BACKGROUND AND PURPOSE: The purpose of the study was to determine whether the use of a toe spreader to inhibit the tonic toe flexion reflex (TTFR) immediately alters temporal-distance gait characteristics, plantar surface contact, or muscle activity in the limb exhibiting the TTFR of subjects with hemiparesis secondary to supraspinal lesion.
SUBJECTS: Eighteen adults with hemiparesis secondary to supraspinal lesions served as subjects for the standing portion of the study. Sixteen of the subjects participated in the gait portion of the study.
METHODS: The study was a randomized, within-subje

Longitudinal plantar approaches are generally considered at risk for wound healing problems. Thus, we wanted to investigate long-term outcomes after a primary Morton's neuroma excision through a longitudinal plantar approach. A retrospective study of patients with primary neuroma excision was conducted. Twenty-four patients (28 feet) were evaluated at a mean 9-year follow-up (range, 6-14) by a single trained examiner using a specific postoperative evaluation protocol, including AOFAS Forefoot subjective and objective scores. Good-to-excellent outcomes were reported in 25 (89.3%) cases. A hyper

The purpose of this study was to analyze radiographic outcome and patient satisfaction in non-operative care of hallux rigidus. Twenty-two patients representing 24 feet were surveyed and radiographed. Average follow-up was 14.4 years (range, 12-19 years). In 75% (18/24) of the feet, the patients would "still chose not to have surgery" if they had to make the decision again. The pain remained about the same in 22 feet, improved with time in one, and became worse in one. The most common reason given for not having surgery was that the pain was not severe enough. The most common type of self-care

The purpose of this study was to describe the use of skin temperature assessment in diabetics with acute Charcot's arthropathy to monitor resolution of inflammation longitudinally throughout the course of treatment and to predict development of neuropathic ulcers. Thirty-nine diabetic subjects presenting with acute Charcot's arthropathy received thermometric monitoring throughout their treatment course. Subjects were treated with a standard protocol involving total contact casting, removable cast walkers, and progression to therapeutic shoes. There was a steady decrease in temperatures during …

Travellers to tropical countries are often extremely concerned about the risk of bites and stings by venomous animals. This fear prompts many enquiries, usually at the last moment before departure, about the possibility of carrying first aid kits and antivenoms. In fact, these accidents are extremely rare because most travellers wear shoes and are far less exposed to venomous animals than indigenous peoples for whom bites and stings may be important causes of death or morbidity.

Iliotibial band syndrome was diagnosed in 48 of 1030 runners treated for lower extremity musculoskeletal complaints. Most athletes had been running 20 to 40 miles a week for one year or longer and had significantly changed their distance, speed, terrain, surface, and/or shoes before the onset of symptoms. Symptoms often persisted for 2 to 6 months. All runners were treated conservatively with rest, stretching, reduced distance, anti-inflammatory medications, local cortisone injections and/or orthoses.

Differentiating individual foot types has decisive advantages in diagnosing and treating fallen arches and splayfoot. The basic methods of classifying different foot types are discussed and the necessity of such classifying is shown. Seemingly different interpretations of foot malformations are thereby explicable. Reference is made to the problem of therapeutic failures resulting from non-observance of the interaction between the arch-support and the shoes worn. The positive effects of indicated alterations in heel height are demonstrated.

One of the most important fundamental cornerstones in treating diabetic foot ulcers involves offloading plantar pressure. Many dressings and devices have been developed with variable success rates secondary to design and patient adherence and tolerance of the designs. The current gold standard involves use of total contact casting or an irremovable cast walker. Following successful healing of a diabetic foot ulcer, devices such as custom orthotics and shoes are essential to reduce plantar pressures implicated in these wounds' recurrence.

Footwear dermatitis is a disabling condition that is identified by blistering and/or weeping feet that may become secondarily infected and painful. Fissuring is a common feature, resulting in an inability to walk, and it may prevent the wearing of footwear, especially when shoes are the cause of the dermatosis. In the absence of adequate care and therapy, footwear dermatitis may turn into a chronic distressing disease that largely effects the daily quality of life of patients.

Allergic contact dermatitis accounts for up to 20% of all dermatitis in childhood. The major sources of contact allergy in childhood are metals, shoes, preservatives, and plants. There is considerable variability from region to region and country to country in the prevalence and causes of contact dermatitis in childhood. We suggest that patch testing be done more frequently in evaluating dermatitis in children. Topical or systemic glucocorticosteroids and allergen avoidance are the mainstays of therapy in childhood.

The increase in strenuous sports participation among adult Americans has led to a high incidence of overuse syndromes. Common running-related problems include iliotibial tract tendinitis, chondromalacia patellae, "shinsplints," stress fractures and various heel and foot syndromes. Most causes of overuse syndromes can be traced to training errors, anatomic factors, poor shoes and uneven running surfaces. These syndromes generally respond to modification of the exercise program, anti-inflammatory drugs and orthotic correction of lower extremity malalignment.

INTRODUCTION: Off-road activities such as trail running and hiking have undergone a significant surge in global popularity over the last 2 decades, but this engagement also raises the potential for musculoskeletal injuries, which are prevalent and comparable to rates reported in road running. Footwear is critical in these dynamic environments, incorporating features such as aggressive outsole patterns and structural stability elements to accommodate varied and rugged terrain.
METHODS: Given the limited synthesis and fragmented nature of the literature, this scoping review systematically mappe

Prevention of diabetic foot ulcers is crucial, given the high risk of ulcer development (34%) and recurrence (> 40%), with up to 85% of amputations being preceded by ulceration. The most effective preventive intervention is the use of therapeutic footwear combined with custom-made insoles, which should be considered a true therapeutic strategy, comparable to metabolic control. Scientific evidence demonstrates that therapeutic footwear, particularly rigid-soled models with multilayer total-contact insoles, significantly reduces the risk of ulcer recurrence, provided that specific plantar pressu

A main challenge in human walking is maintaining stability. One strategy to balance the whole body dynamically is to direct the ground reaction forces toward a point above the center of mass, called virtual pivot point (VPP). This strategy could be observed in various experimental studies for human and animal gait. A VPP was also observed when VPP input variables like center of mass or ground reaction forces were perturbed. In this study, the kinetic and kinematic consequences of a center of pressure manipulation and the influence on the VPP are investigated. Thus, eleven participants walked w

Tarsometatarsal joint injuries can be painful and debilitating and are most commonly due to direct or indirect trauma. Posttraumatic arthritis is a well-known long-term complication, with incidence as high as 58%. Conservative treatment options include shoe modifications, orthotic inserts, topical or oral anti-inflammatories, and intra-articular corticosteroid injections. There are various joint prep and fixation techniques reported in the literature, many with positive clinical and radiographic outcomes. This article discusses nonoperative and operative management of posttraumatic tarsometata

UNLABELLED: Return-to-run protocols following lower extremity injuries commonly include a gradual training and loading progression to allow positive adaptation of musculoskeletal tissue. Before full return to running, training drills designed to provide a low to high external loading progression (i.e., vertical forces) may therefore minimize the risks of re-injury. However, the magnitude of external loading among various training drills has not yet been quantified.
OBJECTIVES: The purpose of this study was to quantify peak vertical forces of multiple common training drills and rank each of th
