadriceps muscle damage affects knee proprioception such as joint position sense (JPS), force sense and the threshold to detect passive movement (TTDPM). Fourteen young men performed sets of eccentric quadriceps contractions at a target of 60% of the maximal concentric peak torque until exhaustion; the exercise was interrupted whenever the subject could not complete two sets. Muscle soreness, JPS, the TTDPM and force sense were examined before the exercise as well as one, 24, 48, 72 and 96 h after exercise. The resul…

Focal muscle spasticity has little impact on muscle power for walking in people with Traumatic Brain Injury.

BACKGROUND: Spasticity is prevalent following Traumatic Brain Injury. 'Focal' muscle spasticity has been defined as spasticity affecting a localised muscle group, but it's impact on gait kinetics remains unclear. The aim of this study was to investigate the relationship between focal muscle spasticity and gait kinetics following Traumatic Brain Injury. METHODS: Ninety-three participants attending physiotherapy for mobility limitations following Traumatic Brain Injury were invited to participate in the study. Participants underwent clinical gait analysis and were grouped depending on the prese

Cross-education effect of vibration foam rolling on eccentrically damaged muscles.

OBJECTIVES: Previous studies showed that vibration foam rolling (VFR) on damaged muscles improves muscle soreness and range of motion (ROM). VFR intervention can also increase the ROM and pain pressure threshold (PPT) in the non-rolling side, known as a cross-education effect. However, this is not clear for the non-rolling side. Therefore, this study aimed to investigate the cross-education effects of VFR intervention on ROM, muscle soreness, and PPT in eccentrically damaged muscles. METHODS: Participants were sedentary healthy male volunteers (n=14, 21.4±0.7 y) who performed eccentric exerci

Basal protein synthesis rates differ between vastus lateralis and rectus abdominis muscle.

BACKGROUND: In vivo muscle protein synthesis rates are typically assessed by measuring the incorporation rate of stable isotope labelled amino acids in skeletal muscle tissue collected from vastus lateralis muscle. It remains to be established whether muscle protein synthesis rates in the vastus lateralis are representative of muscle protein synthesis rates of other muscle groups. We hypothesized that post-absorptive muscle protein synthesis rates differ between vastus lateralis and rectus abdominis, pectoralis major, or temporalis muscle in vivo in humans. METHODS: Twenty-four patients (62 ±

Deficit in knee extension strength following anterior cruciate ligament reconstruction is explained by a reduced neural drive to the vasti muscles.

The persistence of quadriceps weakness represents a major concern following anterior cruciate ligament reconstruction (ACLR). The underlying adaptations occurring in the activity of spinal motoneurons are still unexplored. This study examined the discharge patterns of large populations of motor units (MUs) in the vastus lateralis (VL) and vastus medialis muscles following ACLR. Nine ACLR individuals and 10 controls performed unilateral trapezoidal contractions of the knee extensor muscles at 35%, 50% and 70% of the maximal voluntary isometric force (MVIF). High-density surface electromyography

Ultrasound-Guided Multi-Branch Rectus Femoris Nerve Block for Spasticity Assessment.

Background: Stiff-knee gait commonly involves rectus femoris spasticity in patients with central nervous system lesions. Diagnostic nerve blocks aid in predicting treatment outcomes; however, current techniques may overlook multiple nerve branches that innervate the rectus femoris muscle, potentially resulting in an incomplete assessment of treatment outcomes. Methods: We present an ultrasound-guided approach that we currently use in our practice, using anatomical landmarks, including the femoral artery, the sartorius muscle, and the rectus femoris' characteristic "J-shaped" internal tendon. AltitudeOmics: effects of 16 days acclimatization to hypobaric hypoxia on muscle oxygen extraction during incremental exercise.

Acute altitude exposure lowers arterial oxygen content ([Formula: see text]) and cardiac output ([Formula: see text]) at peak exercise, whereas O2 extraction from blood to working muscles remains similar. Acclimatization normalizes [Formula: see text] but not peak [Formula: see text] nor peak oxygen consumption (V̇o2peak). To what extent acclimatization impacts muscle O2 extraction remains unresolved. Twenty-one sea-level residents performed an incremental cycling exercise to exhaustion near sea level (SL), in acute (ALT1) and chronic (ALT16) hypoxia (5,260 m). Arterial blood gases, gas exchan

The Effects of 2 Different Soft Tissue Mobilization Techniques on Delayed Onset Muscle Soreness in Male Recreational Athletes: A Single-Blinded Randomized Controlled Trial.

CONTEXT: Soft tissue mobilization is frequently employed for delayed onset muscle soreness (DOMS) management. Foam roller and percussive massage are 2 popular soft tissue mobilization methods preferred by various professionals. However, their effects on DOMS symptoms are controversial and there are no studies comparing these 2 methods. The aim of the present study was to compare the acute effects of soft tissue mobilization with a foam roller or a percussive massage device on DOMS in young male recreational athletes. DESIGN: A parallel, single-blinded, randomized controlled trial. METHODS: T

Whole-body vibration training improves flexibility, strength profile of knee flexors, and hamstrings-to-quadriceps strength ratio in females.

OBJECTIVES: Short-term whole-body vibration training (WBVT) has emerged as an exercise method for improving neuromuscular performance and has been proposed for injury prevention and rehabilitation. This study investigated the effects of a short-term (≤2 months) WBVT program using a side-to-side vibration on: (i) strength profile of knee extensors (KE) and flexors (KF), (ii) "functional" hamstrings-to-quadriceps ratio (ECCKF/CONKE), (iii) flexibility and (iv) vertical jumping performance (VJ). Furthermore, we explored the retention of performance gains 21 days following WBVT. DESIGN: Randomize

The effect of knee joint angle on torque control.

The purpose of the author's investigation was to examine the effect of knee joint angle on torque control of the quadriceps muscle group. In all, 12 healthy adults produced maximal voluntary contractions and submaximal torque (15, 30, and 45% MVC [maximal voluntary contraction]) at leg flexion angles of 15 degrees , 30 degrees , 60 degrees , and 90 degrees below the horizontal plane. As expected, MVC values changed with respect to joint angle with maximum torque output being greatest at 60 degrees and least at 15 degrees . During the submaximal tasks, participants appropriately scaled their to…

[Tendon sheath lipoma associated with quadriceps tendon rupture. Case report].

BACKGROUND: Tendon sheath lipoma is an uncommon tumor characterized morphologically by mature fatty tissue lobes adherent to the tendon or intersecting its fibers. The association with quadriceps tendon rupture is unusual. Etiologic factors that may condition those ruptures have not yet been fully described. However, several authors suggest the existence of a subjacent pathology such as metabolic or inflammatory diseases. Minimal trauma may lead to such ruptures as well as associated ligamentous or bony injuries. The prognosis regarding quadriceps tendon rupture mainly depends on the lap betwe

Injuries to accessory head of rectus femoris in professional football players: anatomical description, imaging findings, and clinical implications.

BACKGROUND: The quadriceps femoris muscle compartment comprises four muscles, including the bi-articular rectus femoris, which is particularly prone to strain in kicking athletes. Anatomical variations such as accessory heads occur frequently but are seldom identified clinically. This retrospective case series describes injuries involving a previously undescribed accessory rectus femoris head in professional footballers. MATERIALS AND METHODS: We retrospectively reviewed a series of cases in professional footballers from 2022-2024 with acute anterior thigh injuries. Inclusion criteria require
