Central Serous Chorioretinopathy Following Oral Use of Adulterated Honey Mixed with Tadalafil: A Case Report.

BACKGROUND: Central serous chorioretinopathy (CSCR) is an idiopathic condition characterized by serous retinal detachment and/or retinal pigment epithelial (RPE) detachment, the condition associated with a fluid leak through the RPE into subretinal space. This article reports on an unusual case of CSCR following taking adulterated honey mixed with tadalafil powder. CASE PRESENTATION: A 38-year-old male, not known to have any medical illnesses, came to our ophthalmology clinic complaining of sudden onset of blurred central vision in the left eye for five days after taking an adulterated honey

Moyamoya Disease May Mimic Multiple Sclerosis?

INTRODUCTION: A wide range of medical conditions may mimic multiple sclerosis. Among them, cerebrovascular diseases, including moyamoya disease, need to be excluded since they share common clinical features and radiographic findings with multiple sclerosis. CASE REPORT: A 44-year-old woman experienced transient numbness of her right sided face and arm and was referred to our unit due to small brain lesions in magnetic resonance imaging, with a possible diagnosis of multiple sclerosis. Neurological examination was unremarkable except for plantar reflexes and jerky deep tendon reflexes. Brain m

"Who is talking to me?" - Self-other attribution of auditory hallucinations and sulcation of the right temporoparietal junction.

Brain imaging research in schizophrenia has provided a better understanding of the neural basis of auditory hallucinations (AH). Recently, renewed interest in the phenomenology of AH raised questions related to their neural substrates. Hence, the neural basis of AH self/other attribution have yet to be investigated as beliefs regarding the origin of the voices is a cardinal feature of AH phenomenology. As the right temporoparietal junction (TPJ) and the inferior parietal lobule (IPL) play a key role in disentangling the origin of sensory events and in self/other distinction, we tested the hypo

Jugular foramen syndrome caused by choleastatoma.

Unilateral paresis of 9th-11th cranial nerves together is defined as jugular foramen (Vernet's) syndrome. A cholesteatoma case that penetrated into intracranial area after eroding temporal bone and led to jugular foramen syndrome is presented here, since such a case has not been reported in the literature hitherto. A 46-year-old male patient was evaluated for hoarseness. It was learnt from his anamnesis that he had been operated for otitis media nearly 20 years ago. Uvula deviated towards right. Loss of gag reflex was remarkable on the left. Paresis was found on the left vocal cord. There was

[Usefulness of establishing diagnosis and severity of the most frequent signs and symptoms in preeclamptic patients].

OBJECTIVE: Our aim was to determine that signs and symptoms are tools in establishing diagnosis and severity of preeclampsia. MATERIALS AND METHODS: Our study design was prolective, comparative, cross-sectional for evaluation of diagnosis. Our sample included 408 patients. The study employed classification criteria of the American College of Obstetricians and Gynecologists. One blinded family physician interrogated and examined each patient. The sample included patients with recent diagnosis and without treatment. Patients with HELLP syndrome, eclampsia, and those in Intensive Care were exclu

Effect of colonic distention on ileal motor activity with evidence of coloileal reflex.

Chyme delivery from the ileum to the colon is controlled by various neurologic and hormonal factors, many of which remain to be identified. In this report we investigated the effect of colonic distention on ileal motility with the aim of identifying the mechanism of chyme delivery from the ileum to the colon. The right colon of 16 healthy volunteers (12 men and 4 women; mean age 36+/-9 years standard deviation) was distended by a balloon that was filled with saline solution in increments of 20 ml. The pressure response of the terminal ileum to the colonic distention was recorded by a saline-pe

[Polymyositis with rimmed vacuoles in a case of chronic thyroiditis].

We report a 37-year-old woman who developed chronic thyroiditis, and polymyositis with rimmed vacuoles (RVs). She developed paresthesia of glove and stocking type, muscle weakness of the lower extremities, and slowness in motion. On examination, her lower legs were atrophic and weak, more prominent on the right, while proximal muscles in the upper and lower extremities were slightly affected. Deep tendon reflexes of the upper and lower extremities were normal except for the absent Achilles tendon reflexes. Serum CK was high (1,809 IU/l), and laboratory data indicated chronic thyroiditis and hy

Implication of photic sneeze reflex in ophthalmology.

BACKGROUND: Photic sneeze reflex (PSR) also known as autosomal dominant compelling helioophthalmic outburst is a rare condition characterized by uncontrolled sneezing in response to bright light. The exact mechanism causing this is poorly understood. However, various hypotheses have been proposed. Ophthalmic examination involves exposing the patient to bright light like slit lamp, indirect ophthalmoscopy, and surgical microscope, which can trigger sneezing in PSR patients. PURPOSE: The purpose of this video is to draw light to this rare phenomenon and its implication in ophthalmic surgery. S

The Primitive Palmomental Reflex in Amyotrophic Lateral Sclerosis.

BACKGROUND AND PURPOSE: The palmomental reflex (PMR) is a primitive reflex that might be released due to inhibition in adulthood. It has been associated with several neurodegenerative conditions. The aim of the present study was to evaluate the frequency of PMR in amyotrophic lateral sclerosis (ALS). PATIENTS AND METHODS: Non-demented ALS patients (n = 179) were recruited. Two groups of disease controls were enrolled: (a) non-demented patients with other neurological disorders (NC; n = 86, mean age 60 ± 14 years); (b) healthy subjects, healthy controls (HC; n = 175, mean age 61 ± 12 years). Rifle impulse noise affects middle-ear compliance in soldiers wearing protective earplugs.

OBJECTIVE: We tested middle-ear functioning in humans following intense exposure to noise. Noise generated by small caliber firearms was thought to have no effect on the middle-ear. DESIGN: A cross-over design. We measured middle-ear impedance, acoustic reflex, distortion product otoacoustic emissions (DPOAEs), and transient evoked otoacoustic emissions (TEOAEs) before and after practice rounds performed twice per day. STUDY SAMPLE: Fifty-nine soldiers equipped with earplugs undergoing regular training for a special mission. The mean noise exposure (LAeq8h) was estimated to be 106 ±1 dB SPL.

Antisaccade generation is impaired after parietal lobe lesions.

Antisaccades are directed away from visual targets. Impaired antisaccade generation has been attributed to frontal lobe damage. We studied antisaccades in patients with unilateral focal parietal lobe lesions. Normal subjects (N = 10) instructed to make 10° antisaccades opposite to a 100-ms target flash 10° to the right or left of center made antisaccades in 86.1% of trials. Patients (N = 13) made antisaccades contraversive to their lesions in 55.4% of trials and 50.5% of ipsiversive trials. In other trials, reflexive saccades occurred toward the target flash. Nine patients with imaged lesions

[A 28-year-old woman with newly-onset urinary incontinence].

28 year-old woman developed sensory loss in the gluteal region and involuntary micturition during sleep. After two days she arrived at the Department of neurology. Examination disclosed absence of the right ancle reflex (S1), sensory loss in the saddle area including the dermatomes S1-S5, absence of the ano cutan (S3-S5) and bulbocavernous reflexes (S3-S4). She had sphincter disturbance and a sensory palsy of the bladder, but no retention. She experienced no vertebral, radicular or other pain at any time. Because of the lack of pain, the tentative diagnosis was acute myelitis, and a CT scan wa

Effect of the alpha-adrenergic blocking agent thymoxamine on the neurogenic bladder and urethra.

The effect of the alpha-adrenergic blocking agent thymoxamine by intravenous administration was studied in 25 patients with spastic paraplegia and uninhibited neurogenic bladder. By cystometry a shift to the right of the first desire to void, the threshold of the first uninhibited contraction and a reduction of the pressures of the uninhibited contractions was found, but the bladder capacity was unchanged. The urethral pressure profile studied in 10 patients showed reduction in most and the peak value in all the patients. EMG from the external urethral sphincter in five patients showed damping

Nerve pathways in celiac plexus of the guinea pig.

In vitro preparations consisted of the right and left celiac, superior mesenteric and inferior mesenteric ganglia with attached extrinsic nerves, vasculature, mesentery, and colon. There were no systematic differences in membrane electrical properties (recorded intracellularly) between neurons in the different ganglia. Stimulation of associated nerve trunks produced graded synaptic responses in plexus neurons. Presynaptic fibers were found in splanchnic and mesenteric nerves. Input from celiac nerves dominated in the celiac galglia; input from the intermesenteric fibers dominated in the superi

Robust health-score based survival prediction for a neonatal mouse model of polymicrobial sepsis.

Infectious disease and sepsis represent a serious problem for all, but especially in early life. Much of the increase in morbidity and mortality due to infection in early life is presumed to relate to fundamental differences between neonatal and adult immunity. Mechanistic insight into the way newborns' immune systems handle infectious threats is lacking; as a result, there has only been limited success in providing effective immunomodulatory interventions to reduce infectious mortality. Given the complexity of the host-pathogen interactions, neonatal mouse models can offer potential avenues p

Spontaneous triple flexion after thalamo-capsular haemorrhagic stroke.

A 52-year-old woman presented to the emergency room with a right thalamo-capsular haemorrhage exhibiting involuntary, stereotyped movements in her left lower limb. These movements started at the same time as ictal deficits and consisted of hip and knee flexion, foot dorsiflexion and extension of the hallux, resembling a triple flexion response. They occurred spontaneously, but could also be elicited by stimulating pathological reflexes. Such semiological finding could first be interpreted as a hyperkinetic manifestation resulting from a lesion in the basal ganglia; however, these movements do

'We're Not Really Into Helping, We're Into Being Useful': Service Provider Practices That Support Belonging Opportunities in the Lives of Adults Labelled With Intellectual Disabilities.

BACKGROUND: Social inclusion focussed on belonging is increasingly emphasised by disability advocates and policymakers, yet belonging often remains absent for adults labelled with intellectual disabilities. People labelled with intellectual disabilities often rely on service providers to support their belonging. METHODS: In this critically informed ethnographic work, we draw on interviews with nine disability services leaders and prolonged participatory field explorations with five adults labelled with intellectual disabilities, their friends and support workers to explore how service provide

Neurophysiological mechanisms involved in tonic immobility (TI).

This chapter summarizes the main neurophysiological characteristics of tonic immobility (TI), in many susceptible species of mammals and birds. During TI, cortical EEG shows high voltage slow waves whose amount is affected by events preceding TI induction and is positively correlated with TI duration. The pattern of hippocampal activity helps to predict TI onset and TI termination. Both polysynaptic flexor and monosynaptic heteronymous reflexes are depressed independently from the EEG activity. Brain metabolism, signaled by glycogen mobilization and glucose utilization, indicates a reduced neu

Cauda equina syndrome following combined spinal-epidural anesthesia with levobupivacaine for cesarean section.

Cauda equina syndrome (CES) is a rare but possible complication of neuroaxial anesthesia. Damage to the nerve roots may occur due to compression, inflammation, stretching, direct trauma, spinal ischemia or neurotoxicity, usually with lidocaine or bupivacaine. We describe a case of a 33-year-old patient that underwent an uneventful cesarean section with a combined spinal-epidural technique anesthesia, with levobupivacaine. 48 h after the procedure, she presented diminished muscular strength and abolished osteotendinous reflexes in the left lower limb, limited flexion of the right hallux, urinar
