A 65-year-old woman underwent laparoscopic low anterior resection for rectal cancer with distant metastasis to the right lobe of her liver and mediastinal and right subclavian lymph nodes. Since the postoperative chemotherapy resulted in the disappearance of the lymph node metastasis and diminished hepatic metastasis lesion, the enlarged anterior segment of the liver was performed. She was administered chemotherapy for 1 year after the second surgery. A 70-year-old male underwent left pneumonectomy for lung cancer in November 2008. Although he was diagnosed as pT2bN1M0 (stage II B) after the operation, adrenal metastasis was found 8 months later. Penile cancer is a serious but largely under-represented phenomenon in many of the large national cancer databases. Even more rare is the presentation of solitary metastasis to the penis from a gastrointestinal primary site. This case describes one such case of metastasis of rectal adenocarcinoma and details the patient's treatment modalities. Ultimately, although the precise etiology of this particular manifestation is not well understood, the prognosis is poor in the small group that it affects. No individual treatment has been proven superior with regard to long term survival.

BACKGROUND: We report the second case of enteric-type villous adenoma of the vulva. The differential diagnosis, histogenesis, and pathogenesis are discussed.
CASE: A 66-year-old woman had a tumor resected from the posterior aspect of the vulva. The tumor was characterized by its villous architecture, with columnar epithelium and goblet cells. This vulvar tubulovillous adenoma was identical to a tumor resected 6 months before from this patient's rectal wall.
CONCLUSION: These two tumors behaved benignly, and the vulvar adenoma possibly originated with the müllerian vestige.

Ureteral obstruction resulting from metastatic adenocarcinoma is almost always extrinsic in nature. In contrast, true intraluminal metastases are extremely rare. With this report, we document the videoendoscopic appearance of true intraluminal ureteral metastases from metastatic rectal cancer confirmed with histopathologic examination. The value of transureteroscopic biopsy for accurate diagnosis is also demonstrated.

We studied changes in the prognosis of cancer of the rectum (excluding the rectosigmoid junction) from 1978 to 1986 in the French department of Calvados on the basis of the 616 cases in the cancer registry. Taken as whole, survival has improved slightly with time (P < 0.01), but the improvement is only significant for men (P < 0.02), patients under 70 years (P < 0.01) and patients living in urban areas (P < 0.05).
