Disorders of pulmonary function, sleep, and the upper airway in Charcot-Marie-Tooth disease.

Charcot-Marie Tooth disease (CMT) encompasses several inherited peripheral motor-sensory neuropathies and is one of the most common inherite … omitant restrictive pulmonary impairment. The prominence of peripheral neuropathy as a cause of the restless legs syndrome in CMT may justify treatment with neuropathic medications as opposed to the more commonly recommended dopaminergic agents. The risk of progression to bilateral vocal cord dysfunction in CMT and the risk of aspiration with laryngeal neuropathy may li …

Orthostatic tremor in progressive supranuclear palsy.

Patients with orthostatic tremor (OT) can be classified as having "primary OT," with or without postural arm tremor but no other abnormal neurological features, or "OT plus." We describe a patient with OT, with postural tremor of the arms and restless legs syndrome (RLS), who developed features typical of progressive supranuclear palsy (PSP). PSP can be accompanied by OT.

[Peridural morphine administration in restless legs status].

The restless-legs syndrome is known to occur in a familial form. The paraesthesias, which are hard to define, are sometimes compared to a painful sensation that can be alleviated only by movement. The pathomechanism of the syndrome and its aetiology are still unknown. A 67-year old patient was admitted as an emergency with most severe pain in her legs. In her family the syndrome occurred in an autosomal dominant form. A long-standing history of abuse of various analgesics was known. Initially the symptoms could not be influenced by any of the various drugs given; even epidural bupivacaine show

Management of common sleep disorders.

Sleep disorders are common and affect sleep quality and quantity, leading to increased morbidity. Patients with sleep disorders can be categorized as those who cannot sleep, those who will not sleep, those with excessive daytime sleepiness, and those with increased movements during sleep. Insomnia, defined as difficulty initiating or maintaining sleep that results in daytime impairment, is diagnosed using history findings and treated with cognitive behavior therapy, with or without sleep hypnotics. Restless legs syndrome is characterized by an urge to move the legs that worsens with rest, is r…

[Doctor, I have pain and pins and needles in my legs at night].

Restless legs syndrome (RLS) is a frequent chronic condition. It causes discomfort in the lower limbs with an urge to move the legs and sometimes paresthesias. It's frequently associated with sleep and mood disorders causing a significant impact on quality of life. There are four clinical criteria to diagnose it. Treatment includes management of reversible factors and if needed symptomatic treatment. Depending on symptoms severity, non-drug measures can be tried. First-line medication treatment should be dopaminergic agonists. Second-line treatments include, anticonvulsivants (gabapentine), be…

Thomas de Quincey and his restless legs symptoms as depicted in "Confessions of an English Opium-Eater".

Thomas de Quincey, a British writer of 19th century, suffered insomnia from the age of 17 years. In his famous "Confessions of an English-Opium Eater" (1822), he described a symptomatology that could concord with restless legs syndrome long before he became addicted to opium. In this report, we analyze his clinical description and the circumstances leading to his opium addiction.

Dopaminergic drugs are useful for the treatment of a common sleep disorder in CKD, the restless legs syndrome. However, experiments on animal models of CKD show conflicting results regarding a modification of dopamine neurons. KEY MESSAGES: Several observations suggest a limited relevance of the dopaminergic system in CKD-related cognitive impairment. Howe …

Mechanistic Overlaps Between Sleep and Headache Disorders: From Dopaminergic Dysfunction to Neuroinflammation-A Narrative Review.

Sleep disorders and primary headache syndromes frequently coexist, and accumulating evidence suggests that this relationship is bidirectional and biologically mediated rather than coincidental. Patients with migraine, tension-type headache, and cluster headache commonly report poor sleep quality, insomnia symptoms, and irregular sleep patterns, while individuals with sleep disorders such as insomnia, obstructive sleep apnea, restless legs syndrome, and narcolepsy experience a higher prevalence, severity, and chronification of headache disorders. This narrative review synthesizes current clinic…

Do periodic leg movements influence patients' perception of sleep quality?

BACKGROUND AND PURPOSE: Periodic leg movements in sleep (PLMS) are a common finding in various sleep disorders. Whether PLMS are an epiphenomenon or are causally related to the presence of sleep-wake disturbances is still being debated. We investigated the relationship of the occurrence of PLMS to patients' perception of sleep quality during a night of polysomnography in various sleep disorders. METHODS: The retrospective evaluation included PLMS recordings over two nights of 78 consecutive patients diagnosed with a restless legs syndrome, primary insomnia or insomnia associated with a psychi…

Conduct problems and symptoms of sleep disorders in children.

OBJECTIVE: Conduct problems and hyperactivity are frequent among children referred for sleep-disordered breathing (SDB), restless legs syndrome, or periodic leg movements during sleep (PLMS), but children not referred to sleep centers have received little study. METHOD: Parents of children aged 2 to 14 years were surveyed at two general clinics between 1998 and 2000. A Pediatric Sleep Questionnaire generated validated scores for SDB and PLMS. The Conners Parent Rating Scale (CPRS-48) produced an age- and sex-adjusted Conduct Problem Index (CPI) and Hyperactivity Index. RESULTS: Parents of ab…

Pseudo-Sjögren syndrome with hyperlipoproteinemia.

Eight patients with parotid enlargement were seen because of varying musculoskeletal complaints. All were overweight and hypertensive; six had diabetes mellitus, four had palpable hepatomegaly, and six were 49 years or older. One patient had arthritis type IIB hyperlipoproteinemia. Four patients had varying degrees of fibrositis, and two of these also had osteoarthritis. Of the others, one each had gout, Reiter syndrome, and restless legs syndrome. All had hyperlipoproteinemia type IV or IIB, and none abused alcohol. None of these patients have true Sjögren syndrome.

The correlation between RLS and motor or other non-motor symptoms of PD patients: an observational study.

BACKGROUND: Numerous studies have demonstrated restless legs syndrome (RLS) might worsen motor and non-motor symptoms in patients with Parkinson’s Disease (PD). However, research into the effects of concurrent RLS on the function of the autonomic nervous system remains limited. Our study particularly focused on its effects on the autonomic nervous system. METHOD: From October 2022 to February 2025, 392 patients with PD were continuously included in our study. PD patients were categorized into those with RLS and those without RLS, based on the criteria established by the International Restless …
