Incentive spirometry to reduce pulmonary complications after rib fractures.

Incentive spirometry (IS) is a therapeutic technique to promote maximal inspiration with breathing exercises via a handheld device which gives visual feedback, with the aim of reducing pulmonary complications after surgery. A short cut review of the literature was carried out to examine whether there is evidence for the use of IS as a treatment modality to reduce complications following rib fractures. 157 unique papers were found, of which 3 (a randomised controlled trial, substudy of a prospective observational cohort study and a retrospective observational study) included data on patients re

Pilot study comparing three analgesia approaches for rib fractures.

BACKGROUND: Rib fractures in the elderly are associated with pulmonary complications and mortality. The erector spinae plane (ESP) block might reduce pain and improve pulmonary function. Lidocaine infusions offer a non-invasive alternative delivery of local anesthetic. This pilot study was designed to inform the design of a future large trial. METHODS: Patients ≥ 55 years of age with at least three unilateral rib fractures were randomly assigned to an ESP catheter, lidocaine infusion, or multimodal analgesia alone. Baseline measurements included the Duke Activity Status Index (DASI) score. Pa

Rib quality assessment using quantitative ultrasound imaging of bone.

This feasibility study evaluated quantitative ultrasound imaging of bone (QUSIB) for non-ionizing assessment of rib quality in the context o … irmed sensitivity to pathological changes. In-vivo QUSIB measurements at the antero-lateral 4th-6th ribs and at the tibia midshaft in n = 10 healthy volunteers produced apparent integrated backscatter and attenuation values that closely matched in-silico distributions (p > 0.01) and did not differ significantly from tibia measurements. These results de …

Entropy-guided partial annotation for cross-domain rib segmentation.

Deep learning methods have been widely used in medical imaging, including rib segmentation. Nevertheless, their dependence on large annotate … also effectively addresses the challenges of migrating from source to target domains. On the public RibSegV2 dataset (source domain) and a private chest CT rib segmentation dataset (target domain), EGPA achieved Dice scores of 89.5 and 90.7, respectively, nearly matching the performance of fully supervised models (89.9 and 91.2) with only 19 % and 18 % …

Prospective multicenter external validation of the rib fracture frailty index.

BACKGROUND: The Rib Fracture Frailty (RFF) Index is an internally validated machine learning-based risk assessment tool for adult patients with rib fractures that requires minimal provider entry. Existing frailty risk scores have yet to undergo head-to-head performance comparison with age, a widely used proxy for frailty in clinical practice. Our aim was to externally validate the RFF Index in a small-scale implementation feasibility study. METHODS: Prospective observational cohort study conducted across five ACS COT-verified trauma centers. Participants included ≥18-year-old adults presentin

Factors associated with discharge to home after traumatic rib fractures.

BACKGROUND: Chest wall injury is common among trauma patients. Generally, patients with more medical problems tend to have worse outcomes with rib fractures. Our aim was to determine if surgical stabilization of rib fractures (SSRF) increases the likelihood of discharge to home. METHODS: We performed a retrospective cohort study of patients with ≥1 comorbidity, aged 18-99 with rib fractures from five Chest Wall Injury Society (CWIS) Collaborative Centers. Discharge disposition of patients who underwent SSRF was compared to those who did not undergo surgery. Discharge to home was considered th

Vascularized rib graft for proximal scaphoid reconstruction: An anatomical study.

The loss of cartilaginous substance at the proximal pole of the scaphoid bone poses a therapeutic challenge, with 10% of fractures in this area progressing to pseudarthrosis and necrosis of the proximal pole. Our aim is to propose a conservative surgical alternative involving replacing the necrotic scaphoid with a vascularized osteochondral graft taken from the fourth rib. Angiography of the donor site was performed on seven hemithoraxes to analyze the osteochondral vascularization. Twenty vascularized osteochondral grafts were then performed on anatomical subjects after the proximal pole of t

Surgical rib fixation in patients with cardiopulmonary disease improves outcomes.

INTRODUCTION: The role of rib fixation (RF) in flail chest is debated, and guidelines conditionally recommend RF in highly selected patients. Patients with cardiopulmonary disease (CPD) have traditionally not been deemed surgical candidates. We hypothesize that RF would benefit even high-risk patients with CPD. METHODS: Adult patients with isolated flail chest and CPD were identified from the Trauma Quality Improvement Program database (2016-2018). Hospital transfers, patients dead within 72 h, penetrating mechanism, concomitant thoracic aortic injury or cancer were excluded. Primary outcome

Burosumab treatment of tumour-induced osteomalacia from a rib lesion.

Tumour-induced osteomalacia (TIO) is a rare cause of acquired hypophosphataemia most often due to paraneoplastic overproduction of fibroblast growth factor-23 (FGF-23). A woman in her 60s presented with an 18 month history of insidious back, knee and ankle pain, and progressive weakness. Biochemistry revealed low serum phosphate and a bone scan showed diffuse bony lesions, initially suspected to be metastatic sarcoma given her history of uterine sarcoma. However, a positron emission tomography scan identified a solitary destructive lytic lesion in the right posterior second rib. Endocrine work

Prevalence and Risk Factors Associated with Rib Lesions in Dairy Cows.

Unilateral or bilateral hard bony enlargement is sometimes palpated over dairy cows' ribs. Although usually not painful, these injuries may have welfare and economic implications and can be used as indicators of poor husbandry conditions. The objective of this study was to determine the prevalence of rib lesions in dairy cows kept in intensive systems and to identify the risk factors either associated with the housing or with the animal's clinical history. We examined the ribs of all 1319 lactating cows from 22 Portuguese dairy farms. In each farm, housing and animal potential risk factors wer…

The burden of readmissions after rib fractures among older adults.

BACKGROUND: The index hospitalization morbidity and mortality of rib fractures among older adults (aged ≥65 years) is well-known, yet the burden and risks for readmissions after rib fractures in this vulnerable population remain understudied. We aimed to characterize the burdens and etiologies associated with 3-month readmissions among older adults who suffer rib fractures. We hypothesized that readmissions would be common and associated with modifiable etiologies. METHODS: This survey-weighted retrospective study using the 2017 and 2019 National Readmissions Database evaluated adults aged ≥6

Surgical stabilization of rib fractures in anticoagulated patients: Proceed with caution?

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly performed, however the outcome of patients undergoing SSRF while on pre-injury antithrombotic therapy remains unknown. We compared surgical variables and outcomes of patients who were and were not on antithrombotic therapy. We hypothesize pre-injury anticoagulation is associated with delay in SSRF and worse outcomes. METHODS: For this retrospective cohort study, we queried the Chest Injury International Database, for patients undergoing SSRF between 08/2018 and 03/2022. Antithrombotic therapy was categorized into antip

Inpatient Outcomes of Intercostal Nerve Cryoablation With Surgical Rib Fixation.

INTRODUCTION: Rib fractures are associated with significant pain and morbidity. Intercostal nerve cryoablation (INCA) offers targeted, prolonged pain relief for these patients. Over the last decade, more patients have undergone surgical stabilization of rib fractures (SSRF) after injury. However, data on INCA use in SSRF patients are limited. This study aimed to identify the relationship of INCA in blunt trauma patients (BTPs) undergoing SSRF, hypothesizing INCA coupled with SSRF would decrease hospital length of stay (LOS). METHODS: The Trauma Quality Improvement Program database (2017-2021)

Palliative Care Rounds: A Thanksgiving Break to Discuss Rib Fractures.

Rib fractures are common injuries, especially in the frail and elderly. They can happen in isolation or may be associated with significant concomitant morbidity, including but not limited to pain, pneumonia, or pneumothorax. In the palliative care population, rib fractures can be overlooked or attributed to other entities, which may lead to inappropriate treatment. The commonly accepted standards of care for the treatment of rib fractures are centered around early and adequate pain control, and stabilization of other complications. Accurate diagnosis and management demand a thorough history an

In silico analysis of rib force distribution in postscapulothoracic arthrodesis model.

Scapulothoracic arthrodesis (STA) is carried out by fixing the scapula to thoracic ribs which in turn allows the patient suffering from Facioscapulohumeral Muscular Dystrophy to carry out shoulder-joint dependent activities of daily living. A biomechanical analysis of this procedure has not been conducted in the literature and, for the first time, this study investigates the finite element calculated glenohumeral-applied load distributions on ribs by creating a post-STA model. Three loading directions on the glenohumeral joint are designated: anterior-posterior, superior-inferior, and lateral-…
