BACKGROUND: Arthroscopy has been reported to be an efficient adjunct for the surgical treatment for distal radius fractures (DRF). However, performing wrist arthroscopy during palmar locking plate fixation seems to be troublesome. We have developed a surgical technique involving presetting of a palmar locking plate and an arthroscopic reduction technique (PART) of the fracture that can facilitate the procedure. This study is aimed to investigate the effectiveness of our technique in the treatment of DRF.

PURPOSE: To evaluate the radiographic, objective (range of motion and grip strength), and subjective outcomes of variable-angle volar locking plate (VAVLP) fixation for unstable distal radius fractures.
METHODS: Over a 12-month period, we prospectively evaluated 39 consecutive distal radius fractures treated operatively with VAVLPs. BACKGROUND: With an incidence of about 2-4 per 1,000 residents per year, the distal radial fracture is the most common fracture in the human skeleton. The introduction of fixed-angle plate systems for extension fractures at the radius was evaluated in a prospective study performed at our hospital after selection and acquisition of a new system. We have evaluated the clinical application of the Fernandez classification without questioning the scientific validity, by assessing the interobserver reliability and intraobserver reproducibility. A set of 25 radiographs of distal radius fractures were given to six assessors along with details of Fernandez classification. We describe our experience using a new device that results in a bloodless field in open repair of distal radius fractures. The device, an exsanguinating tourniquet (HemaClear model/40, OHK Medical Devices, Haifa, Israel), replaces the traditional methods of limb elevation, Esmarch bandaging, pneumatic tourniquet pressurizing and the associated components. Forty patients (mean age, 37 years) with intraarticular C2 and C3 Colles fractures were treated by open reduction, internal fixation and bone grafting.
