With more competitive and recreational athletic pursuits now being undertaken by an aging population, the painful shoulder is seen frequently in the weekend athlete. Episodic stresses to soft-tissue restraints and musculotendinous units around the shoulder lead to a variety of overused pathologic entities. The two most common shoulder problems in the weekend athlete are irritation, swelling, and mechanical impingement; and tearing of the rotator cuff with traumatic shoulder instability (typically of an anterior direction).

BACKGROUND: American football quarterbacks (QBs) often sustain shoulder and elbow injuries from throwing. During the late cocking phase of pitching, decreased shoulder abduction and horizontal adduction angles at shoulder maximum external rotation (MER) contribute to these injuries. The relationship between incorrect shoulder positions at MER and the trunk and pelvic rotation angles at stride foot contact (SFC) during the early cocking phase has been analyzed. However, most studies focus on baseball pitchers rather than QBs. The relationship between shoulder kinematics at MER and trunk and pel

Baseball and softball injuries can be a result of both acute and overuse injuries. Soft tissue injuries include contusions, abrasions, and lacerations. Return to play is allowed when risk of further injury is minimized. Common shoulder injuries include those to the rotator cuff, biceps tendon, and glenoid labrum. Elbow injuries are common in baseball and softball and include medial epicondylitis, ulnar collateral ligament injury, and osteochondritis dissecans. Typically conservative treatment with relative rest, medication, and a rehabilitation program will allow return to play. Surgical inter…

In the injured athlete, several criteria must be considered before return to play. There are separate considerations for knee injuries, shoulder injuries, and general upper and lower extremity injuries, as well as concussion, hypertrophic cardiomyopathy, mononucleosis, and spondylolysis. It is important that surgical indications, postoperative rehabilitation, and risk for reinjury are reviewed by the surgeon before the athlete is allowed to resume activity.

The adolescent athlete with an open epiphyseal plate presents with different shoulder problems than does the adult. Most shoulder injuries in the adolescent athlete can be treated conservatively with a good prognosis for full return of function. The exception is a glenohumeral dislocation, which will probably recur and need a surgical reconstruction.

Heavy-weight judokas suffered a remarkably low number of elbow injuries, with more knee and shoulder injuries. Light-weight judokas were more prone to elbow injuries.
CONCLUSIONS: We found there was a low injury rate in top-level competitors, with a greater frequency of elbow injuries in female judokas. During the 15 years of injury collection data, an inj …

The purpose of this investigation was to describe the training history and injury experience of the competitive skier with a disability. A retrospective survey was administered to 68 athletes who participated in the National Handicapped Sports and the United States Association for Blind Athletes Winter National Games held in March 1989. The frequency, length, and specific components of the practice session were obtained. The definition of injury was any trauma to the participant that occurred during any practice, training, or competitive session that resulted in the cessation, limitation, or m

Dystocia of the shoulder is an unpredictable obstetric emergency that may result in injury to the mother or fetus. In an effort to reduce such risks, attempts have been made to identify patients having a fetus who may subsequently develop shoulder dystocia. The literature, however, clearly reflects that even the combination of prenatal historic facts, estimated fetal weight and sequence of intrapartum events is ineffective in prospectively identifying infants whose births are complicated by shoulder dystocia. During a ten year period at the University of Mississippi Medical Center, the inciden

In the daily practice of family medicine, injuries to the upper extremity are frequently encountered. Most of these injuries can by easily treated by the primary care physician who has an understanding of the joint anatomy and treatment principles. Some injuries, however, may appear relatively minor, yet require prompt referral for surgical care. We reviewed the pathoanatomy, historical and physical examination findings, and treatment of the more common injuries to the hand, wrist, elbow, and shoulder.

Coracoid process fractures are rare injuries, usually associated with high-energy trauma and disruption of the superior shoulder suspensory … suture fixation. This case highlights the importance of recognising occult instability in uncommon shoulder injuries, the role of advanced imaging in operative planning, and the need to restore both bony and ligamentous continuity to re-establish shoulder girdle stability.

Rotator cuff tendon tears are among the most common soft tissue injuries that occur at the shoulder. Despite advancements in surgical repair techniques, rotator cuff repairs experience a high rate of failure. The common occurrence of tears and the frequency of re-tears require a further understanding of the mechanisms associated with injuries, healing, and regeneration of the rotator cuff. This paper reviews in vivo studies using the various animal shoulder models of the rat, rabbit, sheep, canine, and primate. These animal models have been used to study intrinsic and extrinsic factors leading

OBJECTIVE: To evaluate the functional outcome of displaced acromioclavicular (AC) injuries in polytrauma patients.
DESIGN: A matched, case-control study using a prospectively gathered trauma database.
METHODS: Twelve polytrauma patients with a concomitant displaced acromioclavicular joint injury were matched to polytrauma patients without an acromioclavicular joint injury and to patients with an isolated displaced acromioclavicular joint injury. They were evaluated with a series of five shoulder questionnaires and the SF-36 general health status questionnaire.
RESULTS: Shoulder function in

Repetitive overhead throwing may result in overuse injuries and a change in the pitching mechanics of a baseball pitcher. Accordingly, the purpose of this study is to quantify the changes in the muscle strength and pitching motion kinematics in the late-innings stage of a baseball game. Sixteen healthy baseball pitchers (16.77 ± 0.73 years) recruited from a high school, which won the National High School Baseball Championship in Taiwan in 2011; each performed 100 pitches in a bullpen throwing session. Isometric muscle strength measurements and joint kinematic data were obtained before and afte

Isolated injury to the subscapularis tendon is an uncommon - but potentially debilitating - injury among athletes. The most frequent injury pattern seen in younger athletes is an acute, traumatic tendon avulsion. In the older athletic population, chronic degenerative tearing of the tendon may precede sports-related rupture, particularly following shoulder dislocation. This article reviews how to diagnose this injury in athletes, and describes both conservative and operative treatment options and their outcomes.

The armies of the North Atlantic Treaty Organization need a shoulder injury criterion for the EuroSID-2re dummy that must be reliable over a large range of loading conditions, from high velocity, short duration impacts (28 m/s - 3 ms) to low velocity long, duration impacts (4 m/s - 50 ms). In the literature, the human shoulder response to lateral impact was investigated at bounds of the loading condition spectrum as previously mentioned. For the low velocities, the injuries were mainly clavicle fractures and the maximum compression between the acromion and the sternum (Cmax) was proposed as an

Shoulder dislocations are an incredibly common orthopaedic injury. Although these injuries can affect patients of almost any age, those between 15 and 20 years of age have been identified as a population at risk. Aside from young age, male sex and participation in contact and overhead sports have also been identified as additional risk factors for these injuries. Despite the frequency with which shoulder dislocations occur, there is controversy regarding the optimal treatment algorithm following a first-time shoulder dislocation. Historically, patients were almost always treated with short-ter

BACKGROUND: Acute or recurrent anterior shoulder instability is a frequent injury for in-season athletes. Treatment options for this injury include shoulder immobilization, rehabilitation, and shoulder stabilization surgery.
PURPOSE: To determine if in-season athletes can be returned to their sports quickly and effectively after nonoperative treatment for an anterior instability episode.
METHODS: Over a 2-year period, 30 athletes matched the inclusion criteria for this study. Nineteen athletes had experienced anterior dislocations, and 11 had experienced subluxations. All were treated with p

Shoulder dystocia is a birth emergency that occurs in approximately 1% of all births. Shoulder dystocia can be followed by broken clavicle or humerus, brachial plexus injury, fetal hypoxia, or death. Although risk factors for shoulder dystocia include previous birth complicated by shoulder dystocia, maternal obesity, excessive prenatal weight gain, fetal macrosomia, gestational diabetes, and instrumental delivery, shoulder dystocia is not predictable. Perinatal nurses can reduce the risk for shoulder dystocia by teaching mothers about optimal weight gain in pregnancy and assisting mothers with

INTRODUCTION: This study sought to characterize the mechanisms of injury responsible for common softball-related injuries in recent years.
METHODS: The National Electronic Injury Surveillance System database was queried for all pediatric softball-related injuries presenting to participating US emergency departments from 2010 to 2019. Patients were classified by age as children (7-12 years), adolescents (13-18 years), or young adults (19-21 years). The case narrative of each injury was used to establish the mechanism of injury: hit by bat, hit by ball, sliding into base, collision with another
