Acute paralysis following "a bad potato": a case of botulism.

PURPOSE: Intensivists often encounter patients with respiratory failure as a result of neuromuscular disease, however, acute neuro-muscular syndromes are less common. We present a case of food borne Clostridium botulism and discuss the diagnostic and therapeutic considerations. CLINICAL FINDINGS: A 35-yr-old healthy male presented with abdominal pain and blurred vision 12 hr after ingesting a "bad" potato. During the next 17 hr, the patient demonstrated a gradual descending paralysis which ultimately resulted in no cranial nerve function and 0/5 strength in all extremities. Sensation was inta

Daytime hypoxemia, sleep-disordered breathing, and laryngopharyngeal findings in multiple system atrophy.

BACKGROUND: The mechanism underlying nocturnal sudden death in patients with MSA remains unclear. It may be explained by upper airway obstruction, such as vocal cord abductor paralysis; an impairment of the respiratory center, such as Cheyne-Stokes respiration; or an impaired hypoxemic ventilatory response. OBJECTIVE: To investigate the mechanism of sleep-disordered breathing in multiple system atrophy (MSA). DESIGN: We recruited 21 patients with probable MSA who were admitted sequentially to our hospital, and performed daytime blood gas analysis, pulmonary function tests, polysomnography, a

Zika virus infection preceding Guillain-Barré syndrome: Two cases from Southern India.

Guillain-Barré syndrome (GBS) is a leading cause of acute flaccid paralysis and has been associated with several arboviral infections. It is an acute immune-mediated polyradiculo-neuropathy characterised by rapidly progressive limb weakness, areflexia, and variable cranial nerve involvement. Antecedent infections are reported in the majority of patients, most commonly gastro-intestinal or respiratory. In recent years, arboviruses such as Dengue, Chikungunya, and Zika have emerged as important infectious triggers of GBS, particularly in tropical regions. Zika virus infection is often asymptomat

Guillain-Barre syndrome complicated by acute fatal rhabdomyolysis.

Guillain-Barre syndrome (GBS) is a heterogenous group of peripheral-nerve disorders with similar clinical presentation characterized by acute, self-limited, progressive, bilateral and relatively symmetric ascending flaccid paralysis, which peaks in 2-4 weeks and then subsides. The usual complications, which occur in a patient of GBS are pneumonia, sepsis, pulmonary embolism, respiratory insufficiency and cardiac arrest. The clinical course of GBS complicated by acute rhabdomyolysis is extremely rare. We present the case of GBS with marked elevation in serum creatine kinase, serum myoglobin lev

Isolation and partial characterization of a toxin from the venom of the East African orthognath spider Pterinochilus spec.

A minor basic polypeptide in the venom of the East African orthognath spider Pterinochilus spec. was the most toxic of 16 basic components in this venom. This toxin A4/4 was purified by gel permeation on Sephadex G-50 and ion exchange chromatography on CM-Sephadex C-25 with a yield of 1.3% of the total protein content of the crude venom. The polypeptide has an isoelectric point of 9.39 and molecular weight of 10,500. The s.c. LD50 in mice is 0.1 mg/kg. Death occurred due to respiratory paralysis. After administration of sublethal doses rapid reversibility of the symptoms and total recovery of …

Congenital airway anomalies.

Congenital airway anomalies (CAA) include a variety of conditions that cause respiratory distress in neonates and infants. These malformations occur at various anatomic levels and manifest in a wide spectrum of airway symptoms, with presentation significantly influenced by the level at which obstruction occurs as well as by the severity of obstruction. The prevalence of congenital airway malformations has been estimated to range between 0.2 and 1 in 10,000 live births. The most frequent CAA are laryngomalacia, bilateral vocal cord paralysis, subglottic stenosis, laryngeal webs, subglottic hema

An Autopsy Case of Idiopathic Pleuroparenchymal Fibroelastosis with Left Vocal Cord Paralysis and a Rapid Deterioration without an Acute Exacerbation.

Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is a generally slow-progressing rare disorder of unknown etiology. The direct cause of death in cases of IPPFE is rarely investigated. We experienced an autopsy case of a Japanese man with IPPFE and found aspiration pneumonia to be the major trigger of death. The individual had left vocal cord paralysis at admission, which may have contributed to aspiration pneumonia, and which probably was affected by the fibrous adhesion of the left apex of the chest wall resulting from IPPFE. The prevention of aspiration pneumonia is important for maintain

Total Spinal Anesthesia Following an Interscalene Block Treated with Intravenous Lipid Emulsion.

Total spinal anesthesia following interscalene block is a rare and life-threatening complication of regional anesthesia. A 56-year-old woman underwent an uncomplicated left shoulder bone spur removal under general anesthesia with an interscalene nerve block at an outpatient surgical center. Subsequently, she developed bilateral mydriasis, paralysis of all extremities, and respiratory arrest. She was intubated and transferred to the emergency department (ED) where she was given intravenous lipid emulsion (ILE) with complete recovery of neurological function. ILE therapy may be considered as a r

Recurrence of childhood Guillain-Barré syndrome after a long asymptomatic interval: a case report.

We present a patient who rapidly developed flaccid paralysis in all 4 limbs on 2 separate occasions 19 years apart. Each episode was accompanied by a respiratory failure that required intubation. Both times, clinical response to immunotherapy was favorable with a near complete recovery. Clinical course, laboratory and electrodiagnostic findings, and favorable response to therapy suggest 2 separate attacks of Guillain-Barré syndrome. The reported case adds to a small series of cases of recurrent Guillain-Barré syndrome after a long asymptomatic period. In such cases, the distinction from chroni

Photo-evoked eyelid microvibration in newborn infants and children--reflex arc and maturational change.

We studied maturational process of the photo-evoked eyelid microvibration (PEMV) in 166 newborns, 72 children and 10 adults free of any neurologic disorders. The results indicate that the latency of PEMV shortens during growth due to the myelination of the nerve fibers of the involved pathway. Age-related normal values were established. The pathway of PEMV probably consists of retina, optic nerve, pretectum, facial nerve, and orbicularis oculi muscle. We also evaluated the effect on this reflex of retinoblastoma, facial nerve paralysis, holoprosencephaly anomalad, and a muscle relaxant used fo

Rapid viral diagnosis by immunofluorescence reactions.

Data are reviewed that illustrate the efficacy of the immunofluorescence (IF) technique as a method of rapid viral diagnosis. Its constant application in the "Stefan S. Nicolau" Institute of Virology has allowed the detection of viral and/or inframicrobial antigens in cells from patients with eye diseases (keratoconjunctivitis, keratitis, uveitis), stomatitis, skin diseases, benign and malignant gynecopathies, male urethritis, respiratory diseases, facial paralysis, etc. The IF technique has an incontestable advantage as regards the detection of the simultaneous presence of several infectious

Vocal Cord Paralysis After Thoracic Aortic Surgery: Incidence and Impact on Clinical Outcomes.

BACKGROUND: Vocal cord paralysis (VCP) is a serious complication associated with thoracic aortic surgery; however, there is a paucity of literature regarding the incidence and impact of VCP on postoperative outcomes. We sought to determine the incidence of VCP and its impact on clinical outcomes in patients who underwent thoracic aortic repair at our center. METHODS: A retrospective chart review was conducted on all patients who underwent thoracic aortic surgery between January 2009 and September 2012. RESULTS: A total of 259 patients underwent a thoracic aortic procedure during the study pe

Subglottic high frequency jet ventilation in surgical management of bilateral vocal fold paralysis after thyroidectomy.

Lesion of the recurrent laryngeal nerves as a consequence of thyroid surgery results in bilateral vocal fold paralysis and respiratory obstruction. The initial treatment involves ensuring an adequate airway and it ranges from tracheostomy to endo-extralaryngeal laterofixating operations in general anesthesia. Subglottic high frequency jet ventilation (HFJV) is an alternative ventilatory approach in airway surgery. HFJV offers optimal endolaryngeal working conditions, immobility of vocal cords, adequate oxygenation and ventilation. The HFJV was prospectively studied in 20 consecutive female pat

The occlusion tests and end-expiratory esophageal pressure: measurements and comparison in controlled and assisted ventilation.

BACKGROUND: Esophageal pressure is used as a reliable surrogate of the pleural pressure. It is conventionally measured by an esophageal balloon placed in the lower part of the esophagus. To validate the correct position of the balloon, a positive pressure occlusion test by compressing the thorax during an end-expiratory pause or a Baydur test obtained by occluding the airway during an inspiratory effort is used. An acceptable catheter position is defined when the ratio between the changes in esophageal and airway pressure (∆Pes/∆Paw) is close to unity. Sedation and paralysis could affect the a

Two case reports and a literature review of typical GBS and rare GBS variants associated with COVID-19.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) emerged in late 2019, and is the infectious agent that caused the coronavirus disease 2019 (COVID-19) pandemic. Although respiratory and gastrointestinal manifestations of SARS-CoV-2 are well defined, the spectrum of neurological involvement is less defined. The classic type of Guillain-Barré syndrome (GBS) progresses over days to weeks and has a monophasic course. Areflexia/hyporeflexia and ascending and symmetrical paralysis are observed clinically in patients. It is an autoimmune process that typically leads to the destruction of

Respiratory obstruction as a sign of brainstem dysfunction in infants with Chiari malformations.

Laryngeal respiratory obstruction associated with Chiari malformations was first described in 1932. We studied this type of obstruction in six children with one or several disorders pointing to brainstem dysfunction (failure to thrive, velopharyngeal incompetence, gastroesophageal reflux, or vagal hypertonia). The nature of the laryngeal obstruction was highly variable (vocal cord paralysis, paradoxical vocal cord motion, laryngomalacia) as were the frequency and severity of associated disorders. Chiari malformations should be routinely sought in a child with laryngeal respiratory obstruction

Concurrent glottic and tracheal stenoses: restoration of airway continuity in end-stage malignant disease.

Six patients known to have inoperable esophageal carcinoma presented with stridor due to both malignant tracheal stenosis (with additional respiratory-digestive tract fistula in 2 patients) and bilateral vocal cord paralysis. Patency was restored by endotracheal stenting plus unilateral cordotomy. Four patients had immediate relief. Two patients required enlargement of the vocal cord incision. One of them declined reoperation and underwent tracheostomy. The stent function was uneventful, with no dislodgment or mucus impaction. The fistula seal was complete, with no aspiration through the newly

The cholinergic properties of some amino-acid esters and amides.

1. Several amino-acid esters and amides have been prepared and their toxicological and pharmacological properties have been investigated. Some of the quaternary esters and amides were lethal to mice at doses below 1 mg/kg; this lethality was usually associated with high nicotinic activity. None of the compounds showed high muscarinic activity.2. The cause of death was asphyxia due to paralysis of the respiratory muscles, but at higher doses there was also failure of the medullary respiratory centre.3. Administration of sub-lethal doses of nicotine to mice was shown to protect the animals from

Unexpected Neurological Deterioration Following Occipitocervical Fusion Surgery: A Case Report.

Occipitocervical posterior decompression and fusion (O-C fusion) surgery is occasionally required for treating high cervical myelopathy due to atlantoaxial subluxation. The advance of the instrumentation systems has led to favorable clinical outcomes following O-C fusion surgery. However, the rate of perioperative complications in O-C fusion surgery is relatively high, including instrumentation failure, respiratory complications, and dysphagia. Here we report a rare case involving an unexpected deterioration of myelopathy following O-C fusion surgery. A 49-year-old male was transported to our
