Proximal Scaphoid Pole Reconstruction Utilizing Ipsilateral Proximal Hamate Autograft.

BACKGROUND: Proximal pole scaphoid fractures complicated by nonunion and osteonecrosis present a challenging scenario with few good reconstructive options, especially when the proximal fragment is small or fragmented1-4. The use of an ipsilateral proximal hamate osteochondral autograft has emerged as a promising technique for reconstructing the proximal scaphoid pole, with the potential to restore carpal mechanics while avoiding the morbidity associated with free vascularized bone grafts or distant autograft sites. DESCRIPTION: The procedure involves replacing the diseased proximal scaphoid w

Case report: False negative 16 detector multislice CT for scaphoid fracture.

We discuss a case of a 19-year-old man with scaphoid trauma. We describe the imaging findings on three sets of radiographs, bone scintigraphy, CT and MRI. CT failed to identify a scaphoid fracture, which was present on 6 week radiographs, MRI and scintigraphy. The case illustrates that despite multidetector technology, CT still relies upon cortical and or trabecular displacement to demonstrate fractures.

Bilateral Traumatic Scaphoid Fracture Managed Surgically with Headless Compression Screw: A Case Report with Review Literature.

INTRODUCTION: The most frequent carpal bone fractures are scaphoid fractures, which frequently affect young people, particularly athletes. These fractures can be difficult to identify, and if left untreated, they can cause problems, including non-union. Although they are uncommon, high-energy trauma can result in bilateral scaphoid fractures, and related injuries can make treatment more difficult. CASE REPORT: A 20-year-old male who had fallen on an outstretched hand from a tractor appeared with pain, edema, and deformity in both wrists and the left elbow. Upon examination, the scaphoid compr

Epidemiology of scaphoid fractures and non-unions: A systematic review.

BACKGROUND: The scaphoid is the most commonly fractured carpal bone in adults as well as in children. Previous studies have reported a wide range of fracture incidences. Scaphoid fractures and non-unions in children have been sparsely investigated. AIM: To perform a systematic review of the current literature on epidemiology of scaphoid fractures and non-unions in adults and children. METHODS: An electronic literature search was conducted investigating all studies in the literature published between January 1989 and June 23 2020. The systematic review following the PRISMA guidelines and sear

Scaphoid Fracture Detection and Localization Using Denoising Diffusion Models.

Background/Objectives: Scaphoid fractures are a common wrist injury, typically diagnosed and treated through X-ray imaging, a process that is often time-consuming. Among the various types of scaphoid fractures, occult and nondisplaced fractures pose significant diagnostic challenges due to their subtle appearance and variable bone density, complicating accurate identification via X-ray images. Therefore, creating a reliable assist diagnostic system based on deep learning for the scaphoid fracture detection and localization is critical. Methods: This study proposes a scaphoid fracture detection

Analysis of the kinematics of the scaphoid and lunate in the intact wrist joint.

In conclusion, this study combined three different technologies to simultaneously monitor scaphoid, lunate, and global wrist motion in three dimensions and concurrently collect data on the pressure distribution in the radiocarpal and ulnocarpal joints. This information was collected dynamically in real time while the wrist was moved in reproducible, physiologic cycles of motion. The scaphoid and lunate flex and extend as well as pronate and supinate while the wrist moves in the plane of flexion and extension. There is minimal radial and ulnar deviation of these carpal bones during this motion.

[Post-traumatic cyst of the scaphoid--a case report].

Carpal bone cysts are very uncommon and they are usually occasionally diagnosed during radiological examination of the wrist after trauma. Some carpal bone cysts are symptomatic and then they are revealed on routine X-ray investigation. We present a case of 14 years old boy, in whom a scaphoid bone cyst developed around the foreign body (a splinter), stacked into the wrist when he fell on the wooden board. Because of pain, swelling and reduction of active range of motion of the wrist occurred at the load of the hand and lasted 3 months, patient was given radiological examination which was nega…

Bone malalignment in acute injuries of the wrist.

The purpose of this study was to analyze radiographic signs of carpal bone instability in patients with an acutely injured wrist. There were 80 patients (52 women and 28 men) with a fall on the outstretched hand. Fifty patients had a fracture of the distal radius, and eight had a scaphoid fracture. The patients with a bone fracture showed a larger scapholunate angle than those without a fracture (P less than 0.001, t-test). However, only four of them showed true carpal bone instability: two patients with a scaphoid fracture and one with a radius fracture had dorsiflexion instability of the wri

Topographical Analysis of the Hamate for Proximal Pole Scaphoid Nonunion Reconstruction.

PURPOSE: To quantify the similarity of the surface topography of the proximal hamate and proximal pole of the scaphoid for nonunion reconstruction. METHODS: Using previously acquired computed tomographic scans of the wrist of 10 patients, the 2 bones were segmented and subsequently aligned using both a manual and automated technique. Surface error between corresponding articular surfaces was computed to determine the similarity of the shape of the 2 bones. RESULTS: The median distance between the 2 articulating surfaces for each patient was 1 mm or less for all cases. Maximum distance varied

Trans-carpal injuries associated with distal radial fractures in children: a series of three cases.

Isolated trans-carpal injuries in children are rare. This paper presents three cases of trans-carpal injury associated with an ipsilateral distal radial fracture. Two cases involved simultaneous fractures of the scaphoid and the capitate. The third case involved the scaphoid and the triquetrum. The full extent of these injuries was not recognized on the initial X-rays. If a child presents with a displaced fracture of one carpal bone it may not be an isolated injury.

Ambulatory Free Medial Femoral Condyle and Trochlea Flaps for Management of Scaphoid Fractures and Nonunions.

BACKGROUND: Fractures of the scaphoid are the most common carpal bone fracture and have high rates of nonunion. Management of nonunions is crucial to preventing the predictable pattern of arthritis seen with scaphoid nonunion advanced collapse (SNAC). The medial femoral condyle (MFC) and trochlea (MFT) flaps have become a popular option for the management of recalcitrant nonunions. We analyzed our experience with outpatient free vascuarlized MFC/MFT flaps for management of scaphoid fractures and nonunions. METHODS: A retrospective review of a 2-surgeon approach to outpatient MFC/MFT flap reco

Navigated scaphoid screw placement using customized scaphoid splint: an anatomical study.

INTRODUCTION: Accuracy in navigated surgery depends on placement and registration of stable reference markers close to the anatomic region of interest. Navigation in small and complex anatomic regions, such as the scaphoid, is challenging due to difficulties in placing a stable reference marker. In the current paper, we describe the use of a customized wrist-positioning device "Scaph-splint" with a built-in reference marker that facilitates navigated scaphoid screw insertion in combination with a 3D imaging device. MATERIALS AND METHODS: Initial cadaveric feasibility study Five fresh-frozen c

Experimental Study and Preliminary Clinical Application of Mini-invasive Percutaneous Internal Screw Fixation for Scaphoid Fracture under the Guidance of a 3D-printed Guide Plate.

This study explored the feasibility of employing computer-aided design (CAD) and 3 dimensional (3D)-printed personalized guide plate for the mini-invasive percutaneous internal screw fixation of fractured scaphoid. The study consisted of two parts: (1) experimentation on upper limbs from corpses and (2) preliminary clinical application. Corpse experiments involved upper limbs of 6 adult corpses. The specimens of upper limbs were subjected to plain CT scan. Then the CT data were input into computer to conduct 3D reconstruction of wrist region. The direction and depth of the guide wire and screw

Scaphoid proximal pole ejection in perilunate injury: a case report and pathophysiological hypothesis.

Isolated ejection of the scaphoid proximal pole in perilunate injuries is rare, with only 4 reports in the literature, and does not correspond to the biomechanical situations in the Herzberg or Mayfield classifications. Bone ejection incurs a risk of avascular necrosis despite good osteosynthesis, notably because of precarious vascularization. We present a case of scaphoid proximal pole ejection and a theoretical hypothesis of the biomechanics of this injury. LEVEL OF EVIDENCE: 4.

Excessive Derotational K-Wire Angulation Decreases Compression by Headless Compression Screw.

Background  Scaphoid fracture is the most common carpal bone fracture. Open reduction internal fixation of scaphoid fractures typically undergo stabilization by a single headless compression screw (HCS). During screw insertion, a derotational Kirschner wire (K-wire) is often placed for rotational control of the near and far fragment. Questions/Purposes  The aim of this study was to determine if there is an angle of derotational K-wire placement in relation to the axis of a HCS that compromises the amount of compression generated at a fracture site by the HCS. We hypothesize that increased off-

Delayed Idiopathic Hardware-Associated Osteomyelitis of the Scaphoid.

Osteomyelitis of the carpal bones is rare and usually occurs in the setting of penetrating wounds or immune compromise. A case of hardware-associated scaphoid osteomyelitis is reported in a 33-year-old man who underwent screw fixation of a scaphoid fracture more than 20 years prior to presentation. The patient was treated with operative debridement including a total scaphoidectomy as well as intravenous antibiotics. Methicillin-sensitive Staphylococcus aureus was isolated from intraoperative cultures. A secondary proximal row carpectomy was performed to provide a stable wrist. Delayed presenta
