Arachnoid Cyst as a Late Complication of Selective Dorsal Rhizotomy: A Case Report.

Background and importance  Selective dorsal rhizotomy (SDR) is a surgical technique to treat spasticity, mainly in children with spastic cerebral palsy (CP). In this report, a unique case of a late arachnoid cyst, causing radiating pain in the left leg, is presented. This is relevant to clinicians managing the long-term follow-up of patients who underwent selective dorsal rhizotomy (SDR). Clinical presentation  A 25-year-old male with bilateral spastic CP, who underwent SDR at the age of 7, presented with symptoms of progressive radiating pain in the left leg. Magnetic resonance imaging (MRI)

Variants of the adenomatoid odontogenic tumor with a note on tumor origin.

Three rare variants of the adenomatoid odontogenic tumor (AOT) are described. A follicular AOT associated with an impacted and displaced 28, an extra-follicular variant mimicking a radicular cyst around the apex of 23 and a peripheral (epulis-like) variant exhibiting a periodontal bone defect palatal to 21. On reappraisal of the origin and pathogenesis of the AOT, it would seem that this tumor or hamartomatous lesion is derived from odontogenic epithelium of the dental lamina complex or its remnants.

Evaluation of Spontaneous Bone Healing After Enucleation of Large Residual Cyst in Maxilla without Graft Material Utilization: Case Report.

Cystic lesions of the jaws such as keratocysts (scientific community still continues to use the term 'odontogenic keratocyst' more favourably than 'keratocystic odontogenic tumour' although both terms remain acceptable synonyms), follicular cysts, radicular cysts, and residual cysts may reach large proportions, causing considerable bone destruction. Enucleation of cystic lesions in the jaw produces bone defects, which may recover spontaneously or with assisted healing with the use of autogenous bone graft or other bone substitute materials. This clinical study presents a spontaneous bone regen…

ALDH1 expression and potential clinical implications in chronic inflammatory periapical lesions.

Aldehyde dehydrogenase 1 (ALDH-1) is a marker of stem cells in a variety of diseases, but its role in individuals with chronic inflammatory periapical lesions remains unknown. The aim of this study was to investigate the presence of cells with a stem cell profile based on the immunoexpression of ALDH-1 in periapical granulomas (PGs) and radicular cysts (RCs). A total of 51 cases of periapical lesions (25 PGs and 26 RCs) were subjected to immunohistochemical study. The anti-ALDH-1 antibody was applied using the immunoperoxidase technique. An immunoexpression score (intensity vs. percentage of c…

Dental Apical Inflammation Score (DAIS): Histopathological scoring for the evaluation of the apical inflammatory activity and local bone destruction.

OBJECTIVE: The purpose of this study was to evaluate 210 periapical lesions with a newly created Dental Apical Inflammation Score/DAIS with regard to their inflammatory cell infiltration, bone tissue, epithelium, bacteria and foreign material. STUDY DESIGN: Specimens were obtained from 51 different dental practices over a period of 11 months. These specimens were then sent in for histopathological routine diagnostics. RESULTS: The DAIS classified 81 cases of Type 1 (acute inflammation = low, chronic inflammation = low), 79 cases of Type 2 (acute inflammation = low, chronic inflammation = hig

Confocal laser scanning microscopy of liesegang rings in odontogenic cysts: analysis of three-dimensional image reconstruction.

Liesegang rings are concentric noncellular lamellar structures, occasionally found in inflammatory tissues. They have been confused with various parasites, algas, calcification, and psammoma bodies. The authors examined Liesegang rings from oral inflammatory cysts by both optical and confocal laser scanning microscopy, and perfomed a three-dimensional reconstruction. These investigations indicate that Liesegang rings are composed of multiple birefringent concentric rings, resulting from a progressive deposition of organic substances, with an unclear pathogenesis.

Differential diagnosis of large periapical lesions using cone-beam computed tomography measurements and biopsy.

This study compared the differential diagnosis of large periapical lesions (granuloma versus cyst) to traditional biopsy using Cone Beam Computed Tomography (CBCT), the NewTom 3G. Large lesions were scanned and a preoperative diagnosis based on gray value measurements of the imaged lesion area was made. After surgery a biopsy report was obtained and compared to the CBCT diagnosis. In 13 out of 17 cases, the diagnosis coincided. In 4 out of 17 cases, the CBCT read cyst with the oral pathologist's diagnosis being granuloma. Thus, the CBCT may provide a more accurate diagnosis than biopsy and his

Severe sequelae of acute dental trauma in the primary dentition--a case report.

The incidence of traumatic injuries to primary teeth is high. Previous studies report frequencies ranging from 4% to 33% of all children. The damage limitation and the prevention of sequelae are goals that shall direct the treatment for dental trauma. Inappropriate approaches to dental trauma can cause more damage than the trauma itself. This article describes the diagnosis, management and follow-up of a 5-year-old boy who presented with uncommon sequelae of an inappropriate approach following a severe luxation of the primary maxillary right central incisors 2 years ago.

[New technic of surgical treatment of periapical lesions using carbon dioxide laser].

After briefly reviewing the use of the CO2 laser in dentistry, the authors will describe the materials and method for laser radiation after peri-apical surgery. The laser application will result in the sterilization of the apical region. The authors explain the importance of the "LASER-MIRROR" a mini contra-angle head which can maintain the integrity of the apex or minimize apical ressection while reflecting the laser beam at a 90 degree angle. Two case reports will be described and the authors will give the indications and contra-indications of their technic.

The rôle of immunological reactions in apical cyst formation and the fate of epithelial cells after root canal therapy: a theory.

Activation of the epithelial cell rests of Malassez by various means, results in proliferation of these cells and formation of apical periodontal cysts. Several theories for the genesis of apical periodontal cysts have been suggested which are not satisfactory. Available evidence indicates that development and destruction of these lesions are mediated by immunological reactions.
