Severe radiation-induced injury after cardiac catheter ablation: a case requiring free anterolateral thigh flap and vastus lateralis muscle flap reconstruction on the upper arm.

Although cases of radiation-induced skin injury after fluoroscopically-guided procedures have been reported since 1996, diagnosis and treatment of such injury remain difficult. We present the case of a patient who complained of two ulcers, one on his right arm and one on his back, and limited motion of the right elbow joint after repeated cardiac radiofrequency catheter ablations. After resection of the skin ulcer on the patient's arm and of degenerated tissue in the distal part of the triceps brachii muscle, a combined free flap incorporating an anterolateral thigh flap and vastus lateralis m

Exosomes Derived from BMSCs Treated with CeONPs Ameliorate Radiation-Induced Jaw Bone Injury via miR-21-5p/STAT3 Axis-Mediated Osteogenesis and ROS Scavenging.

Background/Objectives: Radiation-induced jaw bone injury is a severe and refractory complication following radiotherapy, and the key to treatment is promoting osteogenic differentiation and alleviating oxidative stress injury in irradiated BMSCs. Cerium oxide nanoparticles (CeONPs) exhibit considerable research potential in various oxidative stress injury-related diseases due to their excellent reactive oxygen species (ROS) scavenging capacity; however, its biosafety risk makes direct application in disease treatment a matter of controversy. Methods: Recent evidence suggests that treating cell

An AMPK-Centric Strategy: Foenumoside B Coordinates PINK1/Parkin-Mediated Mitophagy With Nrf2/HO-1 Antioxidant Signaling to Resolve Oxidative Stress in Radiation-Induced Lung Injury.

Radiation-induced lung injury (RILI) is a dose-limiting toxicity of thoracic radiotherapy driven by mitochondrial damage-mediated oxidative stress, persistent DNA damage, and senescence, which together destabilize the alveolar-interstitial niche and promote fibrosis. Here, we identify Foenumoside B (FSB), a natural saponin from Lysimachia foenum-graecum, as a dual-action modulator that preserves mitochondrial quality and restores systemic redox homeostasis to attenuate RILI. In a murine total lung irradiation model (20 Gy), oral FSB (10 mg/kg/day) mitigated inflammation and fibrotic remodeling

[Application of a stent splint to protect intraoral organs from radiation injury in a 97-year-old patient with multiple oral cancers who survived to over 100 years old].

Radiation therapy had been used with increasing frequency in recent years in the management of oral cancers of advanced ages. In those cases we have to take good care to maintain the oral health of patients undergoing cancerocidal dose of radiation therapy. Using splints, as a tissue displacer, during radiation, we could treat a 99-year-old female patient without serious radiation sequelae, successfully she survived over 100 year-old. As she visited us at 97 year-old, the primary lesions located on the left upper lip, nose, upper and lower gums were diagnosed as multiple verrucous carcinoma hi

Immunotherapy may promote the occurrence of radiation-induced brain injury in NSCLC patients with brain metastases undergoing radiotherapy: a retrospective propensity score-matching and inverse probability of treatment weighting study.

PURPOSE: The combination of radiotherapy (RT) and immunotherapy can significantly improve the prognosis of non-small cell lung cancer (NSCLC) patients. However, concerns about whether the synergistic effect may increase the risk of radiation-induced brain injury (RIBI) remain controversial. This study aims to explore the impact of immune checkpoint inhibitors (ICIs) on the occurrence of RIBI in patients with NSCLC brain metastases (BMs). METHODS: This study retrospectively enrolled NSCLC patients with BMs undergoing RT between January 2017 and December 2023. Patients were stratified into grou

Fluoroscopic radiation induced skin reactions: Radiation dermatitis and radiation-induced morphea.

Given the rise of radiation based medical procedures, cutaneous radiation reactions are increasing in frequency. Diagnosis of fluoroscopic radiation-related cutaneous injuries are challenging, as patients are often unaware of or cannot recall radiation exposure. It is important to maintain clinical suspicion of radiation induced skin injuries in patients with persistent morpheaform areas and localized areas of dermatitis or ulceration. Several cutaneous radiation induced injuries have overlapping clinical presentations. Histopathology may be required to help differentiate between these distinc

CCL22 Induces the Polarization of Immature Dendritic Cells into Tolerogenic Dendritic Cells in Radiation-Induced Lung Injury through the CCR4-Dectin2-PLC-γ2-NFATC2-Nr4a2-PD-L1 Signaling Pathway.

Recruitment of immune cells to the injury site plays a pivotal role in the pathology of radiation-associated diseases. In this study, we investigated the impact of the chemokine CCL22 released from alveolar type II epithelial (AT2) cells after irradiation on the recruitment and functional changes of dendritic cells (DCs) in the development of radiation-induced lung injury (RILI). By examining changes in CCL22 protein levels in lung tissue of C57BL/6N mice with RILI, we discovered that ionizing radiation increased CCL22 expression in irradiated alveolar AT2 cells, as did MLE-12 cells after irra

S-RBD-modified and miR-486-5p-engineered exosomes derived from mesenchymal stem cells suppress ferroptosis and alleviate radiation-induced lung injury and long-term pulmonary fibrosis.

BACKGROUND: Radiation-induced lung injury (RILI) is associated with alveolar epithelial cell death and secondary fibrosis in injured lung. Mesenchymal stem cell (MSC)-derived exosomes have regenerative effect against lung injury and the potential to intervene of RILI. However, their intervention efficacy is limited because they lack lung targeting characters and do not carry sufficient specific effectors. SARS-CoV-2 spike glycoprotein (SARS-CoV-2-S-RBD) binds angiotensin-converting enzyme 2 (ACE2) receptor and mediates interaction with host cells. MiR-486-5p is a multifunctional miRNA with ang

Mouse mesenchymal stem cell-derived exosomal miR-466f-3p reverses EMT process through inhibiting AKT/GSK3β pathway via c-MET in radiation-induced lung injury.

BACKGROUND: Radiation-induced lung fibrosis (RILF) is a common complication of thoracic radiotherapy. Alveolar epithelial cells play a crucial role in lung fibrosis via epithelial-mesenchymal transition (EMT). Exosomes derived from mesenchymal stem cells own the beneficial properties to repair and regeneration of damaged tissues, however the underlying mechanisms remain poorly understood. METHODS: Mouse mesenchymal stem cells-derived exosomes (mMSCs-Exo) were isolated by differential centrifugation, and their protective effects were assessed in vivo and in vitro, respectively. Propofol (2, 6-diisopropylphenol) is a potent intravenous hypnotic agent that is widely used in adults and children for sedation and the induction and maintenance of anaesthesia. Propofol has gained popularity for its rapid onset and rapid recovery even after prolonged use, and for the neuroprotection conferred. However, a review of the literature reveals multiple instances in which prolonged propofol administration (>48 hours) at high doses (>4 mg/kg/h) may cause a rare, but frequently fatal complication known as propofol infusion syndrome (PRIS). PRIS is characterized by metabolic acidosis,

Unusual radiologic findings in the thorax after radiation therapy.

Radiation therapy is used to treat many intrathoracic and chest wall malignancies. A variety of changes may occur after radiation therapy to the thorax. Radiation therapy produces dramatic effects in the lung. Pulmonary necrosis is an uncommon, severe, late complication of adjuvant postoperative radiation therapy. Bronchiolitis obliterans with organizing pneumonia is a distinct clinicopathologic entity characterized by patchy, migratory, peripheral air-space infiltrates. Radiation therapy can also cause spontaneous pneumothorax, mesothelioma, and lung cancer. In the mediastinum, radiation ther
